A guide to the completion of the CRB Disclosure Application Form
Please take a moment to read this guide, it has been specifically designed to aid the swift completion of the form and
advise you on the evidence that you will need to bring with you when you hand over the form.
With CRB Disclosure Application Forms issued by the Scout Association you will only need to complete the following
sections (these are denoted by the large letter on the left hand side of the page.), A, C, D, and H. All other sections are
not required and should be left blank (do not cross through or write N/A). ‘Section D’ only needs to be completed if you
have moved your place of residence within the last 5 years and if you have moved more than once your previous
addresses need to be given on Continuation Sheet provided. Do not cross out or change any information in Section B.
Only use black ink and write in BLOCK CAPITALS. If for any reason you have a problem with completing the form please
ask for assistance.
There are boxes on the form to show you where to write. Please use only one letter or number per box and make sure
not to write over the edges of the box. Where relevant, leave an empty box between words but not between numbers or
postcodes. Always mark a cross [ X ] within the relevant boxes. Do NOT put ticks or any other symbol in these boxes.

SECTION A : Complete this section as fully as possible. Note if you enter your title, as Mrs, Ms or Other you must
complete line 20 in “Section C” even if it is the same as that entered in this section.
SECTION B : Leave this section do not make any corrections.
SECTION C : Complete this section as fully as possible. Note lines 28 to 32 must be completed or the form will be
returned unchecked delaying your CRB Clearance.
SECTION D : You must give a full 5 years address history to the month that you sign and date the declaration in “Section
H”. This means that at least 60 months must be covered, any gaps, even 1 month between moves must be accounted for.
Use this section for the address you resided at before your current and then if necessary use the continuation sheet
provided to list the remaining addresses in reverse order to cover the last 60 month / 5 year period. Remember when
entering dates for this information only the Month/Year is required e.g. February 2004 = 022004.
SECTIONS E , F & G : Leave these sections blank, they are no longer required by the CRB.
SECTION H : In this section, you must cross the relevant box for line 66. Cross box 67 only if you are providing additional
information in the continuation form that is not already in the main CRB application form. You must sign the first box
(line 68). You no longer have to sign the second box (line 69) please leave blank. Please remember to enter the date that
you completed and signed the form (line 70).
Once you have completed the CRB Disclosure Application Form it needs Can you produce any

Any two items HIGHLIGHTED maybe used to confirm your current address. (a driving licence may only confirm current address if
not used as the only Group 1 document)

You should read the fact sheet “Disclosure Policy and Procedure for Volunteer Roles” available online from
http://www.scoutbase.org.uk/library/hqdocs/facts/pdfs/crbspp or if you do not have internet access please ask the person
supplying the CRB Disclosure for a copy.
Notes for the responsible adult checking ID documents
SECTION X : You will need to see original documents, you should enter all appropriate details into Section X, where there in no space
available in this section you should use the reverse of the “continuation form”. Please note you must confirm the current address
using two separate documents from different sources, a driving licence maybe used if it is not the primary Group 1 ID. Once you
have check the current address make sure you cross line 15 and write your name in line 16 once you have checked the evidence and
the CRB Application form.
SECTION Y : Leave section Y blank, it is not for you to complete. If this section is completed, the form will be invalid and another new
form will need to be completed by the applicant.
Please note that anyone who helps the group more than once a month (a weekend camp will require a form) is required
to complete the new OH form in preparation of new legislation in October 2009 unless they are completing an AA form for
Membership / Associate Membership.
