SURVEY ADMINISTRATION GUIDE
This document contains instructions for the Survey Administrator as well as instructions that the Survey Administrator will pass on to the survey respondents.
ADMINISTRATION PROCEDURES
General Requirement: 1. Have enough No.2 pencils and surveys for the maximum number of respondents expected, plus extra pencils. Keep some survey answer sheets in reserve in case some of the forms are damaged. Answer sheets completed in “ink” or “felt tip” cannot be processed. Please do not reproduce answer sheets on a copy machine. They must be in their original format for correct processing. 2. Provide to each respondent a No. 2 pencil, the survey booklet and an answer sheet. 3. Provide or read to them the “Instructions to Survey Respondents”, emphasizing the following points: - Military family members should identify their status by writing “FM” at the top of the survey answer sheet. - It is important to list the community on the answer sheet - The survey is anonymous and voluntary. - Remind the respondents that they are taking the “Well-Being Survey”. - There is no time limit for completing the survey, however 30 minutes should be sufficient. 4. Answer and clarify any questions regarding the demographic information being asked; the first seven questions in each survey questionnaire focus on the status of the individual. 5. Collect the completed answer forms, survey questionnaires and pencils. After respondents leave: Count the number of surveys and complete the Survey Cover Sheet, identifying the community the respondents come from. Return the surveys and the cover sheet to USAREUR G-1, SAFR Office. Shipping instructions are detailed on the cover sheet.
INSTRUCTIONS TO SURVEY RESPONDENTS
You are here to complete the WELL-BEING SURVEY The purpose of this survey is to provide the command leadership with information about what you think about the well being of the Army family in Europe. It is directed at Soldiers, US Civilian Employees, Retirees and their family members. An independent organization will compile the survey results and produce a summary report. Only summary reports for all of USAREUR and each USAREUR community will be sent to the command leadership. Completion of this survey is optional. We are not keeping track of who completes or does not complete the survey, but we are grateful for your time and effort if you do. By completing the survey, you can assist in identifying what is going well in your community and clarify what is in need of improvement. We will greatly appreciate your honest responses. Your answer sheet will be scanned into a machine that will read the responses you provided. The scanner cannot read ink, so You must use a No. 2 pencil to mark your answers Please mark the name of your community in pencil at the top of the answer sheet. Completely fill in the oval that most accurately represents your response to each question. Do not mark outside the answer ovals. Only one oval should be filled in for each question. If you change your mind, erase completely and fill in your new selection. Please complete the entire survey. There is no time limit; however, the survey should take no longer than 30 minutes to complete If you have any questions, please ask the administrator.
Thanks in advance for your time, effort, and honesty. When you finish, return your surveys, answer sheets, and pencils to the administrator before leaving.
SPECIAL GUIDANCE FOR SPOUSES AND FAMILY READINESS GROUPS The Well-Being Survey is intended to ensure that family members are afforded the opportunity to provide their perspective on the efforts made by the Army to meet the unique well being related needs of Soldiers, US civilians and their families. In order to facilitate and ease the ability of family members to partake in the survey, USAREUR and IMCOM-E are making the survey available to family members through various methods. As an example, you may be provided a survey booklet and answer sheet by your spouse, you may be asked to complete the survey by the Family Readiness Group to which you belong, and you may be asked to complete the survey while at a community facility. Where to get the survey: The survey questionnaire can be downloaded from the G-1 Website, http://www.per.hqusareur.army.mil/umss/wbs.htm, however you will need an answer sheet from a unit or a community agency for your response. Please read the “Instructions to Survey Respondents” before you begin answering the survey. After completing the survey, please return your answer sheet to the administrator of the survey or to agency personnel where you obtained the answer sheet. The administrators will forward the answer sheets to USAREUR G-1 for mailing to the independent contractor. Note: family members should identify their status by marking “FM” at the top of their survey answer sheet.
