							              INCREMENT REQUEST PROCESS
STEP 1-COURSE APPROVAL
1 When you sign up to take a course that you hope to use for
movement on the salary schedule, you must fill out the COURSE
APPROVAL form (gold sheet) and submit it no later than 15 days after
the course begins. If you a taking an online course, you must submit it
within 15 days after the registration date.

2 To submit your forms, you must either deliver them in person to
the Office of Human Resources (wait for a copy that has been stamped
as received) or send them by certified mail (keeping your postal receipt
that comes back to you as proof). Using the inter-office mail is no longer
an option.

STEP 2-INCREMENT REQUEST

Once you have taken enough approved courses, you are ready to move
on the salary schedule. These courses must be completed prior to
September 1, 2006 to count for movement for the 06-07 school year.

1 You must submit the INCREMENT REQUEST form (blue sheet)
to the Office of Human Resources with transcripts to verify what is on
your INCREMENT REQUEST sheet. These transcripts can be sent
directly from the college or university. *****Please note that the
INCREMENT REQUEST form and transcripts must be in by November
1, 2006.

PLEASE KEEP COPIES OF ALL THE FORMS THAT YOU
SUBMITTED.
