Definition               The classification of each medication as a discrepancy and the assignment of the discrepencies to a classification category. Each
                         medication order identified as having a discrepancy may be assigned to only one "discrepancy category". Clinicians can select whichever
                         they think is the most important. However, for those medication orders for which more than one discrepancy category applies equally it is
                         often helpful to follow the alphabetical order e.g A1, A2, A3...E1. For example: for a medication order which is for an incorrect medication

            discrepancy identified in the sample for this reporting
            period.
      4.03 Total number of medications with a discrepancy
            identified in the sample for this reporting period.
Calculation of Numerator
      4.04 A1 -Total number of medication discrepancies in
            month's sample (# 4.03) associated with a
            Medication not currently prescribed. That is,
            prescription medications which the client is now taking
            but is not currently prescribed by the prescriber
            (includes samples, prescription medications and non-
            prescription medications).
      4.05 A2 - Total number of medication discrepancies in
            month's sample (# 4.02) associated with a clients No
            Longer Taking Medication as prescribed. That is,
            the client is no longer taking the medication but it has
            been prescribed.
      4.06 A3 -Total number of medication discrepancies in
            month's sample (# 4.02) associated with Over-The-
            Counter (OTC) medication not taken as
            prescribed. That is, the Client has not been taking
            OTC medication as directed by family physician/RN
            (NP)
      4.07 A4 -Total number of medication discrepancies in
            month's sample (# 4.02) associated with a Drug
            Allergy. That is, the Client has a clinically significant
            medication allergy to prescribed medication

       4.08 A5 - Total number of medication discrepancies in
            month's sample (# 4.02) associated with a Drug
            Duplication. That is, the Client has inadvertently
            been prescribed two medications from the same
            therapeutic class
       4.09 A6 -Total number of medication discrepancies in
            month's sample (# 4.02) associated with a Drug
            Interaction. That is, the Client has a clinically
            significant drug interaction to prescribed medication.
