                      SUPPORTING DOCUMENTATION – MINIMUM QUALIFICATIONS
Before entering the firefighter application process, you should acquire copies of and prepare to submit
supporting documentation (on request) to verify you meet the minimum qualifications.

     Your documentation must include legible copies of official credentials. In some instances, you may
     be asked to submit an original, unaltered document.

     It is your responsibility to understand the qualifications and accepted types of supporting
     documentation before you apply.

     If you are found to have falsified or intentionally misrepresented your credentials, you will not be
     allowed to proceed and future applications will not be accepted from you.

     You will need to maintain your qualifications throughout the assessment processes. Recertification
     or further validation of credentials may be requested.

     You may be asked to produce documentation on short notice. The timeline to submit your
     documents may be quick with little flexibility. If you cannot meet the deadline, or do not provide
     accepted forms of documentation, your application will not proceed.

                                                      DOCUMENTS REQUIRED
Although you are not required to provide proof of qualifications when you initially apply, applicants
who are shortlisted will be required to submit supporting documentation (on request) soon after the
closing date of the competition. The documents requested will include:

1) Proof of Immunization History confirming immunity to:

            Measles (Rubeola).                            Mumps.                                      Rubella (German measles).
            Tetanus (< 10 years).                         Diphtheria (< 10 years).                    Hepatitis-B.

2) Proof of High School completion or recognized equivalency.

3) Proof of current First Aid certification (Standard level or higher) and current CPR certification
   (Health Care Provider level only).

4) Driver’s Licence Abstract – minimum five year history and dated no earlier than January 1, 2011.

5) Criminal Records Check – dated no earlier than January 1, 2011.

     a) You may also be required to provide proof of a pardon if you have had a criminal conviction.

6) If applicable, proof of a valid Permanent Resident Card.

                                                   ADDITIONAL INFORMATION
Documents from web-based companies will not be accepted unless they are issued directly by a
government agency (i.e., Ontario Ministry of Transportation, RCMP, etc.).

OUT-OF-COUNTRY CERTIFICATIONS

If applicable, please have your documentation translated into English.

You will need to independently have your out-of-country documents assessed by one of the federally
recognized Alliance of Credential Evaluation Services of Canada such as the International Qualifications
Assessment Service (IQAS) http://employment.alberta.ca/immigration/4512.html. IQAS can perform a
basic assessment which typically takes six weeks from receipt of request; a fee will be required.

     Exception: United States credentials. You may not need to obtain a certification assessment if your
     post-secondary training institute is accredited by the U.S. Department of Education
     http://ope.ed.gov/accreditation/.

IMMUNIZATION REQUIREMENTS

You will be asked to provide a complete record of your immunization history or documentation from a
medical authority. Documents must either state dates of injections or otherwise confirm immunity.

If childhood immunization records are not available, you may provide documentation stating titre test
results which confirm your immunity. To order a titre test, please contact your physician.

     Exception: Tetanus and Diphtheria vaccinations require boosters every 10 years. You must provide
     documentation which verifies the date when you last received these injections. If the date listed is
     more than 10 years ago (< 2001), you must be vaccinated prior to applying. Titre test results are
     not accepted for Tetanus and Diphtheria.

Hepatitis-B vaccinations must follow the normal recommended dosing schedules.

If you received your Hepatitis-B vaccinations outside of these schedules, you may be asked to produce
titre test results to confirm your immunity. A minimum of three Hepatitis-B vaccinations and/or proof
of immunity will be required prior to hire.

You may apply after having received at least one Hepatitis-B vaccination. However, you must follow-
through to ensure the remaining doses are received according to schedule (proof required).
