REQUEST FOR PROPOSAL
STATE OF TEXAS COUNTY OF BELL VILLAGE OF SALADO RFP: 0001 DATE: June 8, 2009 Type of contract: • Contract is for road repair work using the Zipper Recycling machine or related equipment. Work to be done: • Repair approximately 17,500 square feet of road from the corner of Mill Creek Drive and Chisholm Trail down to the corner of Chisholm Trail and Stagecoach Circle. • Recycle failed base and asphalt to be pulverize to an approximate depth of six (6) to eight (8) inches using the Zipper or related equipment. • Use approximately three (3) per-cent (%) Portland cement and water to help restabilize the material. • Lay two (2) inches of asphalt plant mix overlay on recycled base for eight (8) to ten (10) inches for a total pavement structure. • Pre sub-grade as needed to install approximately 1395 LF lay down curb. Compacting sub-grade and hauling off spoils. • Install eighteen (18) inches wide by six (6) inches deep with 2 #4 rebar. Expansion joints every forty (40) feet with smooth dowels and dummy joints every ten (10) feet. • Curbs to have clean transition into existing driveways and sidewalks. • Provide a traffic control plan. • Include all labor, equipment and materials. Certifications: • Bidder to certify that company has or has equal equipment to the Zipper. • Provide a list of projects they have used the Zipper or equal equipment on. • Provide a list of paving projects with names of owners. • Provide background and experience of the principal members of the company. Contract terms and conditions: • To be a sealed bid process • Bids to be mailed to the Village Secretary, Village of Salado • Include all labor, equipment and materials • Bid period for this project is June 29 through July 13, 2009. • Work to comments within ten (10) days after award of contract.

Payment for the project will be at the completion of project and in compliancy of the Prompt Payment Act.
