                           Terri,
                           The attached are Certificate of Discharge forms that need docket codes. Kim Eaton did not provide codes when she reviewed the
                           forms. She is out of the office until April 30th. I thought you would benefit from getting the forms as early as possible. Please let
                           me know if you want me to follow-up with my email to Kim Eaton requesting suggested docket codes. The forms are still in draft
                           format. And, I do not have confirmation, yet from the Washington State Patrol that they do not need a separate cover sheet as
                           long as the "cover sheet" information is on the front page of the no-contact order, which it is in the attached draft.
