									                    SIMPLE EYE TEST METHOD

PURPOSE

This procedure defines the method of conducting Eye Test.


METHOD

A) SHORT SIGHTED TEST

   Eye Tester will check for Short Sighted by using Western Optical Reading Card.

   Do and don’t on near Vision Test
   a. Do not take the test if the person is tired.
   b. Do not study the chart before the test.
   c. Choose a well-lighted room away from any window to avoid glaring
   d. Wear the glasses or contact lenses you normally use for reading.
   e. Cover one eye at each time when doing the test.
   f. Hold the Reading chart about 14 inches away from your eye.
   g. Read the sentences started from the tiny J1 to J10.
   h. Acceptable vision is J4 and below eg J3, J2, J1
   i. The employee that could only read the character at J7, J8, J10 is suspected
       to have near vision problem
   j. Repeat the same process for the other eye.

If the personnel fails to read J4, means his/her eye have problem

He/she is recommended to correct his/her eyes by wearing spectacles.

B) LONG SIGHTED

Eye Tester will check for Long Sighted by using Snellen Chart.

Do and don’t on Far sighted Problem Test

   a.   Do not take the test if the person is tired.
   b.   Do not study the chart before the test.
   c.   Choose a well-lighted room away from any window to avoid glaring.
   d.   Wear the glasses or contact lenses you normally use for reading.
   e.   Cover right eye when doing the test for left eye.
   f.   Cover left eye when doing the test for left eye.
   g.   For standard Snellen chart, the distance is 20 feet.
   h.   For reduce size of the snellen chart, take the biggest alphabet width and
        times it by 2.838, this give you the number of feet you must stand or sit from
        your screen while taking the test.
   i.   The best vision is 20/20 the 20/30 is acceptable for Long vision test; the other
        result is not acceptable.
   j.   Repeat the same process for the other eye.


Personnel who are fail to read the alphabet, meaning that his/her eyes having
problem. He/she is recommended to wearing spectacles.

C) COLOUR BLIND TEST

Colour Blind Test will be conducted using Ishihara chart. The testing will be performed
for both eyes. One eye must be covered during testing.

Do and don’t on Color Blind Test
   a. Do not take the test if the person is tired.
   b. Do not study the chart before the test.
   c. Choose a well-lighted room away from any window to avoid glaring
   d. Wear the glasses or contact lenses you normally use for reading.
   e. Cover one eye at one time when doing the test.
   f. Hold the Reading chart about 14 inches away from your eyes.
   g. Read through the numbers on the pages.
   h. All numbers should be identified and correct for non-colorblind defect test.
   i. Wrongly read or failure to see the number is suspected to have colorblind
       defect.
   j. Repeat the same process for the other eye
   k. Refer to Ishihara Chart on type of colorblind.

Personnel who are fail to answer, meaning that his/her eyes having problem (colour
blind)
