Please review the following to ensure the Bid Proposal includes the following items:

A. CONTRACT

    1. Construction Contract (Currently have Three Addendum / 2009-10-20)

    4. Construction Commencement Date will be defined in a Notice to Proceed.
    5. Payment & Performance Bonds - Article 11.4, Supplementary Conditions

    7. Insurance Certificates – See Article 11, Supplementary Conditions for all insurance
         coverage requirements:
            a. Builders Risk Insurance
            b. Owner's Protective Liability Coverage

    8. Permits... Provide an Allowance at Bid to purchase the building permit
    9. G.C. to provide Schedule of Values
    10. G.C. to provide List of Subcontractors / Suppliers
    11. G.C. to provide Construction Schedule for 180 days.
    12. G.C. to keep log or Schedule of Submittals
    13. Provide RFI's for written response.
    14. Testing Lab – Capital One to Retain Lab for Material & Testing
    15. Site Meetings/Application for Payment - Lien Releases. See Article 9 and A201 Exhibit
         A001 of Supplementary Conditions.
         a. Three (3) Original duly notarized Payment Applications shall be prepared
         b. Provide unconditional release of lien for partial payment from each major
             subcontractor and material supplier for monies requested in previous pay application.
    16. Mechanical, Electrical, Plumbing Inspections — Four scheduled site meetings for
         observations by the MEP Engineer. Give 7 days advanced notice.
    17. A complete set of Record Drawings shall remain on the jobsite at all times
    18. Substantial Completion – based on Notice to Proceed.

    19. City Final/ State Fire Marshal & Certificate of Completion/CO required for Substantial
        Completion.
    20. Project Close-out — see Specification Section 01700. GC to complete Annex-B for
        Completion and Gaurantee.
    21. General Contractor shall provide to Capital One and "As-Built" ALTA/ASCM Survey
        and "As-Built" Record Drawings with Close-Out documents.


B. SITE COORDINATION

    1. Contact Louisiana One Call and also obtain approval from DEQ before construction
        commencement.
    2. Install OxBlue Camera... approximately 3-weeks for delivery.
    3. Develop Phasing Plan... After review of the Construction Documents, the General
        Contractor shall propose a phasing plan to Capital One in effort to facilitate daily banking
        operations during the period of construction thru completion.
    4. Provide Temporary Enclosures and Fencing
    5. Coordinate with Utility Agency's for Permanent Service
    6. Provide Access to Site and construction office
    7. Provide Construction Office and Temporary Utilities
    8. Provide Site Telephone/Fax/ or Email
    9. Prepare site for Material Staging
    10. Project Signage — Provide "Coming Soon Sign".
    11. Contractor job signs can be no larger than 4' x 4'. Sub contractors are not allowed signs

C. CHANGES TO THE WORK

Changes to the work will be documented by the Architect with an (ASI) Architectural
Supplemental Instruction, and shall be priced immediately by the Contractor. General Contractor
shall furnish a signed and dated Proposal Request giving complete back-up information to include
breakdown for labor and materials for each sub trade.

Do not proceed with any work until the Proposal Request has been accepted! Proposal
Request shall be duly executed with signature and date by either the Architect, or Capital
One Project Manager before deemed accepted.

Upon acceptance by the Owner, the Architect will combine accepted Proposal Requests and issue
an AIA G701 Change Order for all parties signature. All Proposal Requests to be included in a
Change Order shall be supplemented with detailed back-up information for each Proposal
Request. Capital One will not process Change Orders without the appropriate back-up
information. General Contractor shall prepare and include only signed Change Orders in the AIA
G702 Application For Payment.

D. CONSTRUCTION ISSUES

            Ox Blue Camera:
            a. Order, pay for and obtain Ox Blue Camera from TAG (Information Advisory Group)
               upon contract execution.
            b. TAG website can be accessed through the following email address:
               www.infoadvisory.com . At the top of their web site you will see a tab labeled
               Capital One Camera. Click on that tab. Fill out form and submit. This. will send
               IAG your request for a project camera, initiate the ordering process and provide
               TAG's Procurement Department with contact information.
            c. Contact Jalayn at IAG: sales@infoadvisorv.com for pricing, billing, or shipping
               inquiries.

            d. Contact Ox Blue Corporation for installation and service information: Chandler
               McCormack, Ox Blue Corporation, 888-849-2583 toll free, 404-917-0200 voice.
            e. Ox Blue Camera will be installed and made operational at the same time construction
               trailer and generator are brought on-site.
            f. Camera will be maintained and remain operational until substantial completion.
            g. NO PAY APPLICATION WILL BE PROCESSED IF AN OX BLUE CAMERA
               IS NOT INSTALLED AND OPERATIONAL AT JOB SITE.

    2.Long Lead Items
      a. Steel Package — Discuss obtaining a letter from Capital One to pre-order the steel.
      b. Masonry — Confirm Split Faced CMU and all exterior finishes before placing order.
         Contact material supplier for delivery lead time.
      c. Cast Stone — Approximately 12 weeks depending on the manufacturer. Cast Stone
         manufacturer is to be member of the Cast Stone Institute.
      d. Vault and Bank Equipment — Contact Jim Chatelain with DieTech Bank Equipment
         Services to coordinate lead time and delivery (504) 833-5886.
      e. Carrier Roof Top Units
      f. Site Area Lighting to match shopping center standard — Contact the manufacturer for
         lead times and availability. Please have the subcontractor place orders early so that it
         does not become an issue.
      g. Temporary Drive-up Building — GC shall provide and have delivered the temporary
         drive-up building. Contact Tommie with National Modular Bank Buildings for
         delivery lead times. (972) 412-1200

    3. Construction Specifics

            Concrete Work
               Install 15 mil Stego vapor barrier under slab.
               Provide slab moisture test and ph content.
               Provide Concrete Slab F-number Testing
               Seal concrete slab as specified.

                Provide slab depression for vault, electrical conduit, and contour floor for recessed
            -
                floor drain in the restrooms.

            Materials and Finishes
               Absolutely NO Substitutions of materials and finishes: ceilings, carpets, paint. EIFS,
               roofing, brick, stone, etc. to be as specified.
               An on site mockup panel of all exterior finishes is required, and is to be approved
               before installation of any materials. Quality of installation will be reviewed and
               sealant colors will be selected from this mockup.

            Roofing
                Roofing subcontractor to be a certified Garland company installer.
            - Provide applicator's certification from Garland Company (see specifications
            - Furnish Garland Company Warranty documents.
            - Color of all materials is listed on Sheet A2.1

            Building
                Provide Flood Protection and Flood Barriers. All slab penetrations shall receive
                Flood Proofing.
                Exterior insulation to be unfaced as specified.
                See "Partition Types" on Sheet Al .1. Install slotted slip track at all top rails.
                Coordinate interior electrical and above ceiling observation with Kalmans Marshall
                Engineering. Coordinate trades effectively. Walls may be one-sided at time of
                observation.
                Seal all ceramic floor tile grout joints.
            - Locate floor drains as dimensioned on floor plan in restrooms
                Install greenboard on all walls in restrooms
            - Interior frames by Raco, or equal. No miter joints. Submittals required for approval.
                Interior doors are furnished by Ipik Door Company in Louisiana.
                Gypsum drywall products shall be manufactured by USG, Georgia Pacific, American
                Gypsum, or National Gypsum. Substitutions are not permitted.
                Carpet and Tile flooring in the Lobby/Teller area shall be installed before Teller
                Counters can be installed

            Millwork
                Teller Counters, Check Writing Stand, and Greeter stand shall be provided and
                installed by Capital One.
            - All other Millwork for temporary phasing and final installation shall be furnished and
                installed by General Contractor. GC shall coordinate construction, installation and
                final electrical connections at Teller Counters and other millwork requiring electrical
                or any other trades.
                Cabinet locks to be similar for all millwork except in the Branch Office Manager
                (BOM) / Head Teller.
            -   Provide legal size file holder in the drawers at the BOM / Head Teller area.

            Drive-Thru Teller
            - General Contractor to coordinate all trades relative to the installation and completion
                of the VAT systems as well as all associated teller operations.
               Exact location of night drop — coordinate with DieTech.
               Review type of deal drawer supplied by Diebold.
               Install drive-up teller counter as shown on Sheet 4/A6.1, and coordinate with the
               Diebold shop drawings.

            Vault
            - Coord with American Vault to locate the vault in the recessed slab area..
            - Install ceiling at 7'-10".
            - Light fixtures to be operable. Stay clear from safety deposit boxes.
            - Provide granite counter top covering half height boxes.
            - Electrical outlets above counter top to be accessible.

Vendors (Capital One)
      - General Contractor shall coordinate with all vendors. Ensure sub-contractors and
          relative trades are coordinated with respective vendors for proper installation of all
          components and systems for branch security, electrical, phone, and data
          requirements.
          Coord with NAVCO and HSM to ensure electrician is coordinated and j-boxes are
          located in proper locations.
          3 SI shall use empty conduits from electrical room to exit locations to pull low
          voltage wiring for Dye Pack systems.

ATM
                Confirm Opening in Equipment Room for ATM equipment to be a minimum 1-0"
                from wall.

Electrical
        - Support all 2'x 4' light fixtures on all four corners.
           Support all 2'x 4' light fixtures on all four corners.
        - Install greeter station floor outlets as located on Al .1.
           ATM Room lighting on 24-hour circuit.
        - Install floor outlet in Conference Room.
           Ensure all conduit and j-boxes are properly supported per NEC and specifications.
           BX and MC cable is not permitted
           Light fixture whips to originate from J-box (Do not daisy chain fixtures).
           Provide caddy clip support for fixture whips.
           Mount boxes at specified height (refer to Contract documents).
           Provide proper identification for above ceiling j-boxes per specifications (color code,
           circuit identification).
        -  Provide insulated throat connectors at all conduit terminations.

                Provide bushings at conduit turnout prior to pulling cables (low voltage wiring).
                Mount TVSS per manufacturer recommendation (panel extension).
                Provide underground conduit as specified — "Plasti-Bond" PVC coated steel from
                underground to above slab.
                Provide specified disconnects for rooftop equipment.
                Provide proper grounding and bonding of building electrical system.
            -   Contractor is to ensure all work is completed per specifications and all applicable
                codes.
            -   All electrical covers are to be ivory colored. Provide color coded dots for dedicated
                and emergency power.
                Schedule meeting with Kalmans Marshall Engineering for interior electrical and
                plumbing observations before walls are concealed.


Plumbing
            -   Site/Plumbing Rough-In:.. Schedule meeting with Kalmans Marshall Engineering for
                observation before backfilling trenches. Domestic water line to the building pad shall
                be pressurized and holding.
                Install Point-Of-Use (Insta Hot) hot water heater in the Mop Room, restrooms, and
                breakroom.
                Install water hammer arrestors at fixture banks per manufacturer's recommendations.
                Provide unions at backflow preventors as specified.
                Ensure cleanouts are installed at all locations specified or indicated on contract
                drawings.
                Provide specified no hub clamps (Husky, Mission).
                Do not install any PVC piping in return air plenum areas.
            -   Piping insulation is to be continuous through wall cavity (provide sleeves as
                specified).
                Support for piping is to be external of insulation (provide rigid insulation at
                supports).
                Provide factory molded fitting covers for pipe insulation as specified (1" and above).
                Do not use PVC fitting covers for factory molded insulation segments.
                Provide insulation at storm drain piping as specified (including drive thru overhang).
            -   Contractor is to ensure all work is completed per specifications and all applicable
                codes.
                Schedule meeting with Kalmans Marshall Engineering for interior electrical and
                plumbing observations before walls are concealed.

Mechanical
                General Contractor provides and installs the Building Management Control System
                (BMCS). See Section 15907 and 15911 of the specifications. Contact Brian Tarbert
                with TCS Basys Controls at (800) 288-9383, or (608) 836-9034 ext. 200.
            -   General Contractor is responsible for the HVAC Testing and Balancing. Please
                contact Wayne Schexnayder with Coastal Air Balancing at (504) 834-4537.

             Coordinate installation of ductwork with light fixtures in lobby.
             Coordinate all ducts with light fixtures.
             Provide internal duct lining for first 10' of supply and return.
             Provide external duct insulation for all ductwork including first 10' (minimum R-6
             installed value).
             Provide specified duct sealant, ensure all joints, screw penetrations are properly
             sealed.
             Straps supporting ductwork are to be installed internal of insulation as specified.
             Provide raised quadrant volume dampers with end bearings; ensure they are
             accessible.
             Provide proper support for rooftop condensing units per specifications (Thycurb).
             Ensure proper spacing between supports for rooftop condensing units.
             Provide piping penetrations through roof as specified in detail (refer to sheet M2.1,
             detail # 6).
             Provide counter flashing at roof curbs.
             Provide head pressure control, sight glass for air-cooled condensing units as
             specified.
             Insulate and support condensate drain piping as specified (refer to sheet M2.1, detail
             # 7).
             Contractor is to ensure all work is completed per specifications and all applicable
             codes.
             Schedule meeting with Kalmans Marshall Engineering for interior above ceiling
             HVAC and plumbing observations before ceiling grid is installed.
             Re-schedule meeting with Kalmans Marshall Engineering for final meeting to
             confirm correction of all items as identified on previous site visits and Observation
             Reports.
