            IASC GUIDELINES FOR ADDRESSING HIV IN
                   HUMANITARIAN SETTINGS

                            Feedback questionnaire

The purpose of these Guidelines is to enable governments and cooperating agencies,
including UN Agencies and NGOs, to deliver the minimum required multi-sectoral response
to HIV&AIDS during emergency situations.

This questionnaire is designed to elicit feedback on the Guidelines, as they are used in the
settings within which you work. Your suggestions will be used to revise and update future
versions of the Guidelines. If you include your name and postal address (optional) in your
response, we will send you a copy of future versions of the Guidelines.
