   1. Admit under the service of Dr. __________
   2. Diagnosis: AECB
   3. Monitor vital signs and respiratory index q ___ hr or ___
   4. Monitor with pulse oximeter
   5. Diet:    ( ) NPO or ( ) Clear Liquids
   6. IV Fluid: D5MM 1 L at ___ ml/ hr
   7. Laboratory:
                       ( ) CBC
                       ( ) Serum Na, K
                       ( ) Chest x-ray
                       ( ) ABG
                       ( ) Sputum G/s, C/S
                       ( ) Spirometry


   8. Therapeutics: Nebulize with combivent q __ hour
   9. Peak Flow Rage: Pre and Post therapy
   10. O2 at ___ lpm. Supplemental oxygen should be maintained at a partial pressure of arterial
       oxygen or just above 60 mm Hg
   11. Systemic steroids followed by oral steroids
   12. PPI
   13. Antibiotic therapy
   14. Smoking cessation
   15. Annual influenza vaccination
   16. Pneumococcal vaccination
