									AMERICAN FOUNDATION FOR SUICIDE PREVENTION
NATIONAL SURVIVORS OF SUICIDE DAY — PLANNING CHECKLIST

These are just guidelines. Except for items in bold face, feel free to modify them as necessary.

FIRST STEPS
        Find a location that has a satellite downlink, high-speed Internet access or DVD player and large screen TV or
        projection system, meets your local conference requirements (anticipated audience size and breakout rooms,
        if necessary) and is available on a Saturday.
        REGISTER YOUR SITE by submitting the Site Registration Form by Oct. 22, 2008.
        If you are requesting a DVD version of the broadcast, submit the DVD Request Form
        (www.afsp.org/files/Surviving/dvdrequestform.pdf) by Oct. 22, 2008.
        Designate a local technical contact for your site and give him/her the Technical Information Sheet.
        Establish a planning committee and recruit volunteers to help you.
        Create a budget, determine registration fee, and/or seek local sources of funding (such as an interested survivor family
        or local foundation).

PLANNING THE CONTENT
Some sites include local programming in addition to watching the broadcast. If you decide to include some, you’ll want to create
a schedule, identify topics, and find appropriate speakers, panelists, and/or breakout group leaders. If you’d like more guidance,
we can put you in touch with others who’ve organized successful conferences in the past.

LOGISTICS
        Confirm technical arrangements and room set-up with your local technical contact.
        Spread the word!
                  Print publicity materials using AFSP’s templates, and distribute them widely.
                  Use AFSP’s press releases for local newspapers.
                  Enlist your local network of survivors and support groups to publicize the Day.
        Arrange for food and beverages, if applicable.
        Keep track of registration.
        Download materials from www.afsp.org/survivorconference and put together information packets for attendees.

THE DAY OF THE CONFERENCE
       Prepare for arrivals, including walk-in registrations.
       Set out information packets and any other materials, such as pads, pens, and nametags
       Breakout rooms will need signs, chairs arranged in a circle, and boxes of tissues.
       Have each attendee complete an Evaluation Form.

AFTER THE CONFERENCE
       Return the completed Evaluation Forms to AFSP together with an approximate count of your attendees.
       Save the date for next year: Saturday, November 21, 2009!
