Report required?
The 2007 Preliminary Annual Report is required for
some schools, such as those on low outcomes
monitoring.
If the report is required for your school, you would
have been notified in a letter from NACCAS.
Consult this letter for any additional
requirements that are specific to your institution.

may find it beneficial to download and complete
the worksheets anyway.
Completing the Preliminary Annual Report will help
you determine your outcome rates before the
final report is due, allowing additional time for
you to take necessary steps if your rates are
below the minimum standard.
Step-by-Step Instructions
The following slides will take you step-by-step
through completing the Preliminary Annual
Report forms.
Each page gives you instructions for one step in
the process as well as shows you how using
pictures of the actual forms.
Make sure you have these documents:

• Cohort Grids 1, 2, and 3
• Enrollment lists (may not be required for your
institution – consult the letter you received from
NACCAS)

Preliminary Annual Report
Step 1:
Completion Rate
Cohort 1
Your completion rate is calculated using the Cohort 1 Grid.
Cohort 1 is all the students scheduled to graduate between January 1 st 2007
and December 31 st 2007.
Cohort 1 includes students who were originally scheduled to graduate in
another year, but whose graduation dates were changed to 2007 by an
amendment to the enrollment agreement (for example, a student whose
expected graduation date is changed by a documented leave of absence)
Cohort 1
Cohort 1 does not include:
• Students who were scheduled to graduate in another year but graduated in
2007 instead without a documented change to the enrollment agreement.
• Students who were originally scheduled to graduate in 2007, but then took a
leave of absence that changed their graduation year to 2008.

Start with your Cohort 1 Grid.
This is what a blank Cohort 1 Grid will look like.
You’ll need to fill out a separate Cohort 1 Grid for each program.

Cohort 2

Step 3:
Licensure Rate
Cohort 3
Your licensure rate is calculated using the Cohort 3 Grid.
Cohort 3 is all the students who took the licensing exam for the first time in
2007, no matter what year they graduated. If the licensing exam has multiple
parts, students in Cohort 3 must take the last part in 2007.
Cohort 3 does not include:
• Students who failed the last portion of the licensing exam in 2006, and then
re-took it in 2007.
• Students who took one part of the licensing exam in 2007 and took another
part of the exam in 2008.

For every student, include state examination reports for both
parts of the exam that show whether the students passed.
Providing the Proper Documentation
States vary in the amount of information available on the licensing examination
reports.
For example, some states may not list student names individually, but only list the total
number of students who passed or failed.
You must provide the most detailed report that is available in your state.
If the state reports do not list students individually, you must also include the same
type of back-up documentation that is accepted for verifying placement rate, such as:
• Written licensure tracking procedure
• Copies of student surveys
• Correspondence logs

3-Year Averages
3-year Averages
After completing Forms 1 and 2 for 2007, you may find that your outcome rates
are below the minimum standards. In that case, you may want to calculate
your 3-year average outcomes. Your 3-year average may pull your outcome
rates above the minimum standard.
You may use 3-year averages for your outcome rates if:
• Your outcomes for 2007 are below the minimum standard, and
• Each of your cohorts contain 100 students or less.
To calculate the average rates, you will need to know Items 1 – 7 from the
Annual Reports for the current year (2007) and the previous two years (2005
and 2006).
