‫اﺳﺘﻤﺎرة ﻣﺮاﺟﻌﺔ اﻟﺘﻘﻴﻴﻢ‬ ‫ﻣﻜﺘﺐ ال ﺗﺮﺑﻴﺔ اﻟﺨﺎﺻﺔ ﺑﺎﻟﻤﻌﺎﻗﻴﻦ‬ ‫وﺧﺪﻣﺎت اﻟﺘﺪﺧﻞ اﻟﻤﺒﻜﺮ‬ ‫1 ﺗﻤﻮز )ﻳﻮﻟﻴﻮ(، 5002‬ ‫ﻣﺮاﺟﻌﺔ اﻟﺘﻘﻴﻴﻢ‬ ‫ﺗﺎرﻳﺦ اﻟﻤﺮاﺟﻌﺔ‬ ‫ﺗﺎرﻳﺦ اﻟﻮﻻدة‬ ‫إﻧﻬﺎء اﻷهﻠﻴﺔ‬ ‫رﻗﻢ هﻮﻳﺔ اﻟﺘﻠﻤﻴﺬ‬ ‫إﻋﺎدة اﻟﺘﻘﻴﻴﻢ‬ ‫اﺳﻢ اﻷب اﻻول‬ ‫اﻟﺘﻘﻴﻴﻢ اﻹﺑﺘﺪاﺋﻲ‬ ‫اﻻﺳﻢ اﻷول‬ ‫اﺳﻢ ﻋﺎﺋﻠﺔ اﻟﺘﻠﻤﻴﺬ‬ ‫_______________| _________|______________|______________|_______________|_____________________‬ ‫إن هﺪف هﺬﻩ اﻟﻤﺮاﺟﻌﺔ هﻮ اﻟﺘﺨﻄﻴﻂ ﻟـ:‬ ‫اﻟﻤﺸﺘﺮآﻴﻦ ]443.003 § ‪[34RFC‬‬ ‫إﺷﺘﺮك اﻻﻓﺮاد اﻟﻤﺪرﺟﺔ اﺳﻤﺎ ؤ هﻢ ﻓﻲ اﻷﺳﻔﻞ ﻓﻲ ﻣﺮاﺟﻌﺔ هﺬا اﻟﺘﻘﻴﻴﻢ. ﻳﺠﺐ ﺗﺪوﻳﻦ اﺳﻤﺎء اﻟﻤﺸﺘﺮآﻴﻦ اﻹﺿﺎﻓﻴﻴﻦ وإرﻓﺎﻗﻬﺎ ﺑﻬﺬﻩ‬ ‫اﻻﺳﺘﻤﺎرة‬ ‫])‪.[20 USC §1414 (d)(1)(B‬‬ ‫_____________________________________‬ ‫___________________________________________________________‬ ‫ﻣﻤﺜﻞ ﻣﺪﻳﺮﻳﺔ اﻟﺘﻌﻠﻴﻢ أو اﻟﺸﺨﺺ اﻟﻤﻌﻴﻦ‬ ‫اﻟﺘﻠﻤﻴﺬ )ﻋﻨﺪﻣﺎ ﻳﻜﻮن هﺬا ﻣﻼﺋﻤً(‬ ‫ﺎ‬ ‫_____________________________________‬ ‫___________________________________________________________‬ ‫ﻣﺪّس ﺗﺮﺑﻴﺔ ﻋﺎﻣﺔ‬ ‫ر‬ ‫أﺣﺪ اﻟﻮاﻟﺪﻳﻦ‬ ‫_____________________________________‬ ‫___________________________________________________________‬ ‫ﻣﺪّس أو ﻣﺰ ّد ﺧﺎص ﺑﺎﻟﻤﻌﺎﻗﻴﻦ‬ ‫و‬ ‫ر‬ ‫أﺣﺪ اﻟﻮاﻟﺪﻳﻦ‬ ‫_____________________________________‬ ‫__________________________________________________________‬ ‫ﺷﺨﺺ ﺁﺧﺮ‬ ‫ﺷﺨﺺ ﺑﺈﻣﻜﺎﻧﻪ أن ﻳﻔﺴﺮ أﺑﻌﺎد ﻧﺘﺎﺋﺞ اﻟﺘﻘﻴﻴﻢ اﻟﺘﻌﻠﻴﻤﻴﺔ‬ ‫__________________________________‬ ‫ﺷﺨﺺ ﺁﺧﺮ‬ ‫ﻣﺮاﺟﻌﺔ اﻟﺘﻘﻴﻴﻢ‬ ‫راﺟﻊ ﺟﻤﻴﻊ ﻣﻌﻠﻮﻣﺎت اﻟﺘﻘﻴﻴﻢ اﻟﻤﻮﺟﻮدة ، ﺑﻤﺎ ﻓﻴﻬﺎ اﻟﺘﻘﻴﻴﻤﺎت اﻟﺘﻲ أﺟﺮﻳﺖ ﻋﻠﻰ ﺻﻌﻴﺪ اﻟﺼﻒ اﻟﻤﺪرﺳﻲ واﻟﻘﻄﺎع اﻟﻤﺪرﺳﻲ اﻟﻤﺤﻠﻲ وﻋﻠﻰ‬ ‫ﺻﻌﻴﺪ اﻟﻮﻻﻳﺔ واﻟﻤﻼﺣﻈﺎت اﻟﺘﻲ أﺧﺬت ﻓﻲ داﺧﻞ اﻟﺼﻒ ])‪. 1[H.R. 1350.614(c‬‬ ‫إﻋﻤﻞ وﺻﻔً:__________________________________________________________________________________________‬ ‫ﺎ‬ ‫___________________________________________________________________________________________________‬ ‫راﺟﻊ ﻣﻼﺣﻈﺎت اﻟﻤﺪرس وﻣﺰودي اﻟﺨﺪﻣﺎت اﻟﻤﺘﻌﻠﻘﺔ ])‪. 1[H.R. 1350.614(c‬‬ ‫إﻋﻤﻞ وﺻﻔً:__________________________________________________________________________________________‬ ‫ﺎ‬ ‫___________________________________________________________________________________________________‬ ‫راﺟﻊ اﻟﺘﻘﻴﻴﻤﺎت واﻟﻤﻌﻠﻮﻣﺎت اﻟﺘﻲ ﻳﻮﻓﺮهﺎ اﻟﻮاﻟﺪان])‪. 1[H.R. 1350.614(c‬‬ ‫إﻋﻤﻞ وﺻﻔً:__________________________________________________________________________________________‬ ‫ﺎ‬ ‫___________________________________________________________________________________________________‬ ‫ﺑﻨﺎ ً ﻋﻠﻰ اﻟﻤﺮاﺟﻌﺔ ﻓﻲ اﻻﻋﻠﻰ وﻋﻠﻰ ﺣﺎﺟﺎت اﻟﻄﻔﻞ اﻟﺘﺮﺑﻮﻳﺔ ورأي واﻟﺪي اﻟﺘﻠﻤﻴﺬ، ﺣﺪد اﻟﻤﻌﻠﻮﻣﺎت اﻹﺿﺎﻓﻴﺔ اﻟﻤﻄﻠﻮﺑﺔ ﻟﺘﻘﺮﻳﺮ‬ ‫ء‬ ‫])‪:1[H.R. 1350.614(c‬‬ ‫1. ﻣﺎ إذا آﺎن اﻟﺘﻠﻤﻴﺬ ﻳﻌﺎﻧﻲ ﻣﻦ اﻹﻋﺎﻗﺔ2 أو ، ﻓﻲ ﺣﺎﻟﺔ إﻋﺎدة اﻟﺘﻘﻴﻴﻢ، ﻣﺎ إذا آﺎن اﻟﺘﻠﻤﻴﺬ ﻻ ﻳﺰال ﻳﻌﺎﻧﻲ ﻣﻦ اﻹﻋﺎﻗﺔ3. اﻋﻤﻞ وﺻﻔً ﺑﺎﻟﺘﻘﻴﻴﻢ‬ ‫ﺎ‬ ‫3‬ ‫واﻟﻤﻌﻠﻮﻣﺎت اﻹﺿﺎﻓﻴﺔ اﻟﻼزﻣﺔ‬ ‫__________________________________________________________________________________________‬ ‫إذا وﺟﺪ ﻓﺮﻳﻖ ﺑﺮﻧﺎﻣﺞ اﻟﺘﻌﻠﻴﻢ اﻟﻔﺮدي)‪ (IEP‬أن ﻟﻴﺲ هﻨﺎك ﺣﺎﺟﺔ ﻟﻤﻌﻠﻮﻣﺎت إﺿﺎﻓﻴﺔ ﻟﺘﺤﺪﻳﺪ ﻣﺎ إذا آﺎن اﻟﺘﻠﻤﻴﺬ ﻳﻌﺎﻧﻲ ﻣﻦ اﻹﻋﺎﻗﺔ وﻻ ﻳﺰال‬ ‫ﺑﺤﺎﺟﺔ ﻟﻬﺬا اﻟﻨﻤﻂ ﻣﻦ اﻟﺘﺮﺑﻴﺔ، ﻳﺠﺐ إﻋﻄﺎء اﻟﺴﺒﺐ:‬ ‫_____________________________________________________________________________‬ ‫_________________________________________________________________________________________________‬ ‫ﺎ‬ ‫2. اﻟﻤﺴﺘﻮﻳﺎت اﻟﺤﺎﻟﻴﺔ ﻟﻺﻧﺠﺎز اﻻآﺎدﻳﻤﻲ واﻻداء اﻟﻮﻇﺎﺋﻔﻲ وﺣﺎﺟﺎت اﻟﻨﻤﻮ اﻷﺧﺮى ﻟﻠﺘﻠﻤﻴﺬ. اﻋﻤﻞ وﺻﻔً ﺑﺎﻟﺘﻘﻴﻴﻢ واﻟﻤﻌﻠﻮﻣﺎت اﻹﺿﺎﻓﻴﺔ اﻟﺘﻲ ﻻ‬ ‫ﺗﺰال ُﺤﺘﺎج إﻟﻴﻬﺎ3 :__________________________________________________________________________________‬ ‫ﻳ‬ ‫__________________________________________________________________________________________________‬ ‫3. ﻣﺎ إذا آﺎن اﻟﺘﻠﻤﻴﺬ ﻳﺤﺘﺎج ﻟﻠﺘﺮﺑﻴﺔ اﻟﺨﺎﺻﺔ ﺑﺎﻟﻤﻌﺎﻗﻴﻦ واﻟﺨﺪﻣﺎت اﻟﻤﺘﻌﻠﻘﺔ أو، ﻓﻲ ﺣﺎﻟﺔ إﻋﺎدة اﻟﺘﻘﻴﻴﻢ، ﻣﺎ إذا آﺎن اﻟﺘﻠﻤﻴﺬ ﻻ ﻳﺰال ﻳﺤﺘﺎج ﻟﻠﺘﺮﺑﻴﺔ‬ ‫اﻟﺨﺎﺻﺔ ﺑﺎﻟﻤﻌﺎﻗﻴﻦ واﻟﺨﺪﻣﺎت اﻟﻤﺘﻌﻠﻘﺔ. اﻋﻤﻞ وﺻﻔً ﺑﺎﻟﺘﻘﻴﻴﻢ واﻟﻤﻌﻠﻮﻣﺎت اﻹﺿﺎﻓﻴﺔ اﻟﺘﻲ ﻻ ﺗﺰال ُﺤﺘﺎج إﻟﻴﻬﺎ3 : __________________‬ ‫ﻳ‬ ‫ﺎ‬ ‫__________________________________________________________________________________________________‬ ‫4. ﻣﺎ إذا آﺎن هﻨﺎك ﺣﺎﺟﺔ ﻟﻺﺿﺎﻓﺎت واﻟﺘﻌﺪﻳﻼت ﻟﺒﺮﻧﺎﻣﺞ اﻟﺘﺮﺑﻴﺔ اﻟﺨﺎص ﺑﺎﻟﻤﻌﺎﻗﻴﻦ واﻟﺨﺪﻣﺎت اﻟﻤﺘﻌﻠﻘﺔ ﻟﺘﻤﻜﻴﻦ اﻟﺘﻠﻤﻴﺬ ﻣﻦ إﻧﺠﺎز أهﺪاﻓﻪ‬ ‫اﻟﺴﻨﻮﻳﺔ اﻟﺘﻲ ﻳﻤﻜﻦ ﻗﻴﺎﺳﻬﺎ واﻟﻮاردة ﻓﻲ ﺑﺮﻧﺎﻣﺞ اﻟﺘﻌﻠﻴﻢ اﻟﻔﺮدي )‪ (IEP‬اﻟﻤﻌﻴﻦ ﻟﻠﺘﻠﻤﻴﺬ وإﺣﺮاز اﻟﺘﻘﺪم، آﻤﺎ ﻳﻼﺋﻢ، ﻓﻲ اﻟﻤﻨﻬﺎج اﻟﺘﺮﺑﻮي اﻟﻌﺎم.‬ ‫اﻋﻤﻞ وﺻﻔً ﺑﺎﻟﺘﻘﻴﻴﻢ واﻟﻤﻌﻠﻮﻣﺎت اﻹﺿﺎﻓﻴﺔ اﻟﻼزﻣﺔ3 : _______________________________________________________‬ ‫ﺎ‬ ‫__________________________________________________________________________________________‬ ‫ﻳﺘﻄﻠﺐ ﻗﺎﻧﻮن ﺗﻌﻠﻴﻢ اﻷﻓﺮاد اﻟﻤﻌﻮﻗﻴﻦ )‪ (IDEA‬ﻟﻌﺎم 4002 ﻣﺮاﺟﻌﺔ اﻟﻤﻌﻠﻮﻣﺎت ﻓﻲ اﻷﻋﻠﻰ. اﻟﻮﺻﻒ اﻟﻜﺘﺎﺑﻲ إﺧﺘﻴﺎري.‬ ‫إﺗﺒﻊ إﺟﺮاءات ﻓﺮﻳﻖ اﻟﺘﻌﻠﻴﻢ اﻟﻤﺘﻌﺪد اﻟﻤﻨﺎهﺞ )‪ (MET‬اﻟﺤﺎﻟﻴﺔ ﻓﻲ ﺟﻤﻴﻊ اﻟﺘﻘﻴﻴﻤﺎت اﻻﺑﺘﺪاﺋﻴﺔ ﻋﻨﺪ ﺗﺤﺪﻳﺪ أهﻠﻴﺔ اﻟﺘﺮﺑﻴﺔ اﻟﺨﺎﺻﺔ ﺑﺎﻟﻤﻌﺎﻗﻴﻦ.‬ ‫إذا آﺎﻧﺖ ﻻ ﺗﻮﺟﺪ ﺣﺎﺟﺔ ﻟﻠﺘﻘﻴﻴﻢ أو اﻟﻤﻌﻠﻮﻣﺎت اﻹﺿﺎﻓﻴﺔ، اآﺘﺐ "ﻻ ﺷﻲء".‬ ‫1‬ ‫2‬ ‫3‬
