Schedule A
The fees to be paid to SBC Advanced Solutions, Inc., Ameritech Advanced Data Services of Illinois, Inc., Ameritech Advanced Data
Services of Indiana, Inc., Ameritech Advanced Data Services of Michigan, Inc., Ameritech Advanced Data Services of Ohio, Inc.,
Ameritech Advanced Data Services of Wisconsin, Inc., (Seller) for Services provided to Illinois Bell Telephone Company, Indiana
Bell Telephone Company, Incorporated, Michigan Bell Telephone Company, The Ohio Bell Telephone Company, Wisconsin Bell,
Inc., Nevada Bell Telephone Company, Pacific Bell Telephone Company, and Southwestern Bell Telephone, L.P., (collectively as
12-State Regulated Telcos) (Buyer) will be as follows:
Section I. Scope of Services
1. T1 IAS Customer Premise Equipment (CPE) Installation: This covers nonrecurring labor cost of installing Adtran
TA850/TA624 or comparable Integrated Access Device (IAD) equipment. It includes the cost of ordering CPE and provisioning at
customer site. 2. Post-Installation Maintenance: This covers recurring cost of IAD maintenance, including 24X7 customer
phone support, defective part replacements and on-site repair/replacement. 3. Carrier Coordination: This covers recurring cost
of coordinating customer T1 service trouble reports with voice carrier. 4. Voice CPE Installation & Repair: This covers the
technician's hourly wage rate for voice CPE I&R. 5. Field Engineers - Installation Service (Standard Rate per Device) -
(Covers non-recurring cost of field engineer performing installation/testing of Cisco 26XX router on customer premises on behalf of
SBC Telco). 6. Network Monitoring Service (Rate per Device/Month): (Covers recurring monthly rate per router for
complete/high complexity remote network management and monitoring service performed on behalf of SBC Telco).
This Agreement relates only to non-regulated BUYER CPE and does not apply to the installation, maintenance or testing of network
elements or services provided by SELLER under any other agreement. SELLER will be obligated by this Agreement to provide CPE
qualified technicians. Qualifications will be determined by successful completion of the BUYER training curriculum (Addendum D)
for the supported Products identified in Addenda B and C.
Section II. Pricing
Service Rate Elements Rate
1. T1 IAS CPE Dispatch, Installation, Maintenance, & Testing $196
2. T1 IA Post-Installation Maintenance $1,572
3. T1 IAS Carrier Coordination included in 2.
4. Voice CPE I&R 59.46 per hr
5. Fleld Engineer - Installation Service $293.00
6. Network Monitoring Service $160.00
ANY ADDITIONAL DIRECT EXPENSES WILL BE PASSED THROUGH TO BUYER AS INCURRED.
Section III. Term
This pricing addendum will commence upon signature by both buyer and seller and will continue thereafter until
canceled in writing by either party, as provided in the General Service Agreement.
