Please follow instructions for online enrollment and online payment, OR download and print the form for
the class you want and return it to the school (classroom or office) with your payment check. A limited
number of forms are in the school office for those who are unable to access the internet.

Classes are on a first come, first served basis
Minimum enrollment must be met for the classes to be held. Classes have limited enrollment.
PLEASE READ CAREFULLY: Enrollment check list:
The following items must be received no later than January 7, 2011:
PAY ON LINE OR PAY BY CHECK
Waivers (2 forms) and dismissal instructions** REQUIRED! SEE PTA WEBSITE.
Completed registration form and payment check payable to the program provider (from the flier of
the class you are enrolling in), n/a if you enroll online.
Return check, registration form, (*&waivers and dismissal instructions) to the school office or
classroom.
**NOTE: If your child was enrolled for the Fall session, your waivers are on file.
If this is your child’s FIRST TIME enrolling this school year, please download, print and complete
the waivers and dismissal instruction forms on our website: www.villageviewonline.org). These
forms are required for enrollment and are due with your registration!
Classes not meeting the minimum enrollment will be cancelled by January 7. WE WILL NOTIFY YOU
ONLY IF A CLASS IS CANCELLED. ***NO CONFIRMATION NOTICE WILL BE SENT ***:
If you are not contacted about a class being cancelled or full, consider your child enrolled and instruct
them to report to the lunch benches (or Kids’ Club) after school beginning January 12.
The after school class schedule for Wed. 1/12 – Wed. 3/2 is as follows:
• 12:50 pm enrolled children report to lunch tables for supervised snack/lunch time - (parents should
provide a snack or lunch).
• 1:10pm Program instructors will take roll at the lunch tables
• 1:20pm Classes start
• 2:20pm Classes end (except Lego Engineering which will end at 2:50pm)
Parents/care givers are responsible for picking up their child(ren) immediately after their class ends,
or having them report to Kids’ Club.
The location of each class will be posted in front of the school office on the first day of classes
(Wednesday, January 12).

After-School Enrichment Classes at Village View Elementary School
RELEASE AND WAIVER OF LIABILITY AND INDEMNITY AGREEMENT
IN CONSIDERATION of being permitted to utilize the facilities, services and classes provided as after school enrichment classes at
Village View Elementary School (or for my children to so participate) for any purpose, including, but not limited to observation or use
of facilities or equipment, or participation in any on site class affiliated with after class enrichment, the undersigned, for himself or
herself and such participating children and any personal representatives, heirs, and next of kin, hereby acknowledges, agrees and
represents that he or she has, or immediately upon entering or participating will, inspect and carefully consider such premises and
facilities or the affiliated class. It is further warranted that such entry into Village View Elementary School for observation or use of any
facilities or equipment or participation in such affiliated classs constitutes an acknowledgment that such premises and all facilities and
equipment thereon and such affiliated class have been inspected and carefully considered and that the undersigned finds and accepts
same as being safe and reasonably suited for the purpose of such observation, use or participation by the undersigned and such children.
The undersigned understands that OCEAN VIEW SCHOOL DISTRICT has no responsibility for their child upon dismissal of regular
school hours. The undersigned further understands that it is their responsibility to assure that their child is escorted and checked into
Village View Elementary School each day that their child is to participate in After School Enrichment Classes. The undersigned
understands that it is their responsibility to promptly pick up their child upon dismissal of their class, unless enrolled in Kids’ Club.
IN FURTHER CONSIDERATION OF BEING PERMITTED TO ENTER VILLAGE VIEW ELEMENTARY SCHOOL AND
ATTENDANCE AT ANY AFTER SCHOOL ENRICHMENT CLASSES FOR ANY PURPOSE INCLUDING, BUT NOT LIMITED
TO OBSERVATION OR USE OF FACILITIES OR EQUIPMENT, OR PARTICIPATION IN ANY ON-SITE CLASS AFFILIATED
WITH AFTER SCHOOL ENRICHMENT CLASSES, THE UNDERSIGNED HEREBY AGREES TO THE FOLLOWING:
1. THE UNDERSIGNED, ON HIS OR HER BEHALF AND BEHALF OF SUCH CHILDREN, HEREBY RELEASES, WAIVES,
DISCHARGES Ocean View School District and Village View Elementary School PTA, its directors, officers, volunteers, employees,
and agents (hereinafter referred to as “releasees”) from all liability to the undersigned or such children and all his personal
representatives, assigns, heirs, and next of kin for any loss or damage, and any claim or demands therefor on account of injury to the
person or property or resulting in death of the undersigned, whether caused by the negligence of the releasees or otherwise while the
undersigned or such children is in, upon, or about the premises or any facilities or equipment therein or participating in any class
affiliated with AFTER SCHOOL ENRICHMENT CLASSES.
2. THE UNDERSIGNED HEREBY AGREES TO INDEMNIFY AND SAVE AND HOLD HARMLESS the releasees and each of them
from any loss, liability, damage or cost they may incur due to the presence of the undersigned or such children in, upon or about Village
View Elementary School’s premises or in any way observing or using any facilities or equipment of OCEAN VIEW SCHOOL
DISTRICT or Village View Elementary School or participating in any class affiliated with AFTER SCHOOL ENRICHMENT
CLASSES whether caused by the negligence of the releasees or otherwise.
3. THE UNDERSIGNED HEREBY ASSUMES FULL RESPONSIBILITY FOR AND RISK OF BODILY INJURY, DEATH OR
PROPERTY DAMAGE to the undersigned or such children due to negligence of releasees or otherwise while in, about or upon the
premises of Village View Elementary School and/or while using the premises or any facilities or equipment thereon or participating in
any class affiliated with AFTER SCHOOL ENRICHMENT CLASSES.
THE UNDERSIGNED further expressly agrees that the foregoing RELEASE, WAIVER AND INDEMNITY AGREEMENT is
intended to be as broad and inclusive as is permitted by the law of the State of California and that if any portion thereof is held invalid, it
is agreed that the balance shall, notwithstanding, continue in full legal force and effect. THE UNDERSIGNED HAS READ AND
VOLUNTARILY SIGNS THE RELEASE AND WAIVER OF LIABILITY AND INDEMNITY AGREEMENT, and further agrees that
no oral representations, statements or inducement apart from the foregoing written agreement have been made. I HAVE READ THIS
RELEASE.

After School Enrichment Classes
DISMISSAL INSTRUCTIONS:

Children must be picked up promptly at 2:20pm (unless they attend Kids’ Club). (Lego Class
finishes at 2:50pm) A late fee may be charged by the instructor if you are more than 10
minutes late. If you know you will be late please call the school office 714-846-2801.

Kid’s Club Authorization

for my child to be signed out of Kids’ Club on session Wednesdays for the 2010-2011 school
year, in order for him/her to attend the after school program named above. My child will be
signed out by a PTA board member (Elected / Appointed PTA Board Members including Principal – Jodi
Novy, President – Rebecca Wallihan, etc.) and will be brought to the lunch tables for snack/lunch under
supervision. After school program instructors will escort children to their classrooms. When the
after school program concludes, my child will be escorted back to Kids’ Club by their instructor
and will be signed back in by their after school enrichment class instructor.

Permission to Walk or bike home
I, ________________________parent of the above named child hereby give him/her permission
to walk or bike home at the end of their after school enrichment class. I assume all risks and
understand no adult will accompany him/her. I accept full responsibility for my child and hereby
discharge, release and waive all responsibilities and liabilities of Village View PTA, Ocean View
School District and after school enrichment class providers.
