This is a policy a company can use to establish paid time off for its employees. Full-
time employees are entitled to paid time off and the company can customize the number
of hours of work they must complete in order to receive paid time off. Part-time
employees are not entitled to receive paid time off unless authorized by the company.
This policy should be used by small businesses to establish a uniform and consistent
paid time off policy.
TIME OFF POLICIES
TIME OFF POLICY
(a) “Employee” shall mean any person who is employed by the Company.
(b) “Full-Time Employee” shall mean any Employee of the Company who is scheduled to
perform work for the Company for no less than ______ (___) hours per work week.
(b) “Part-Time Employee” shall mean any Employee of the Company who is schedule to
perform work for the Company for less than _____ (___) hours per work week.
(c) “Work Week” shall mean the days Monday through Friday.
2. Paid Time Off
Any Full-Time Employee who is scheduled to perform work for the Company shall be
eligible to receive paid time off from the Company. For every _____ (___) hours of work
performed by a Full Time Employee in one (1) calendar month, the Full-Time Employee shall
accrue _____ (___) hours of paid time off.
A Part-Time Employee shall not be entitled to receive paid time off from the Company
unless authorized by the _______________ of the Company [PROVIDE JOB TITLE OR JOB
DESCRIPTION FOR THE PERSON OR PERSONS, IF ANY, WHO ARE ENTITLED
TO AUTHORIZE PAID TIME OFF FOR PART-TIME EMPLOYEES]. Any authorized
paid time off for any Part Time Employee shall be on a case-by-case basis.
Paid time off does not accrue during an unpaid leave of absence.
Any Employee shall use his or her paid time off for any reason as the Employee deems
appropriate. Any Employee wishing to use his or her paid time off shall complete and submit
any and all necessary forms and documents as may be required by the Company. Any requests
by any Employee for paid time off shall be submitted to the appropriate person not less than
_____ (___) days/weeks [SPECIFY WHICH. CROSS OUT OR DELETE THE TERM
THAT DOES NOT APPLY] in advance save and except for illness or a family member's
illness or death. In the event of illness or death, Employee shall provide as much advance notice
as is reasonable in that specific circumstance. In the event that Employee requests time off due
to illness or death and the required advance notice was not provided, Company may, in its sole

discretion, request medical or other documentation. Employee agrees to provide such
documentation to Company within a reasonable period.
In the event any Employee is absent due to illness, the Employee shall contact the
appropriate personnel prior to the commencement of the Employee’s work day each day and
each day thereafter until the Employee returns to his or her work. The Company requires any
Employee who is absent from his or her work for more than _______ (___) days to provide to
the appropriate personnel a doctor’s letter stating that the Employee is absent from work due to
medical reasons and an expect return date to work. In the event of hospitalization or other
serious medical condition, Employee is excused from making daily contact--as called for in this
paragraph--until his or her medical condition improves enough to allow this activity. In this
event, Employee agrees to make all reasonable efforts to alert Company to the seriousness of his
or her medical condition.
In the event an Employee is absent, any income from the Company’s disability benefits,
paid time off or any workers compensation, which may be due and payable to the Employee,
shall be synchronized so that the Employee’s total compensation from any of the above will not
exceeded the Employee’s normal base wages.
In the event any Employee is absent from his or her work, and then fails to complete and
submit the appropriate and required forms and documentation, such absence shall be unpaid by
the Company. Company will not fail to pay Employee for the above absence, unless Employee's
failure to supply the required documentation occurs after Employee has recovered enough to
physically comply with the above-described requirements. In the event any Employee
terminates or the Company terminates his or her employment with the Company for any reason
whatsoever, any and all unpaid paid overtime shall be paid by the Company to the Employee
upon such termination of employment.
