H.K. Lee Tae Kwon Do will provide qualified instructors and use of its training facilities
(Including time for supervised game time and organized sports) for a period of time not
to exceed 3 hours. The birthday child’s parent or guardian will provide all additional
party supplies (i.e. birthday cake, decorations, paper goods, etc.), clean up party room,
and agree to abide by the rules and regulations of H.K. Lee Tae Kwon Do. Parent or
Guardian agrees that their signature on this agreement constitutes a non-refundable and
binding agreement between H.K. Lee Tae Kwon Do and the Parent/Guardian.
Furthermore, the Parent/Guardian agrees to absolve H.K. Lee Tae Kwon Do and its
employees and representatives from costs or damages arising from injuries and or
liabilities suffered on these premises. It is understood that the Parent/Guardian must
provide H.K. Lee Tae Kwon Do with signed permission slips and or waivers from all
participants, parents, or guardians.

*Note: At least $20 gratuity is required for each instructor.
       (Gratuity for instructors is not included in the total cost of the Birthday Party.)
