How to obtain a copy of an Ohio State Highway Patrol Traffic Crash Report
The Ohio State Highway Patrol (OSHP) maintains complete reports (including witness statements, supplements, scale drawings, etc.) for OSHP investigated traffic crashes. Purchase ($4.00) the crash report with a credit card by visiting the OSHP Web site: http://statepatrol.ohio.gov/crash.htm OR • Complete the reverse side of this application and mail with the $4.00 processing fee. • If the report cannot be located due to incorrect or incomplete information, the processing fee will NOT be returned. • Be certain to include the address where the report is to be mailed in the space provided. • If you require assistance in completing the application or require additional information, contact the Highway Patrol Post that investigated the crash. for crime victim compensation — The Victims of Crime Compensation Program may provide compensation to qualified individuals involved in alcohol-related fatal or injury crashes. Information about qualifications can be obtained by calling 1-877-5VICTIM, via the Web at http://www. ag.state.oh.us/victim/pubs/cvc_details.pdf, or by contacting your local patrol post.
