Letter of Recommendation from Employer for Graduate Studies Computer Science

1. SUPPLEMENTARY ADMISSION DATA FORM
      Please fill out the attached Supplementary Admission Data form.

2. LETTERS OF RECOMMENDATION
      A minimum of two Letters of Recommendation are required for each application.
         As part of a pilot project for the fall 2011 session, graduate admissions office will contact the referee for the Computer Science
         graduate program on behalf of the applicant. Once an online application form is submitted along with the referee information

         an e-mail will be sent to the referee with detailed instructions (starting the week of January 10 , 2011). Applicants are NOT
         required to submit hardcopies of the recommendation form.

         It is extremely important that you provide a correct and verified referee e-mail address (on the online application
         form) as this would be the only form of communication between the admissions office and the referee.

         Although the two recommendations should be from academic referees – former professors or research
         supervisors familiar with applicant’s abilities, one recommendation may be substituted with a professional letter
         from a previous employer.

3. STATEMENT OF INTEREST
      You are required to submit a brief statement (about 500 words in length) that should address the following:
              your reasons for graduate study in Computer Science;
              the research interest(s) you may wish to pursue;
              how your previous studies and experience have prepared you for the program; and
              your career objectives and how this degree program relates to them.
                   Please note:        Your research interests will be examined by faculty to determine whether they
                                       are willing to serve as your supervisor.

4. CURRICULUM VITAE
      Please submit curriculum vitae detailing your education, work experience, technical skills, awards and
      publications along with your application.

5. APPLICATION CHECKLIST (# OF COPIES REQUIRED TO SUBMIT)
   Please submit all required items in one package.

         □ All Official Transcripts (2 official copies), Hard copies not required ONLY if an official transcript(s) from an Ontario university was
         requested through the online OUAC application form.
         □ English Proficiency Test Score (if applicable)
         □ Write “Computer Science” on the outside of your application package

         Important: Incomplete applications will NOT be processed. It is the responsibility of the applicant to ensure that
         all required documentation is submitted correctly and promptly.
