4.1: Training in Priority Areas Undertaken during 01 April 2008 - 31 March 2009
Please describe the type of training programmes which you ran in your organisation during the reporting
period, in the categories listed below.

Note that this is an ESTIMATE of the percentage training offered in the various categories, using training
days to determine the breakdown.
You can work the percentage out in the following way:
For each category: No of persons who have undergone training x no. of days = sub-total person days
Sub-total 1 + sub-total 2 + sub-total 3 etc. = total person days = 100%
To work out % per category:
Sub-total 1 person days divided by total person days x 100% = % category 1
Sub-total 2 person days divided by total person days x 100% = % category 2
% must = 100%
Please do not forget to specify your 'others' categories.

4.4: Induction and Ex-Leave Refresher Training for 01 April 2008 - 31 March 2009
Please list the people who received induction and ex-leave refresher training during the reporting period. Please note that each person should be counted once,
regardless of the number of training courses they attended. Note that this form needs to be completed against last year's WSP.

Definitions:
Induction Training - Prior to a mine employee first starting work, the employer must provide the person with general induction to the company and train them:
• To deal with every risk to the employee’s health and safety for the work to be assigned;
• In what measures are necessary to eliminate, control and minimize the applicable risks to health and safety;
• To carry out the procedures to be followed for the assigned work;
• To follow the emergency procedures.

Refresher Training - The need for refresher training and the intervals between such training are determined by the Employer after consulting with the Health
and Safety Committee. Please include returns from annual leave. However refresher training also includes cases where after a specified period of time
regulations require a person to be re-licensed as competent, such as the driver of a self-propelled machine. Such refresher training should NOT be included in
form 4.4 and 5.4 but reflected as part of training beneficiaries in 4.2 and 5.2.

Did you provide any Induction/Refresher training in the period 01 April 2008 - No If NO, you do not have to complete this form.

• In what measures are necessary to eliminate, control and minimize the applicable risks to health and safety;
• To carry out the procedures to be followed for the assigned work;
