REFERRAL PROCESS
 All referral faxed to the NSFHT
 Referrals are triaged to NSRDP if not a client of
  a NSFHT primary care provider
 Together, the NSRDP and the Diabetes
  Management Program of the NSFHT will offer
  joint classes
 Internist services one day/month for clients who
  are having difficulty controlling their condition
 NSRDP staff work with Medical Directives,
  primary care providers, a lead physician and an
  internist
