     I believe in the Triune God, Father, Son and Holy Spirit, as revealed in the
     Holy Scriptures and acknowledge Him as the Sovereign Creator, Sustainer,
     Redeemer and Judge of all things visible and invisible.
     Recognising that all creation is fearfully and wonderfully made, I confirm that
     life is a sacred gift from God and that all human life bears the image of God.
     I affirm that the purpose of my creation is to glorify God throughout my life
     and to praise His name in all I think, say and do.
     Being created in God’s image, I understand that I am called to love, honour
     and obey Him, to enjoy fellowship with Him and with people and to build
     relationships that reflect His glory, particularly within marriage, family and the
     body of believers.
     I declare my faith in God’s revelation of His Son in the man Jesus, by whose
     death and resurrection we receive salvation. Humbly confessing my sin and
     inability to please God by my own initiatives, I continually give thanks for His
     unmerited grace by which He accepts me as His child.
     I long and joyfully hope for the redemption and restoration of God' creation in
     fulfilment of His promises.
     Confirming my calling and equipping to serve broken people as a wounded
     healer, I call on my colleagues, family and fellow Christians, to encourage me
     (through help and correction) to serve each patient' need just as if I were
     serving Christ, without letting my work become an idol in itself.
     Regardless of any personal bias I may have, I undertake to serve each
     human life entrusted to my care with deepest respect, including the unborn,
     the poor, the terminally ill, the incurable, the disabled and the elderly.
     Being a trustworthy steward, I promise to live with integrity, speaking the truth
     to my patients in a way they can understand and handling their trust
     I uphold marriage as a lasting and divinely inspired bond. In my practice and
     my counsel to my patients, I will respect the integrity and sanctity of the
     marriage relationship.
     It is my prayer that the wonder of God’s creation will never be lost to me, its
     miracles of life and love remaining forever new, and the practising of my art
     and service always filled with love and joy.

I now call on those here present to witness the humble
dedication of my life and career to the Lord Jesus Christ, and
to remind me of this as often as may be necessary.
