IMPORTANT SUBMISSION NOTES:
 1. You MUST fill out the sample submission table below. If you are submitting
samples in tubes, each tube must be numbered in addition to having a
sample name. The number must appear on the tube.
 2. Genotypes will be reported in the order that you submit your samples.
Samples cannot be resorted during the genotyping process.
 3. Samples may be submitted in numbered tubes or 96-well plates (preferred).
 4. Samples must be submitted at approximately the same concentration.
A typical concentration is 200 nanograms per microliter. For some samples
this is not possible (such as Blood/Normal/Tumor MSI plates) – in this case
indicate the sample type in the sample comment column. If submitting tissue,
    this is not applicable.
 5. If submitting in 96-well plates, please leave at least three empty wells for our
internal control samples that we will add. If you are submitting non-human
DNA, you will need to provide 2 samples to use as controls.
 6. If submitting more than one 96-well plate for a project, please send your own
Excel spreadsheet with the following columns: Plate number, Well position,
Sample name, Comment. E-mail this to Mike at mklein@cores.utah.edu.
 7. DNA Quality - DNA samples submitted to the Core should be free of protein,
phenol, excessive EDTA and other contaminants. Poor quality DNA will
result in a higher than usual drop-out rate and significantly increase the
time it takes to complete your project. If you are diluting DNA samples to
submit to the Core, please use sterile distilled water to avoid unnecessary
EDTA. Some samples, such as those micro-dissected from stained and
archived slides, are notoriously poor quality and the effective concentration is
unknown. In past projects we have still been able to obtain results for
approximately 80% of these samples through hot start PCR, less stringent
Mg and/or temperature conditions and increased cycle number.
 8. How much DNA? For genomic DNA please submit at least 50ng per
sample per marker. If your DNA is in very short supply, let us know and we
will try to work with what you have. All samples should be at the same
concentration. Many labs have a "standard" concentration of 200ug/ml and
this is acceptable for core use. For DNA from tumors or micro-dissected
slides where the actual quantity of DNA is unknown, please submit equal
volumes of the samples to be tested.

PLEASE COMPLETE THE TABLE ON THE NEXT PAGE!
