SUBJECT: Department Policy Reporting Procedures and Requests for Leave: Sick, Personal and Annual

The following procedures are in effect upon receipt of this memo. When you are unable to report for work due to sickness or for
personal reasons, the procedures listed below are to be followed:

SICK REPORTING

1.      Calls are to be made to the manager on duty two hours before your shift. If your shift begins at or before 6:30 a.m. (or the
        first shift for the day), you may call at the beginning of the shift. YOU MUST SPEAK WITH A MANAGER TO REPORT
        THE ABSENCE. Do not leave a message with fellow employees, students, clerical staff or the answering machine.
        Managers calling in sick must also speak to their supervisor in addition to the manager on duty. If their supervisor is
        not present when they call in they must call back or call their supervisor (or higher authority, if their supervisor is not
        available) at home.

2.      If you are on sick leave, you must call in every scheduled work day (including weekends) to update us on your progress.

3.      If you are hospitalized, or are on sick leave for more than seven work days, you must make arrangements with the Dining
        Services Human Resources Office to avoid calling every day. However, your call to the Human Resources Office on Monday
        of each week is required so that the Administration is informed of your progress.

4.      In cases of injury /leave, you must call the Human Resources Office on Monday of each week to keep the Department
        informed of your progress.

5.      If you are sick for more than five consecutive days, or if you have more than five sick occurrences in a twelve month period,
        you must submit an original certificate from a licensed medical provider that authenticates the period of absence and states
        you are well enough to return to work and perform you duties, including working with food (if appropriate). This certificate
        must be given to the manager on duty before beginning work.

6.      Sick leave may be requested in advance in writing for medical appointments, examinations or treatment with an accredited,
        licensed or certified medical provider that cannot be scheduled during non-work hours. An original certificate from the
        licensed provider authenticating the appointment must be given to the Unit Administrator before returning to work. Requests
        for sick leave for appointments should be made at the time the appointment is scheduled and must be at least 24 hours in
        advance in writing, unless the appointment is an emergency.

LATE REPORTING

1.      If you are going to be late, a call is required prior to the start of your shift. Failure to call in prior to the start of your shift if
        you are going to be late will result in disciplinary action.

2.      Repeated tardiness will result in disciplinary action.
PERSONAL LEAVE REQUESTS                      (Regular Staff Only)

Personal leave must be requested from the Unit Administrator prior to use, but it is subject to the approval of the Unit Administrator
in case it would create a staffing shortage, unless being used for the observance of a recognized religious holiday.

ANNUAL LEAVE or “Time Off”

Annual Leave or time off from work without pay or for medical appointments (if you do not have accrued leave) must be requested in
writing and be approved at least 24 hours in advance by the Unit Administrator or higher authority. Annual Leave and “time off
without pay” requests are not automatically granted.

GENERAL LEAVE REQUEST INFORMATION

Requesting leave is the sole responsibility of the employee, not the Unit Clerk, Manager or the Unit Administrator.

Requests for Annual or Personal Leave or time off without pay or for medical appointments are to be made on the “Leave/Request
Report” form. Sick Leave is recorded by the manager on that same form. The work group supervisor is responsible for entering the
correct information into the system.

Please note that the approved requests for leave results in an official change in your schedule and any additional changes in
your schedule require prior approval from your Unit Administrator.

If you do not call and follow these policies when you are going to be absent or late, or if the call is reported to someone other than an
on-duty manager, the incident will be considered an unexcused absence and will be cause for disciplinary action (i.e., written warning,
suspension without pay or termination).
