      How has the nominee demonstrated excellence in their chosen field of study,
       community or public service?
The Alumni Association will use this initial application to evaluate all nominees
and will then create a list of finalists. Should your nominee become a finalist, you
will be contacted by March 4, 2011. You will be asked to provide, at minimum, the
nominee’s current curriculum vitae, a summary of their achievements and
contributions, and two (2) letters of support. These items must be submitted by
March 18, 2011 at 4:30pm. Finalists will need to sign the Nominee Release Form.


                  Deadline for submissions is 4:30 pm on Feb. 24, 2011.
