The Council represents consumers, businesses, insurance companies, hospitals and physicians. Again this year, we are providing hospital inpatient charge and utilization information on deliveries to consumers through the Guide to Deliveries in Illinois Hospitals for 1999. This report focuses on the delivering of babies in Illinois hospitals. We hope that Illinois’ expecting mothers and other interested parties will find this information useful, and we encourage anyone to contact us with suggestions on how our services can be improved. Introduction The delivery of a baby is the most common reason for hospitalization in Illinois. In 1999, deliveries accounted for 11 percent of all inpatient hospitalizations statewide. The Council’s mandate requires the agency to periodically supply state government officials and residents with information on Cesarean section deliveries. The Guide to Deliveries in Illinois Hospitals for 1999 fulfills that mandate by reporting, by hospital, information on both Cesarean section and vaginal deliveries. Also, the purpose of this guide is to inform expectant parents on which hospitals are performing Cesarean and vaginal deliveries and the amount, on average, they are charging for those services. It presents statistics by age group, payer type, hospital health service area, and a trend section covering the years 1990 through 1999. A hospital table at the end of the report lists the number of discharges for deliveries, the average charge and the average length of stay (LOS) at each hospital. This guide does NOT intend to rate any one hospital over another. II. Source of Data By state law (Public Act 88-535), hospitals in the State of Illinois provide the Illinois Health Care Cost Containment Council (IHCCCC) records for all inpatient discharges. The data source for this report was the Council’s Illinois Uniform Billing (UB-92) database covering the years 1990 through 1999. Data are maintained in computerized files by the Council. The UB-92 database includes approximately 1.5 million discharge records per year among Illinois’ approximately 220 acute care hospitals. The Council’s data are edited using the federal Health Care Financing Administration’s (HCFA) Medicare Code Edits. Hospitals consistently, on a statewide basis, have a data submission compliance rate of 99 percent. In other words, the Council's database contains quality, validated data for 99 percent of the inpatient discharges that occur in the state. Coding Classification for Delivery Hospitalizations Delivery hospitalization records compiled for this report were identified using the Diagnosis Related Group (DRG) classification of each discharge. The DRG system, developed by HCFA, classifies patients into clinically cohesive groups that demonstrate similar consumption of hospital resources and length of stay patterns. 2 The six DRGs used for this report were: Vaginal Deliveries DRG 372 DRG 373 DRG 374 DRG 375 Vaginal Deliveries WITH Complicating Diagnoses Vaginal Deliveries WITHOUT Complicating Diagnoses Vaginal Deliveries WITH Sterilization and/or D and C Vaginal Deliveries WITH O.R. Procedure except Sterilization Cesarean Section Deliveries DRG 370 Cesarean Section WITH Complication and/or Comorbidity DRG 371 Cesarean Section WITHOUT Complication and/or Comorbidity Data Considerations The Council asks that the following points be considered while viewing this guide: U The monetary data in this report are based on hospital charges, rather than payments. Actual payments made to the hospital are likely to be less than the initial charge. Also, the charges are hospital charges only and do not include physician or other professional fees that may be charged. These data reflect actual charges and lengths of stay for each hospitalization. The charges and lengths of stay are then totaled, and an average for each hospital or region is derived. While charges and lengths of stay are affected by the severity of illness of the patient, these data have not been adjusted for any differences across hospitals. Thus, each hospital’s data may include discharges that are considered outliers due to extreme charges and/or lengths of stay. These data reflect inpatient hospital deliveries, which represent the mother’s hospitalization. Newborns’ hospital stays are represented by other DRGs not included in this report. A single discharge in this report may possibly represent the birth of multiple babies in the case of twins, triplets, etc. Also, these data include only inpatient deliveries occurring within a hospital. Numbers for births occurring outside of the hospital and information on who attends births are presented in Tables 1 and 2 on Pages 4 and 5. These numbers, obtained from the Illinois Department of Public Health Center for Health Statistics, represent babies born to Illinois residents inside or outside of Illinois. Becoming an Effective Consumer of Health Care Listed below are some things individuals can do to make themselves more effective consumers of health care: General Tips U Take advantage of telephone nursing services provided by health care providers, insurance companies, health maintenance organizations (HMO), etc. Such services allow the consumer to speak to a nurse and ask questions about appropriate medical care, find a physician or obtain health information. Prepare ahead of time for doctor’s appointments. Write out a list of questions and concerns and bring it to the appointment. If 5 U unclear about anything, ask the doctor for further explanation. U U Before services/treatments are done, ask questions about the care, the costs and alternate treatment options available. Always review your insurance policy and benefits package to decide what steps need to be followed to insure proper coverage. Also look to see what percentage of the cost will have to be absorbed by yourself. Not all health plans offer complete, total coverage of care or payments. For instance, it may be necessary to call your insurer when you are pregnant to maximize your benefits. When the health care bill arrives, check it carefully and contact the provider’s billing department if an error is suspected. This review should be done whether the claim is submitted to the insurance company or not. Obtain educational materials from your doctor and/or hospital. For example, find out what types of prenatal classes are offered at the hospital. Many hospitals offer classes about Lamaze, breast-feeding, parenting, vaginal births after a cesarean section (VBAC) and sibling preparation. Join an organized patient group. Ask the doctor or area hospital about support groups. Such a group can provide you with more information and support. Other resources you can use to find this information are the telephone directory, the newspaper, the Internet or a local bulletin board. U U U Prenatal Care Tips Expectant mothers can benefit from proper prenatal care. It may improve the chances of having a healthy baby and going through a normal, vaginal delivery. This can in turn reduce the cost of delivery. The following are some basic prenatal care tips offered by the American College of Obstetricians and Gynecologists: U Nutrition. First, when attempting to become pregnant, talk to your doctor about taking some prenatal vitamins, which are a good nutritional supplement during pregnancy. Without a proper diet during pregnancy, the growth of the baby could be adversely affected. Choose a diet that gives needed calories and nutrients. Eat a variety of foods from the Food Guide Pyramid, which was developed by the U.S. Department of Agriculture. Consider eating smaller meals more frequently throughout the day, remembering to drink plenty of water and other non-caffeine liquids. Your physician can help develop a specific diet plan. Weight Gain. Once pregnant, extra calories will be needed each day to stay healthy and help the baby grow. This means gaining a certain amount of weight, depending on your weight before pregnancy. The doctor should discuss proper weight gain with you along with nutrition. 6 U U Exercise. Proper exercise can help prepare you for delivery and lessen some discomforts during pregnancy. It can even give you more energy and make you feel better. Some good activities in moderation include walking, swimming and prenatal exercise classes. Although exercise is important, discuss options with your doctor before engaging in any exercise while pregnant. Doctor’s visits. It is important to make all prenatal care visits with your doctor. If an appointment is missed, reschedule another one right away. Don’t wait until next month’s visit. Concerns during pregnancy. You may have certain concerns during pregnancy that you should think about or deal with. Here are some topics you may want to discuss with your doctor: * * * * * Breast-feeding Home life Work Travel Domestic abuse * Elective delivery (doctor/hospital or certified midwife) * Emotional changes * Sexually transmitted diseases U U IV. Why Hospital Charges Vary The charges in this report should be used only as a guide since they vary depending upon a variety of factors. Listed in this section are some common reasons why hospital charges may vary. Type of Payer Charges paid to the hospital vary depending upon who pays the bill: Patients with insurance coverage. Patients with insurance pay discounted, capitated or customary and usual fees negotiated by their insurance company, HMO or PPO. Many policies only provide partial coverage. Patients covered by government programs. Medicaid and sometimes Medicare reimburse hospitals at preestablished rates for services rendered. Patients who are under insured or lack insurance and do not qualify for Medicaid. The unpaid portion of a bill is absorbed by the hospital and paid, in part, through rates charged to those who have coverage. 7 Cross Subsidization Charges may vary among hospitals because of cross subsidization, which is used to help fund care for government-assisted patients, the under insured and uninsured. Cross subsidization, or cost-shifting, means that charges to patients may reflect an amount to help pay for part of the uncompensated care provided by the hospital to the indigent population. The amount that needs to be subsidized will vary from hospital to hospital, depending on the number of indigent patients that particular hospital serves. Other Hospital Level Factors Technology. Not only is medical technology very expensive, but employing highly-trained personnel to administer “state-of-the-art” technology is also costly. Thus, the level of technology in which a hospital invests will affect charges. Types of services offered. Hospitals often offer different levels of care and may “specialize” in one or more types of services. For instance, Level III perinatal centers (hospitals) specialize in high-risk deliveries, and may show higher average charges than other hospitals with obstetrical services because of the expense in handling severe cases. (see page 21 for an explanation of Illinois’ Perinatal Network) Research and teaching hospitals. Although many benefits are returned to the community, providing research and medical training is costly and can influence a hospital’s charges. Business costs. Hospitals must pay for new equipment, construction, maintenance/renovations and malpractice insurance. Increased costs associated with these activities may be reflected in patient charges. Pricing practices. Some facilities charge a flat fee, which includes one charge for all services for a given procedure. Others charge according to actual time, services, consultation and supplies required for the individual patient. Miscellaneous Factors Patient severity of illness. Two people admitted to the hospital for the same condition may require different levels of care due complications, comorbidities and other related difficulties. Length of hospital stay. Longer hospital stays are likely to result in higher charges. 8 Physician practice/treatment patterns. The kinds of diagnostic tests ordered, treatments preferred, etc., vary somewhat from physician to physician and explain, in part, the difference in charges among patients. Geographic area. Labor and other costs vary by geographic area within the state (urban vs. rural) and may influence charges. V. Delivery Guide Results: 1999 Illinois State Totals ‘ ‘ ‘ Discharges .................. 173,121 Average Charge .......... $6,124 Average LOS .............. 2.4 days There were 173,121 deliveries in Illinois during 1999. The average charge per discharge for all deliveries was $6,124, and the average length of stay (LOS) was 2.4 days. Total charges in 1999 reached nearly $1.1 billion, and total patient days totaled 416,703. The delivery of babies is the most common reason for hospitalization in Illinois, accounting for 11 percent of all inpatient discharges, five percent of total charges, and five percent of total patient days. Age Group Comparisons The most deliveries occurred among mothers age 25-34 at 89,071 discharges (51%). The average charge among this age group was $6,140, and the average LOS was 2.4 days. The next highest number of deliveries occurred with the 18-24 age group at 52,909 discharges. The average charge for this group was $5,827, and the average LOS was 2.2 days. The highest average charge and longest average LOS occurred with the 45 and older age group at $8,370 and 3.3 days respectively. This age group also had the highest Cesarean section rate at 46 percent. The lowest C-section rate occurred among those under age 18 at 12.5 percent. The trend shows that the C-section rate rises with age. The rates for the five age groups were 12.5% (<18), 15.6% (18-24), 21.8% (25-34), 27.3% (35-44), and 46.1% (45+). This DRG had the lowest average charge at $4,684 and the shortest average LOS at 1.9 days. The second most occurring DRG was Cesarean Section WITHOUT Complication and/or Comorbidity(DRG 371) at 25,827 discharges. This type of delivery had an average charge of $9,048 and an average LOS of 3.4 days. The highest average charge occurred with Vaginal Delivery WITH O.R. Procedure Except Sterilization(DRG 375) at $13,908. This is higher than the average charge of $13,186 for Cesarean Section with Complication and/or Comorbidity(DRG 370); however, DRG 375 had only 112 discharges. The longest average LOS occurred with DRG 370 at 4.7 days. The average charge for this payer type was $6,213, and the average LOS was 2.4 days. Medicaid accounted for the next highest number of discharges at 53,469 (30.9%). The average charge to Medicaid for deliveries was $6,204, and the average LOS for Medicaid was 2.3 days. The other payer types by number of discharges are self pay (5,684), selfadministered (4,953), Medicare (525), and other (7,470). The next highest number of deliveries occurred in the city of Chicago (HSA 6) with 47,677 discharges. Together, these two areas had 59% of all deliveries in the state during this year. The least number of deliveries occurred in HSA 10 at 2,467 discharges. There are only five hospitals in HSA 10. S EE FIGURE 1 BELOW. The highest average charges occurred in the two HSAs with the most discharges (HSAs 6 and 7). The average charge in HSA 6 was $7,692, and HSA 7 had an average charge of $6,714. The lowest average charge occurred in HSA 5 at $3,915, almost half of the Chicago average charge. SEE FIGURE 2 BELOW, AND REFER TO PAGE 1 FOR HEALTH SERVICE AREA AND COUNTY LOCATION . Figure 2: There was little differences in average LOS for deliveries this year across the state. The average LOS for the state as a whole was 2.4 days. The longest average LOS occurred in HSA 2 at 2.7 days, while the shortest LOS occurred in HSA 8 at 2.1 days. The rate varied among the regions of the state during this year. The highest rate occurred in HSA 1 at 23.1%, followed by HSA 2 at 22.8%. Historically, HSA 11 has had the highest Cesarean section rate in the state, but this year it was third highest at 22.4%. The lowest rate occurred in the city of Chicago (HSA 6) at 18.7%, followed by HSA 10 at 19.5%. S EE FIGURE 4 BELOW, AND REFER TO PAGE 1 TO HEALTH S ERVICE AREA AND COUNTY LOCATION . Figure 4: 14 VI. Deliveries in Illinois - Summary of State Trends: 1990-1999 Discharges There were 173,121 total inpatient deliveries at Illinois hospitals in 1999, a decrease of 320 from 1997 when there were 173,441. Inpatient discharges for deliveries had decreased steadily from 1990 to 1999, with increases occurring only in 1991 and 1998. Discharges have decreased, on average, about 4% per year from 1990 to 1999. SEE FIGURE 5 BELOW. There were 138,042 vaginal deliveries in 1999, a decrease of 1,653 from 1998. Except for an increase in 1998, the number of vaginal deliveries has been decreasing steadily from a high point of 142,073 in 1994. Prior to that, vaginal deliveries had been increasing since 1990. While vaginal deliveries decreased about 1.2% in 1999, Cesarean section deliveries increased nearly four percent that year. In 1999, there were 35,079 Cesarean discharges, an increase of 1,333 from the previous year. The number of Cesarean deliveries decreased gradually from 1990 to 1997. Noticeable increases occurred in both 1998 and 1999. Figure 5: 15 Average Charges ($) The average charge for all deliveries in 1999 was $6,124, an 8% increase from 1998 when it was $5,669. Average charges for deliveries have risen steadily since 1990. The average annual percent increase in average charges for deliveries has been about 9.1% from 1990, when the average charge was $3,367. SEE FIGURE 6 BELOW. The average charge for vaginal deliveries was $5,112 in 1999, an 8.1% increase from the previous year. Charges for vaginal deliveries have also risen steadily since 1990, increasing, on average, 10.4% per year. In 1990, the average charge for a vaginal delivery was $2,640. The average charge for Cesarean section deliveries was almost twice that of vaginal deliveries, reaching $10,140 in 1999. This amount represents a 6.1% increase from 1998, when the average charge was $9,554. While slowing down in recent years, the average charge for Cesarean deliveries has increased, on average, 8.4% per year since 1990, when it was $5,783. This represents the fourth year in a row that the average LOS has increased or stayed the same. The lengths of hospital stays for deliveries had been decreasing steadily until 1996, the year that Illinois passed minimum length of stay legislation. SEE FIGURE 7 BELOW. Public Act 89-513, effective September 1996, requires insurance companies to allow mothers to stay in the hospital at least 48 hours following a vaginal delivery and 96 hours after a Cesarean section. The average lengths of stay reported in this guide represent the time period beginning at admission, not delivery. The average LOS for vaginal deliveries was 2.1 days in 1999, an increase of one-tenth of a day from the previous year. Vaginal deliveries reached a low point of 1.6 days in 1995 before gradually rising each year to 1999. The average LOS for a vaginal delivery in 1990 was 2.3 days. The average LOS for Cesarean section deliveries was 3.8 days in 1999, the same as the two previous years. Cesarean deliveries saw dramatic decreases in LOS from 1990 to 1995, dropping a full day to a low of 3.6 days. The average LOS in 1990 was 4.6 days. The Cesarean section rate had dropped significantly from 1990 to 1997 (23.1% and 19.3% respectively), only to increase again the following year. SEE FIGURE 8 BELOW. The U.S. rate followed a similar pattern in the 1990's, dropping from 22.7% in 1990 to 20.7% in 1996. In 1997 the national rate began to increase as the Illinois rate did in 1998. By 1999, it had reached 22%, according to the U.S. Centers for Disease Control and Prevention’s preliminary report. The Illinois rate was actually above the national rate in 1990, before dropping below for good in 1992. Figure 8: * The national cesarean section rates presented in Figure 8 were obtained from the CDC National Center for Health Statistics. 18 VII. ‘ Explanation of Terms General Terms Average Charge per Day ($) The average dollar amount patients within a given DRG are charged per day for a hospital stay. Diagnosis Related Group (DRG) The Diagnosis Related Group (DRG) system organizes ICD-9-CM diagnosis and procedure codes into a complex, comprehensive system. A DRG is one of approximately 500 groups that classify patients into clinically cohesive groups that demonstrate similar consumption of hospital resources and length of stay patterns. Average Length of Stay (LOS) The average number of days patients within a given DRG stay in the hospital. ‘ Obstetrical Terms International Classification of Diseases, 9th Revision, Clinical Modification (ICD-9-CM) ICD-9-CM is a system designed for the indexing of hospital records by disease and operations, and for data storage and retrieval. It is also used to classify morbidity and mortality information for statistical purposes. Total Deliveries Includes both vaginal and cesarean deliveries. (DRGs 370, 371, 372, 373, 374 and 376) Vaginal Deliveries without Complicating Diagnoses Includes normal vaginal deliveries without complicating diagnoses. (DRG 373) Health Services Area (HSA) An HSA is one of 11 areas of the state designated by the federal government for health planning purposes. Vaginal Deliveries with Complicating Diagnoses or O.R. Procedure Includes vaginal deliveries with complicating diagnoses, or followed by tubal ligation, dilation & curettage or other operating room procedure. (DRGs 372, 374 and 375) Complication and/or Comorbidity Some DRGs will include complication and/or comorbidity in their titles. Complication is defined as a condition that arises during a hospital stay that prolongs the length of stay by at least one day in approximately 75 percent of the cases. Comorbidity is defined as a preexisting condition that, because of its presence with a specific diagnosis, causes an increase in length of stay by at least one day in approximately 75 percent of the cases. Cesarean Deliveries and Cesarean Percentage A delivery by means of incision into the uterus. The cesarean percentage shows cesarean deliveries as a percentage of all deliveries. (DRGs 370 and 371) Primary Cesarean and Primary Cesarean Percentage Average Charge ($) The average dollar amount patients within a given DRG are charged for a hospital stay. Actual payments are likely to be less than charges. A cesarean section performed on a woman who has not had a previous cesarean. (Primary cesarean deliveries are identified by the absence of ICD-9 diagnosis code 654.2, which indicates the presence of a uterine scar from a previous cesarean delivery). 19 Repeat Cesarean and Repeat Cesarean Percentage A cesarean section performed on a woman who has had a previous cesarean. The repeat cesarean percentage shows repeat cesarean deliveries as a percentage of all deliveries. (Repeat cesarean deliveries are identified by the presence of ICD-9 diagnosis code 654.2, which indicates the presence of a uterine scar from a previous cesarean delivery). About Table A The hospital-specific table (A) presents charge and length of stay information from hospitals that reported inpatient discharges for deliveries, occurring in 1999, to the Council. While a few hospitals without obstetrical services reported deliveries, only data from those with obstetrical services are shown in this report. Hospitals in the table without obstetrical services are listed with a zero (0) next to their names. To protect patient confidentiality, the Council has suppressed certain low numbers in this guide. Those deliveries reported from hospitals without obstetrical services are very few and are most likely emergency deliveries. For discharge numbers of three or less, all numbers have been masked with asterisks. For discharge numbers of four through six, the discharge number has been masked but leaving all other information; caution should be exercised for these charges and lengths of stay because the averages are based on low numbers. The hospitals are listed alphabetically by region and Health Service Area (HSA). Earlier in this report, an HSA map page identified the regions which grouped the hospitals that perform child deliveries. The five regions in order are: Chicago, Illinois Region (HSA 6) Suburban Chicago Region (HSA 7, 8, 9) Northwest Illinois Region (HSA 1, 10) Central Illinois Region (HSA 2, 3, 4) Southern Illinois Region (HSA 5, 11) Listed in the table for each hospital are the average charge, average charge per day, average length of stay (LOS) and number of discharges for deliveries. Across the top of the tables are deliveries broken down by type. In the tables, the six delivery Diagnosis Related Groups (DRG) are grouped into the following column headings: ö Total Deliveries DRG 370 - 375 Vaginal Birth After Cesarean Section (VBAC) A vaginal delivery occurring after a previous cesarean section. In this report, VBAC deliveries are reported by the actual number of discharges and as a proportion of all deliveries to women who had a previous cesarean. To calculate the VBAC rate, multiply the proportion by 100. (VBACs are vaginal deliveries identified by the presence of ICD-9 diagnosis code 654.2) VIII. Hospital Comments All hospitals included in this report were given the opportunity to review their own information for accuracy. One hospital responded and submitted written comments to the Council’s Springfield office. They were: ” Glenbrook Hospital, Glenview Their comments are on file and can be obtained by contacting the Council’s Research Unit at (217) 786-7001. IX. About the Hospital Table (Table A) Before you turn to the hospital table found on the following pages, please take a moment to read the following which will assist you in the use and interpretation of the table. Should any terms not be familiar, please refer to the Explanation of Terms (Section VII) on pages 19 and 20. 20 ö Vaginal Deliveries without Complicating Diagnoses DRG 373 Vaginal Delivery without Complicating Diagnoses resources. The statewide system consists of 10 regional networks (six in Chicago and four downstate), each coordinated by a designated regional perinatal center comprised of one or more Level III hospitals. Hospitals in the table with an asterisk (*) next to their names are regional perinatal centers for that network. Hospitals within each network are designated by the level of care they can provide. The levels are as follows: Level I: Level II: Provides routine uncomplicated care. Refers highrisk mothers and infants to specialized facilities. Provides routine care. Provides care for most common conditions encountered in mothers and infants. Provides routine care. Provides care for most common conditions encountered in mothers and infants. Provides some aspect of high-risk care, usually neonatality services. Provides full range of care from routine to very advanced intensive care. ö Vaginal Deliveries with Complicating Diagnoses or Operating Room Procedure DRG 372 Vaginal Delivery with Complicating Diagnoses DRG 374 Vaginal Delivery with Sterilization and/or D&C DRG 375 Vaginal Delivery with O.R. Procedure Except Sterilization ö Cesarean Section Deliveries DRG 370 Cesarean Section with Complication and/or Comorbidity DRG 371 Cesarean Section without Complication and/or Comorbidity Other column headings include total, primary and repeat cesarean section rates, and vaginal births after cesarean (VBAC). Refer to Section VII for further explanation of these columns. Please note that with the data entries for Roseland Community Hospital, Chicago, the average charges have been masked due to a hospital system conversion problem with calendar year 1999 data reported by Roseland Community Hospital. The data could not be corrected in time once the quarterly verification process was completed. The Illinois Regional Perinatal Network All Illinois hospitals that provide maternity and newborn care belong to a regional perinatal network developed by the Illinois Department of Public Health. Every hospital with obstetrical services belongs to this network. Established in the mid-1970's, this system has three main objectives: 1. To assure that women and infants have access to quality pregnancy-related health services, despite financial status. To reduce maternal, fetal and neonatal mortality and morbidity. To make efficient use of available health care Level II+: Level III: Each network has established rules for consultation, referral and transfer of patients to facilities with the appropriate level of care. Each hospital’s perinatal designation is listed next to its name in Table A. It is important to remember the Level III hospitals routinely handle high-risk deliveries, requiring costlier, more intensive services than normal deliveries. This is one reason that Level III facilities may show higher average charges when compared to other hospitals. 2. 3.
