HOW TO USE THE BMV 3772 FORM
We have been receiving numerous questions related to the use of the BMV 3772 form from Ohio dealers’ as well as Clerks of Court Title Offices. Can a BMV 3772 form be used with or without an Ohio title? • • When using the form with a physical title, the back of the Ohio title still has to be completed but the BMV 3772 has an optional provision for the owner to appoint a power of attorney. When using the form with an electronic title the BMV 3772 can be used in its entirety. There is a provision for a dealer to use this form to or from a dealer when making assignments; an owner can appoint a power of attorney, or to state federal mileage; and it can be use as an application for a certificate of title.
