Documentation Checklist
The following documentation must be submitted with your application please submit all
documentation on separate sheets.


      College Diploma (Or a letter from college confirming graduation date)

      High School Diploma or official transcript

      AEMCA Certificate or letter of registration to write AEMCA testing

      Current CPR-Level C Certification and First Aid Certificates

      Driver’s Abstract – 3 year search issued within the last 60 days.

      Class F Driver’s License (front and back)

      Successful Physical Agility Evaluation (in last 8 months)

      MOHLTC mandatory training record or letter from college confirming mandatory training
      was received.

      Copy of any relevant certifications (i.e. Base Hospital certifications, BTLS, etc.)

      Criminal Record Check (ensure this check includes a reference to your ability to work
      with the vulnerable sector) – Issued within the last three months.

      Proof of Immunization for the following:
             Measles, Mumps, Rubella
             Diphtheria, Polio,
             Chicken Pox,
             Hepatitis B
             Tetanus (issued within last ten years)
             Influenza

The following training / courses below will be considered an asset during the candidate
selection process as Perth County PCP Paramedics are IV, CPAP, King LT and 12 lead

      Proof of 12 lead ECG certification / education with STEMI recognition training

      Proof of a Medical Advisory Committee (MAC) approved Intravenous course and
      certification for PCP IV initiation.

      Proof of continuous positive airway pressure (CPAP) training / certification

      Supraglottic airway training / Certification (King LT airway)
