3.1    INTERNAL SCHOLARSHIPS


       3.1.1   PRO VICE CHANCELLOR‟S INTERNATIONAL SCHOLAR
               3.1.1.1 The Scholarship is only valid for Non Malaysian Citizen and Full time
                        enrolled student Degree programme at Curtin Sarawak.
               3.1.1.2 Two 50% scholarship toward course fee only for the entire duration of
                        programme (3 -4 years depending on programme of study)

               3.1.1.4 All applications received must be stamped with date and signature of the
                        staff who received the application
               3.1.1.5 Applications must be completed with all the required documents
                        (a) Completed Application Form
                        (b) Parents‟ pay slip/J Forms/BE Forms/Pensioners Card or Statement.
                             Parents without this relevant document or are unemployed must
                             submit a letter certified by the Commissioner of Oath or any class A
                             government official
                        (c) Certified true copy of examination results
                        (d) Certified true copy of testimonial and certificates of extra-curricular
                             activities
               3.1.1.6 Eligibility criteria
                        (a) Non Malaysia Citizen
                        (b) Full time enrolled student Degree programme at Curtin Sarawak
                        (c) Excellent GCE A‟ Level results with 2B‟s or equivalent, CWA
                             Foundation 80% and above or UEC with 2A‟s and 2B‟s
                        (d) Current CSM students must have latest CWA 80% or CGPA 3 and
                             above
                        (e) Active involvement in extra curricular and community activities
                        (f) Family financial background

        (c) Prepare summary of budgeted allocation for PVC‟s scholarship and
            roll over amounts from previous semester
        (d) Arrange for Scholarship Selection Panel to meet and finalize
            scholarship recipients
        (e) Issue Scholarship Conditional Letter of Offer signed by the Dean with
            a photocopy to be attached to application form
        (f) Students will be informed to collect the Offer Letters by pop-up notice
            on the student server, notices around campus or e-mail and phone
            calls
        (g) Issue letters to unsuccessful applicants

 3.1.1.8 Conditions
          (a) Recipients will be briefed in person on the detailed conditions of
              the award
          (b) Offer letters can be signed immediately or be taken home for
              review by students but must be returned to Loans & Scholarships
              on date stated on the letter
          (c) Offer Letter and agreement must state that the student is to
              complete at least four (4) years of his/her studies at Curtin
              Sarawak for the Business or Engineering and Science program
              and in the event that the students fail to comply to the terms of
              agreement, the student shall reimburse the University the amount
              totaling the value of the scholarship
          (d) The award is conditional on the student accepting the following
              responsibilities to study diligently, to pass all examinations and
              have not been offered/ holding any scholarship or loan.
          (e) A student who withdraws from the course or program after the
              commencement of class but within the first four (4) weeks, shall
              reimburse the University 50% of the scholarship value made for
              the current semester following the entitlement stipulated in their
              offer letter, or student withdrawing from the course or program
              after the first four (4) weeks, shall reimburse the University 100%
              of the scholarship value made for the current semester following
              the entitlement stipulated in their offer letter
          (f) The student must not at any time during his/her study at Curtin
              Sarawak behave or act contrary to the rules and regulations of the
              country and the University. If he/she is involved in any undesirable
              activities, the scholarship will be withdrawn and disciplinary action
              will be instituted against him/her in accordance with the University
              Policies

                    (g) A refund form will be attached together with the offer letter for
                        students to request for refund of payment made prior to application
                        for the scholarship
                    (h) After signing of the offer, student will be given the original copy of
                        the offer letter and a copy for safe keeping in student‟s file.


        3.1.1.9 Refund Procedures
                  (a) The student must pay the tuition fees before or during registration
                      period. For a student who has made payment for tuition fees, the
                      student must raise the refund themselves
                  (b) A database of recipients verified by the Head of Student Services will
                      be forwarded to Financial Services for reference and records. This is
                      to monitor those students who had paid the tuition fees prior to the
                      offer and the percentage of scholarship offer from us
                  (c) The student must fill in the Refund Request form and forward to the
                      Cashier
                  (d) A full refund will be issued to those recipients receiving the
                      scholarship
                  (e) Students should receive their refunds in approximately two weeks
                      from Financial Services


3.1.2   DEAN‟S SCHOLAR
        3.1.2.1     Two 100% scholarships toward course fee only for the entire duration
                    of programme (3-4 years depending on programme of study)
        3.1.2.2     Current students in the foundation programme and students enrolled
                    into the Diploma programme are not eligible
        3.1.2.3     The Scholarship is only valid for students with the following criteria:
                    (a) Malaysian Citizen
                    (b) Full time enrolled student in a degree programme at CSM
                    (c) Excellent GCE A-Level results with 2 B‟s or equivalent, CWA
                        Foundation 80% and above, UEC – 2A‟a and 2 B‟s
                    (d) Family Financial background

               Semester 2 and roll over amounts from previous semester
           (d) Arrange for Scholarship Selection Panel to meet and finalize
               scholarship recipients
           (e) Issue Scholarship Conditional Letter of Offer signed by the Dean
               with a photocopy to be attached to application form
           (f) Students will be informed to collect the Offer Letters by pop-up

           (g) Students will be requested to forward actual results if initial
               application was made from forecast results
           (h) Issue letters to unsuccessful applicants
           (i) Appeals based on extra-ordinary cases may be submitted to the
               Scholarship Selection Panel. Otherwise, the decision of the Panel
               is final
 3.1.2.8   Conditions

3.1.3   CSM SCHOLARSHIP

        3.1.3.1     The Scholarship is offered twice a year during the semester and the

                    (a)   100% scholarship towards course fee only for the first year of
                          study
                    (b)   50% scholarship towards course fee only for the first year of
                          study
                    (c)   25% scholarships towards course fee only for the first year of
                          study
        3.1.3.3     All applications received must be stamped with date and signature of
                    the staff who received the application
        3.1.3.4     Applications must be completed with all the required documents
                      (a) Completed Application Form

3.1.4   SHELL-CURTIN SCHOLARSHIP
        3.1.4.1 The Scholarship is offered once a year in March for two recipients per
                  year ending in 2008 where 50% of the semester tuition fees are shared
                  by both Curtin and Shell.
                    (a) The balance amount will be paid by Shell to the student as living
                        allowance
                    (b) The total amount shall not be more than RM12, 500
        3.1.4.2     Other expenses are to be borne by the student and successful
                    applicants are not bonded to work in Shell after completion of the
                    course
        3.1.4.3     Criteria:
                    (a) STPM/A‟ Level results with at least 2A‟s and 2B‟s
                    (b) CSM Foundation results with CWA 80% and above
                    (c) Accepted to Curtin first year Engineering degree
                    (d) No siblings currently holding Shell Scholarship and not holding any
                        scholarships or loans including PTPTN
                    (e) For Sarawakian and Sabahan students
                    (f) Not more than 23 years old
                    (g) Excel in academic, co-curriculum activities and able to interact and
                        communicate with wide range of people
        3.1.4.4     Procedures
                    (a) Create a database with the following categories:

 3.1.4.8   Conditions

           (c) Students need to open Bank Bumiputera-Commerce for the
               purpose of payment where Shell will pay the 50% of the tuition
               fees to Curtin University and the remaining of the amount will be
               paid directly to the students themselves as the living allowances
           (d) The agreement must state the value e.g. (RM 12,500) of
               scholarship given per semester and the validity period
           (e) The student must complete the Bachelor of Engineering Program
               at Curtin University of Technology, Sarawak Campus and in the
               event the student fails to comply, shall reimburse the university the
               amount totaling the value of the scholarship disbursed
           (f) The award is conditional on the student accepting the following
               responsibilities to study diligently, and to pass all examinations

          (g) A student who withdraws from the course or program after the
              commencement of class but within the first four (4) weeks, shall
              reimburse the University 50% of the scholarship value made for
              the current semester following the entitlement stipulated in their
              offer letter, or student withdrawing from the course or program
              after the first four (4) weeks, shall reimburse the University 100%
              of the scholarship value made for the current semester following
              the entitlement stipulated in their offer letter
          (h) The student must not at any time during his/her study at Curtin
              Sarawak behave or act contrary to the rules and regulations of the
              country and the University. If he/she is involved in any undesirable
              activities, the scholarship will be withdrawn and disciplinary action
              will be instituted against him/her in accordance with the University
              Policies

 3.1.4.9 Refund Procedures
        (a) The student must pay the tuition fees before or during registration
            period. For a student who has made payment for tuition fees, a
            refund request must be raised for the student
        (b) A database of recipients verified by the Head of Student Services will
            be forwarded to Financial Services for reference and records. This is
            to monitor those students who had paid the tuition fees prior to the
            offer
        (c) The student must fill in the Refund Request form and forward to the
            Cashier

3.1.5   WESTERN AUSTRALIAN GOVERNMENT SCHOLARSHIP
        3.1.5.1 The scholarship is only for CSM‟s existing students who will be going for
                their the final year
        3.1.5.2 The scholarship includes return airfares, traveling expenses, on-campus
                accommodation sponsored by the Western Australia government
        3.1.5.3 Curtin Bentley, Perth will grant the tuition fees for one year, the Overseas
                Student Health Coverage, the visa as well as the medical report fee,
                which will is first paid by the students and subsequently to be refunded
                upon arrival in Curtin Perth
        3.1.5.4 Other expenses such as textbooks, living expenses will be borne by the
                students themselves
        3.1.5.5 The Scholarship is only offered to two Business students, two
                Engineering students and the final candidate from any school
        3.1.5.6 The agreement between Curtin and the Western Australian government
                have been extended to 2009 till further notice
        3.1.5.7 Criteria:
                (a) Malaysian citizen
                (b) At least CWA of 80%
        3.1.5.8 Procedures
                (a) Create a database with the following categories:
                    i.      applicant‟s reference number

        (c) Arrange for Scholarship Selection Panel to interview short listed

        (d) Offer letters, Acceptance of Offer, Visa and Overseas Student Health
            Coverage forms will be issued by Curtin Perth
        (e) The next best candidate will be selected if the offer is rejected
        (f) The decision of the Panel is final
 3.1.5.9 Conditions
          (a) Recipients will be briefed in person on the detailed conditions of
              the award
          (b) Offer letters can be signed immediately or be taken home for
              review by students but must be returned to Student Affairs on date
              stated on the letter
          (c) Submitted documents will be faxed to Perth, while a copy will be
              made for reference before the originals will be sent to Perth
          (d) Students will then be advised to apply for their visas and to
              proceed with their medical check through the University‟s
              appointed panel of doctors
          (e) Students must inform the Enrolment Office of the units they plan to
              enroll in for the coming semester
          (f) Reports of the medical check and the plan units to be enrolled in
              will be sent to Perth
          (g) Loans & Scholarships Office to notify Housing Officer in Perth,
              Andrew Lee; and Airport Reception Services, Ian Farnie of
              students‟ confirmed date of arrival
          (h) Loans & Scholarships Office will issue a memo to notify Financial
              Services that CSM will pay the tuition fee as well as the visa on
              behalf of Curtin Perth
          (i) Loans & Scholarships Office will book a return air-ticket (Economy

3.3   STUDENT EXCHANGE PROGRAMME
      3.3.1   The programme is offered twice a year:

3.4   PAT ADDISON AWARD
      3.4.1   Offered once a year usually in the middle of the semester one during CSM‟s
              week free for the top student in the School of Business and top student in the
              School of Engineering & Science
      3.4.2   Nomination and selection is carried out by the Directors of Schools
      3.4.3   The   Award    covers    return   air-fare   (economy       class)    and   on-campus
              accommodation
      3.4.4   The Loans & Scholarships Officer will liaise with Perth for the Letter of Offer,
              details of accommodation, contact person over at Curtin Perth, airport pick-up
              and details of the one week itinerary
      3.4.5   Award recipients will be requested to complete a short write –up of themselves
              for press release purposes prior to departure

      diploma program in institution need to get a completion letter from their previous
      institution and inform PTPTN that they have completed their diploma program.

3.5.1.3 APPLICATION PROCESS
   Before students intend to apply the PTPTN loan, we have to make sure he/she meets
    the current minimum University requirement in order to minimize number of rejection
    which will affect out institution grade.
   PTPTN forms are available from the Loans & Scholarships Office and the cost of RM
    6:00 which had to pay at Cashier Counter.

   Application need to be filled in 2B or BB pencils otherwise they will need to purchase
    a new form.
   Upon receiving the application and other documents, we must check to make sure
    the documents given are completed and forms are properly filled. To make sure that
    the IC/names/course code & name/ID/income tax number of parents/date of
    commencement of course/details of SPM result/qualification entry to degree/salary of
    parent (must fill in net amount).
   Create a database on list of student with details such as IC/ID/Course enrolled/
    names/Gender/BCB account number/telephone number/min. entry requirement. This
    name list in database should be group according to course enrolled.
   The documents must be send to PTPTN at least 5 days before submission due date
    to avoid institution marks being deducted which will affect our grades.
   All forms and other documents must be photocopy before being sent out.
   Documents send out are:
             OMR Application form (attached a cover letter and list of students according
              to course they are in and have to be put on top of all application.)
             1 copy of JPS letter for course approval.
             1 copy of PTPTN Course code
             PTPTN submission due date.
   Individual application send not through the institution will not be entertained.
   Upon received of the documents, PTPTN will inform in writing on the status of
    application as of;
      I.    Completed documents – will be brought up for committee meeting and board
            of Directors meeting for approval.
      II. Not completed - will not be considered.
     III. Not qualified applicants – will not be considered. This might be due to no
            course approval from JPS/still owing PTPTN or other reason stated by PTPTN


3.5.1.4 APPROVAL PROCESS
   For normal application send by institution, PTPTN will take approximately 21 days for
    the loan to be approved.

         Arus perdana will take only 14 days which is including upon PTPTN receive of the
          form, processing, signing of agreement and transferring /disbursement of loan to the
          student.
         Arus Perdana can only be when the institution can get about 150 student to apply for
          PTPTN then we can invite PTPTN to come over to Curtin to give instant checking
          and approval and at the own expenses otherwise if lower than 150, we will have to
          bear the expenses.
         Upon approval by board of Directors, PTPTN will send out the offer letter and
          agreement to Institution and together with the list of names of those being offered.
         PTPTN had given a schedule timetable on submission of students‟ agreement for
          Curtin University .Refer table as below.

         Failing to follow their submission date will have our institution marks deducted and
          will affect our grading.
         Upon received of the agreement, we will have to arrange for a date for the students
          to come for signing of the agreement.
         Loans & Scholarships must make sure to return the agreement before due date given
          in the PTPTN cover letter failing which will affect our institution grade.
         Before filling in the offer letter and agreement, the student must check through their
          the name/address/course program/bank account number/IC are correct. If the bank
          account number/address and IC are not correct, student will have to fill a form on
          changes of these particulars. See attachment on sample of form.
         Student must fill the agreement carefully and must not make mistake. All forms must
          be filled in with black/blue ball point pen and must be capital letters. Any mistake
          made, the student must make a straight line across the word/sentence and sign at
          the side of the word/sentence. Students are not allowed to use blanco/correction pen.

   Students‟ copy of offer letter can be kept by student. Copy for PTPTN and Surat
    Kebenaran 1, 2, 3 will be return to PTPTN. In the offer letter student will need to fill in
    their names/ course offered/ agreement date/account number/bank name. Refer
    attachment on how to fill offer letter and agreement.
   Students will need to pay at cashier counter RM 45.00 to buy LHDN stamp. RM
    40.00 for buying LHDN stamps and RM 5: 00 as service charge. The stamps can be
    bought from Post Office. 2 copies of student agreement have to go for stamping at
    LHDN.
   Before proceeding for stamping, a memo must be sent to Financial Services,
    attention to Ms Sara Wong to request for cash payment. The memo should be done
    as soon as all students are done with the signing of agreement. This is to avoid
    delaying of time due to the process of cash payment from the financial services which
    will affect our due date to return the documents.
   Payment will starts effective from the date of their offer letter and agreement and first
    disbursement will be on that current semester their loan approved.
   Maximum amount will be RM16,000 per year for Commerce and Arts degree and
    RM16,500 per year for Engineering and Science degree
   The loan only covers for the tuition fee only not other expenses


3.5.1.5 PAYMENT PROCESS
     st
   1 disbursement of payment to the student are done only after the agreement is
    successfully completed by student and accepted by PTPTN. For the following
    semester‟s payment, it will be made after the confirmation status is received from the
    institution (Curtin). The following status are as follows;
     I. Active and is still studying
    II. Pass previous semester with CGPA 2:00 and above
    III. Does not hold any other loan/scholarship
    IV. Any other status stated from time to time
   Every semester PTPTN will send a diskette with list of students under PTPTN loan
    for confirmation of their status.
   Upon receiving of the diskette (hardcopy) on the student list, Loans & Scholarships
    will have to get copy of the student‟s transcript from the examination department.

      As required by PTPTN, the results will then be calculated to get the CGPA of the
       student.
      Once the CGPA are obtained, then we can proceed to change the status of the
       student according to the code that had been given by PTPTN. (see attachment)
      Loans & Scholarships will deduct RM 100.00 from payment amount in the listing and
       this will acknowledge PTPTN to direct credit the RM 100.00 to students account as
       notification for students and that the balance of PTPTN payment is paid direct to
       Curtin account.
      Upon received of the payment from PTPTN, PTPTN will issue a list of names of
       student under each payment voucher. We have to contra check all the names in
       order to make sure that no students are left out for payment. Nevertheless refund to
       the student can only be done after we received listing of student names from Bank
       Bumiputra as this listing is the final listing on successful transaction.
      PTPTN had given a schedule for confirmation status and payment. Please refer table

      Any refund to the student must be done within 14 days from the date of payment .
       PTPTN has the right to take any appropriate action should they find out any
       institution that hold student monies without proper reason and PTPTN had the right to

    do auditing to the Institution at any time to make sure there is no misuse of PTPTN
    fund.
   All students who have been offered the PTPTN loan are requested to have an
    insurance . This is to ensure that the repayment of the said loan be continued by the
    insurance, should the student passed away or permanent disablement. PTPTN will
    decide on the insurance cover and payment premium will be deducted from the loan
    amount.
   Payment to student who go for leave of absence or postpone his course will be on
    hold upon reasons given by the institution. If a student goes for leave of
    absence/postponed his course after payment from PTPTN then the next payment will
    be on hold by PTPTN.
   PTPTN will hold any payment made to student if the exam result for previous
    semester is less than GPA 2.00.
   There is no postponement of payment to any student on any payment withhold from
    them.
   Student who transfer/withdrawn from an institution because the course had been
    stopped are allowed to appeal for continuation of loan but the level and course
    offered must be the same. Documents needed to forward to PTPTN are appeal letter
    from student that states reason for transfer of course, release letter from previous
    institution and acceptance of offer from the new institution.
   A student who was offered PTPTN loan in his previous institution for his previous
    course but has withdrawn/terminated can only apply for the loan after he settled the
    previous loan amount.
   A student taking PTPTN loan in his Diploma course and has successfully completed
    the course can apply for second application to PTPTN in degree course unless
    second application of PTPTN loan is of the same course which are not allowed by
    PTPTN.
   Appeal for extension on PTPTN loan will not be considered unless for reasons such
    as changes in course structure and health problem of a student.
   PTPTN loan will not cover for summer course.

           3.5.1.6 REPAYMENT PROCESS
                 PTPTN will charge a 3% interest on loan and administration cost and every year
                  based on balance of loan each month.
                 Repayment for PTPTN loan will start six (6) months after graduation or after a
                  student has secured a job, whichever is earlier. Payments can be made through
                  Lembaga Hasil Dalam Negeri(LHDN), Maybank2U, Bumiputera Commerce Bank,
                  Bank Islam or Post Office. Payment made to PTPTN must be by Bank Draft and paid
                  to Ketua Eksekutif, Perbadanan Tabung Pendidikan Tinggi Nasional. A student can
                  pay PTPTN the whole amount upon graduation, or any amount but not less than the
                  amount stated by PTPTN in their agreement. If a student settles the whole amount
                  within six months after graduation, there will be no interest incurred.
                 A student who appeal for deferment of payment or rescheduling for reasons such as
                  extension of duration of course/have not completed studies can write a letter to
                  PTPTN. The student must request a confirmation letter from the student services
                  department on his status.


3.5.2      YAYASAN SARAWAK LOAN
        3.5.2.1       Application received with all the relevant documents:
                     Sarawakian only
                     Identity card of Student
                     Students need to show the evidence of rejection (i.e a letter from PTPTN) that
                      they did not get the offer from PTPTN/get a support letter from head of student
                      services stating the student condition.
                     Full-time enrolled Foundation and Degree student of CSM
                     BM in SPM must be credit
                     Testimonial of the evidence of extra-curricular activities and socio-economics
                     Offer letter from Curtin
                     Not holding any scholarships or loans
                     The closing date is ongoing each year.
                     The form can be obtained from Loans & Scholarships or download from the
                      Yayasan website, Resident Office or by mail with the self-addressed envelope of
                      RM1 for Yayasan staff to post the form to the student later.

             The form must be sent together with all relevant documents and must be certify
              by class A government officials /Registrar/Head of Student Services.
3.5.2.2       The student need to submit the documents together with the application form to
              Yayasan directly as per mentioned in the application form.
3.5.2.3       Approval of application depends on the number of student apply for that
              particular year.
3.5.2.4       There is no conversion of loan to scholarship but this depends on the offer and
              agreement made between Yayasan and the student.
3.5.2.5       Beginning of a semester, Loans & Scholarships will send student transcript to
              Yayasan.
3.5.2.6       Payment to student will depend on the CGPA obtained. Please refer as follows;
                     2:00 pass and with payment.
                     1:8 – 1:9 Yayasan will pay but with a reminder letter
                     1:00 – 1:79 – Yayasan will pay but the student will have to produce a
                      show course letter.
                     1:00 – No payment at all unless there is improvement in the student
                      performances the next semester.
3.5.2.7       Payment of tuition fees are paid to Curtin account instead of student account.

3.5.3   SARAWAK HIGHER EDUCATION FUND CHARITABLE FUND CHARITABLE TRUST
        (YAYASAN SARAWAK HI-ED STUDY LOAN)


        3.5.3.1 Application received with all the relevant documents:
                       Sarawakian only
                       Full-time enrolled Foundation and Degree Engineering student of CSM
                       BM in SPM must be credit
                       Testimonial of the evidence of extra-curricular activities and socio-
                        economics
                       Offer letter from Curtin
                       Not holding any scholarships or loans
                       The closing date is ongoing. It will be advertised in the local newspaper
                        from time to time. (e.g Borneo Post). Loans & Scholarships will also do
                        the pop-up notice and notices around the campus to notify the students
                        to do the submission of form to us. The form can also be obtained from
                        Loans & Scholarships counter or download from the Yayasan website
                       The student needs to submit the documents together with the application
                        form to us directly before the due date.


        3.5.3.2 Once approved, Yayasan Hi-Ed will send us the statement of the amount each
                student received together with the agreement which the student needs to sign
                before the loan actual took effect and there is a dateline for submission which
                they will inform us when we received the agreements.


        3.5.3.3 If agreement received

                 We will arrange a date, time and venue to group the students to brief them on
                    the step-by-step to sign the agreement

                       Signature of sureties usually should be the parents, otherwise if the

                the sureties as they will act as a guarantor if the students failed to pay in
                future.
               Both the sureties need to provide photocopies of ICs and their salary
                slips.
               The letter of offer should be signed and sent back a copy to Yayasan,
                and another copy will be kept by the students.
               4 sets of agreements should be registered at the Income Tax
                Department for stamping by the students ( 2 set of agreements are for
                the sureties, 2 set for Yayasan and another one for student reference
                and safekeeping).
               However, all four set of agreements need to send back to Yayasan
               Form 3 (Reply Form) need to be filled and signed by the student and
                faxed to Yayasan
               Form 4 (Surety Form) also need to be filled by the sureties and gave to
                us o Form 5 (Letter of Authorization) must be filled and signed by both
                the student and a witness and gave to us
         We will send the agreement together with Form 4 and 5 to Yayasan before the
           due date


3.5.3.4 If loan approved
        a) Payment will be made:

        3.5.4.1 Application received with all the relevant documents:

                        Foundation/Matriculation need to be submitted together
                       Birth certificates of the applicants as well as the parents‟

                       Testimonial of the evidence of extra-curricular activities and

                       Not holding any scholarships or loans
                       Sponsored for one year only of RM3,000 only
                       The scholarship offered once a year in April and closed in July.

        3.5.4.2 The student need to submit the documents together with the application
                form to Tabung Amanah Bakun directly as per mentioned in the
                application form
        3.5.4.3 Once, been selected it is the responsibilities of the student to pay the
                tuition fee partly with the money given by them.


3.5.5   EPF WITHDRAWAL SCHEME FOR EDUCATION
        3.5.5.1 Application received with all the relevant documents:
                       EPF members and/ or EPF member‟s children
                       Full-time enrolled students of CSM
                       Offer letter from Curtin
                       Letter of confirmation of the course study, duration, course fees
                        and others.
                       Student need to liaise directly with nearest EPF office for further
                        details
                       Amount up to the maximum amount of total fees or the entire
                        balance in Account II, whichever is lower

        3.5.5.2 The student needs to submit the documents together with the application
                form directly to EPF office.
        3.5.5.3 Once, approved it is the responsibilities of the student to pay the tuition
                fee with the money given by them. If EPF not approved the withdrawal
                and it is due date for payment, then the student need to pay the tuition
                fee first and get the refund from us later in order to do enrolment.


3.5.6   THE SOCIAL SECURITY ORGANIZATION (SOSCO) EDUCATION FUND
        3.5.6.1 Application received     with all the relevant documents          with the
                requirements stated below:
                       Dependent children of an employee receiving invalidity Pension
                        or periodic permanent disablement who are below 21 years of
                        age and unmarried at the time of the application
                       Dependent children of an insured person who has died as a
                        result of an employment injury or while receiving invalidity
                        pension or before reaching age 55 and who fulfils the
                        contribution qualifying conditions. They should be below 21
                        years of age and unmarried.
                       Dependent children who are above 21 years of age, receiving a
                        monthly benefit and already studying in an institution of higher
                        learning and unmarried at the time of application.
                       Dependent children who have been offered places and have
                        registered in a university of college or Local Institution of Higher
                        Learning including institution that has twinning programme on
                        condition the degree is completed locally. They must be also
                        registered with the Ministry of Education, to acquire a degree,
                        diploma or a certificate which is awarded by the National
                        Vocational Training Council, Ministry of Human Resources.

                Dependent children whose families‟ finances are insufficient to
                 support the cost of the education
                The amount of the loan depends on the course undertaken, and
                 includes the cost of living and other expenditure as determined
                 for fund by SOCSO. The actual amount of the loan includes the
                 following:
                        Course fee
                        Other expenses which will be determined form time to
                         time are:
                                  Subsistence allowance
                                  Flight cost
                                  Books
                                  Instruments
                                  Expenditure for preparation of thesis
                                  Practical/training costs

 3.5.6.2 The student needs to submit the documents together with the application
         form directly to SOSCO office in KL which mentioned in our website or
         as per the application form.
 3.5.6.3 Once, approved it is the responsibilities of the student to pay the tuition
         fee with the money given by them.           If SOCSO not approved the
         withdrawal and it is due date for payment, then the student need to pay
         the tuition fee first and get the refund from us later in order to do
         enrolment.

    3.5.7   RHB BANK – RHB STUDY LOAN
            3.5.7.1 Application received with all the relevant documents:
                            Full-time enrolled undergraduate student and must be at CSM at
                             the point of applying.
                            The student can jointly apply with the parent or guardian /brother
                             or sister/spouse or children
                            Students need to liaise with RHB branch for further details or
                             look at their website for any updates.
                            The form can be obtained from either their bank or download
                             from their website
            3.5.7.2 The student need to submit the documents together with the application
                    form to RHB Bank
            3.5.7.3 The student need to submit the documents together with the application
                    form to RHB Bank directly as per mentioned in the application form
            3.5.7.4 Once, been selected the student need to show us the evidence of
                    approval and the student need to inform us or either Financial services
                    on the payment will be made to student and it is the responsibilities of
                    the student to make the payment.


3.5.8 MAJLIS AMANAH RAKYAT (MARA)
            3.5.8.1 Application received with all the relevant documents:
                        Offered to Malaysian - Bumiputera students under these category
                         only:
                        Students who have mixed parentage which either one is not
                         Malaysian are not categorized as Bumiputera for MARA
                        Only eligible for student enrolled in:
                                Chemical Engineering;
                                Either Electrical or Electronics & Communication Engineering
                                 (This we will need to write in for apology of such a mistake
                                 done by former party);
                                Computer Science; and
                                Accounting & Finance

             Pass with five credits which inclusive BM, Science, any Science
              subjects (Physics, Biology or Chemical), Mathematics or Additional
              Mathematics and any other subjects in SPM
             Matriculation certificate with CGPA 2 and above
             Diploma students who credit transfer to our Curtin without
              completion of Diploma certificate are eligible to apply as long as one
              have five credits in SPM as mentioned above
             STPM with 3 principle passes including General Paper or Diploma
              with CGPA 2 or equivalent qualification
             Student must achieve minimum academic performance of CGPA 2
              every semester for the duration of the study
             Full loan for students that parents‟ family income are less than
              RM50,000 annually
             Those above RM100,000 (Family income) are not eligible to apply
             The sponsorship are tuition fee, allowance for books, transportation
              for non-Sarawakian,
             They also can request for their first allowance before the
              subsistence allowances from the MARA office in Miri to be posted to
              Kuching branch for processing which cost of RM1,500
             Duration of loan and amount based on agreement
             We will open the application only in March and April and July and
              August since the only accept Letter of offer without conditional offer
             For further courses and amended tuition fee so that they can
              sponsor the students‟ tuition fee at all due to our new tuition fee, we
              will need to inform the Head of School to file the documentation
              again and write in for permission for approval through presentation
              in front of the selected committee
 3.5.8.2 The due date will be set by us due to they will receive the application the
         as a whole not individual
 3.5.8.3 The application, approval process and payment process roughly take
         three months depending on the smooth process of application as well as
         the agreement received

      3.5.8.4 Forms are available from Loans & Scholarships counter at Student
               Services either the original or photocopy
      3.5.8.5 We are yet to receive application from our students due the limited
               Bumiputera students in the approved courses by MARA
      3.5.8.6 Due to limitation of courses for such as loan, fewer students applied.
               Therefore, we are in the middle of negotiation and compromise for more
               courses given to the students.
      3.5.8.7 Students will need to pay them back a year after they graduate according
               to the statement posted to them for repayment.
      3.5.8.8 Students are not allowed to extend their courses which vary from the
               duration mentioned in their agreement. Any extra extension, the students
               will need to bear the cost by themselves.
      3.5.8.9 Summer course will not covered by MARA. If the students apply for
               Student Exchange to Curtin Perth, MARA might consider the payment
               since the tuition fee is applicable with Curtin Sarawak but is yet advised
               by the MARA officer in charge (En. Khaizan)


3.5.9 AGRO BANK INSANI STUDY LOAN
      3.5.9.1 Open to all Malaysians
      3.5.9.2 Certificate, Diploma, Advanced Diploma, Degree and Advance Degree
               Levels from local and international tertiary institutions that are accredited
               By Ministry of Higher Education, Malaysia.
      3.5.9.3 Loan amount (minimum RM1, 000 to RM100, 000 maximum)
      3.5.9.4 Eligibility:
                       Fresh school level who has just been accepted into institution of
                        higher learning (previously unemployed) will be eligible as co-
                        borrower
                       For co-borrower, should not be above 60 years of age on the
                        date loan matures
      3.5.9.5 Amount or duration of payment:
                       Up to 100% of total financial needs
                       Up to 10 years

              3.5.9.6 For more information, the student needs to liaise with the Agro Bank

3.6   STUDENT ACTIVITIES/EVENTS


      3.6.1   STUDENT COUNCIL
              3.6.1.1 Student council election and voting:
                               Usually fall in October each year
                               A meeting will be called upon to discuss both promotional, voting
                                and election period
                               Will either ask two of our staff to assist in the registration or ask
                                outsider to work part-time as electoral assistants
                               After 2 weeks, the students are not allowed to put the notices
                                and need to take it down. Specific location will be selected for
                                the notice display.


              3.6.1.2   During voting time
                               A meeting will be called upon to discuss on the voting period
                               Once the Returning Officer did the forms and notices, we will on
                                behalf distribute to the rest of the students.
                               Returning Officer and the rest of the committee will determine
                                the location and the set-up for the voting (usually at the Lecture
                                Theatre Foyer with barricades, registration tables, voting box and
                                three or four voting tables)
                               Assist Returning Officer, Assistant Returning Officer and Student
                                Advisor for the election
                               Assist in putting up the notices
                               Forms distribution (such as election form, voting form)
                               Collecting the RM10 from each student who hand in the election

                Check for the eligibility for election – no pending payments, good
                 standing and not leave of absence status
                Must not be final year students
                Forms sending in after 4pm on the last day not allowed
                A budget will be given for each group (each person only RM15),
                 a selected color will be determined for their promotional
                 materials, timeline for the voting is only 2 weeks
                All forms must be put in the box and locked and will be count at
                 the end of the day to avoid cheating
                Registration based according to student IDs or ICs which names
                 appeared on the electoral roll list.
                Students need to come by themselves for voting in person, no
                 taking on behalf allowed.
                Forms will be tampered to avoid student cheating


 3.6.1.3 During election time
                A meeting will be called upon to discuss on the election.
                A few staff to assist for counting on the counting day depending
                 on the numbers of forms received
                Counting and recounting will be done.
                Those that did not fill correctly as per instructions will be void
                Preferences based according to ticks or numbering, no repeating
                 numbers allowed
                The Returning officer announce who win and in what post
                The next day, the notices will be out to notify everybody who are
                 elected around the campus

 3.6.1.4 After Election

                There should be a proper handover by the previous president to
                 the new president

3.7   CERTIFICATES


      3.7.1   Any clubs or societies asking for printing certificates:
                     They will need to give us the details and names
                     We will check the spelling and the sequence of the names given
                     After that, we will ask the students to come and check before we sent to
                      DEAN for signature
                     Roughly, the students can come and collect the certificate in three to four
                      weeks time depending whether it is peak period. Otherwise, it will be
                      done in a day or two.
                     We will either e-mail or call the students up to collect the certificates.


              3.7.1.1 Dean‟s List Award:
                             Only given to top student in that unit in that class
                             Previously, it was chosen by the unit coordinator and passes
                              down to us the list of the successful students for certification.
                              Starting from semester 1, 2009, the list of top student will be
                              generated from the system given by Examination Unit to Loans &
                              Scholarships Office. Then Loans & Scholarships Office need to
                              distribute the generated list to the Administration Assistant for
                              each schools.
                             Formerly, it was only given to both Commerce and Engineering
                              degree.
                             Just this year, this semester, it was decided that Foundation
                              students will have Dean‟s list award from this semester onwards
                             The award is awarded twice a year which is very semester
                             They will need to give us the details and names
                             We will check the spelling and the sequence of the names given

       3.8.1   Any clubs or societies asking for permission to put up notices:
                      They will need to pass us the posters and we will help them to stamp or
                       vice versa


       3.8.2   Lecturers asking for permission to put up notices:
                      They will need to pass us the posters for verification
                      Then, we will on behalf stamp our department stamp




3.9    TESTIMONIALS


       3.9.1   Any students asking for testimonial:
                      They will need to pass us the details of their activities or participation in
                       the clubs at Curtin University
                      Then, we will write a testimonial and will pass to our Academic Registrar
                       for signatory
                      Later, we will call or e-mail the students for certificate collection




3.10   ORIENTATION

    During orientation week, we ask our Student Ambassador and Student
     council to act as a mentor to assist the new students on tour around
     Curtin, any enquiries regarding filling the form and etc
    Reconfirm the venue and time with the parties involved such as Student
     Council president; lecturers, Head of the school, Campus services, IT
     department, Student Services (International Office, Housing, Admission
     & Enrolment), DEAN, PVC, Campus Life Project and Student
     Recruitment Department.
    The Orientation kit will be ready for collection once they enrolled at the
     Admission and Enrolment counter (Counter 3 and 4 at Student Services
     Department).
    Either Student Recruitment staff; Student Service Staff will or Student
     Ambassador or Student Volunteer will brief the students on the
     orientation kit.
    Give information about loans and scholarships requirement and etc.

3.11   GROUP PERSONAL ACCIDENT (GPA)


       3.11.1 CLAIMS
             3.11.1.1 Student claims can only be done when the documents are in order:
                           Insurance claim form filled in by both the student and the doctor
                           Medical certificate (if unfit to attend class)
                           Original receipts
                           Photocopy of the student Ids, passport validity or ICs
                           No duplicate allowed otherwise clarification and support letter
                            from the University of the Student Status. Duplicates need to be
                            originals
                           Support letter from the University to notify them on the claims
                           Need the support letter from the club advisor or the president to
                            clarify if the injury caused by activities involved in the clubs or
                            societies
                           Need to fax to them to notify them on the claims while sending
                            the originals to them
                           Claims not more than RM1,500 for medical claims unless total
                            disability .
                           After that, a week or two, the Insurance company (SARATIM) will
                            send a cheque to us and we will give to Finance Services
                           Send an acknowledgement of receipt by fax then mail the
                            acknowledgement of the letter


       3.11.2 CREDIT/DEBIT NOTE
             3.11.2.1       After we inform the insurance company of the latest student
                            population coverage:
                                    The insurance company will send us a credit note if we
                                     overpay the insurance premium according to the student
                                     population given

                            student population increase than the number given
                            according to the policy statement, then we will need to
                            pay them whatever extra premium to cover the
                            insurance of our students

3.11.3 POLICY STATEMENT
      3.11.3.1 Student claims can only be done when we update the student
              population:
                           Need to write to Saratim Insurance, attention to Mdm
                            Voon to notify them on the latest student population
                           The student population need to be updated every
                            semester after the census date
                           Usually after the last orientation of the course (for e.g
                            first semester, the last orientation will be the Diploma
                            course, then the second semester will be our Intensive
                            English Program)
                           Fax to them the letter first, then followed by mailing them
                            the original copy to the Insurance company, and we will
                            keep the photocopy
                           The statement will consist of information such as the
                            expiry date for the insurance coverage, number of
                            students covered, types of medical claims, premium that
