					                          Suspensions and Expulsions Data Collection

Data collection year: July 1, 2009-June 30, 2010


Districts are required by law to report all suspensions and expulsions to the state by June 30 of each year. The
state collects this data in order to fulfill both state and federal reporting mandates as well as measure statewide
school safety.


We use suspension and expulsion data to complete the following required reports: Gun-Free Schools,
Consolidated Performance Report, SPED Report of Children with Disabilities Subject to disciplinary Removal,
and the EDEN Discipline Incidents, Students Involved with Firearms, and Firearm Incident reports.

As of the 2009-2010 school year, the data collection method has switched from the online system to the current
Excel data file. While districts may use different methods for collecting data, districts are required to report data
using the template provided on the Incident Reporting tab.

Instructions:
Please include all requested data elements. To ensure that items are properly coded, please consult the code list
tab. Also, review your data for accuracy before submitting. Any incomplete or inaccurate data submission will
be returned to the district for completion.
When entering the Alaska Student ID, please ensure that the number entered is correct. We do not track District
Student ID numbers and will not be collecting them on this report.

The district ID requested in this collection refers to the ID number assigned to your district by EED. This code,
along with the school codes for your district, is located on the School Codes tab.

When entering the weapon ID and original length of disciplinary action, keep in mind the distinctions made
between a firearm and deadly weapon. Firearms are defined as "a weapon designed to expel a projectile by the
action of an explosion and includes guns, bombs, grenades, and any other device designed to explode and is
capable of causing bodily harm." By law, students caught possessing a firearm must be expelled for no less than
365 days. Conversely, deadly weapons are defined as "any instrument, material, or substance that is used for, or
is readily capable of causing death or serious bodily injury." Students caught possessing deadly weapons must be
suspended for a minimum of 30 days. If the disciplinary actions were modified, you will need to indicate this
and provide the actual length of suspension/expulsion.
All dates should be in the MM/DD/YYYY format (Example: 06/30/2010).

Please do not include suffixes with the last name field as this will generate errors in our database.
Please save the required data in this workbook and return it to EED as a complete workbook. EED
requires that data be submitted on a district level. District staff are responsible for consolidating data into
one workbook before submitting. Due to the confidential nature of the data, please responsibly handle the
workbook and the information it contains. Completed workbooks should be returned by email attachment
to Todd Brocious at todd.brocious@alaska.gov.

Interim Alternative Educational Setting
                                  Definitions
A weapon, device, instrument, material, or substance, animate or
inanimate, that is used for, or is readily capable of causing death or serious
