INSTRUCTIONS FOR COMPLETING AFFIDAVITS AND AUTHORIZATION The following three documents have significant consequences and must be completely and accurately filled out in order to process your application: 1) Eligibility Affidavit; 2) Knowledge of Laws Affidavit; and 3) Authorization for Release of Records.
Please complete the following steps in filling out these forms: 1) At the top of each form, fill in the name of the county you are in and print your name. 2) On the Eligibility Affidavit and the Authorization for Release of Records forms, please read each line carefully, and in your own handwriting, place your initials in the space provided to the left of each numbered line. By initialing each line on the Eligibility Affidavit, you are certifying that each line on the information on that line is true and correct. By initialing each line on the Authorization for Release of Records, you are giving your express permission for the organization or individual who holds those records to release them to the Texas Department of Public Safety for the sole purpose of investigating your fitness to carry a concealed handgun, or to be a concealed handgun instructor. 3) On each form, in the presence of a notary public, please sign your name, and then have the notary fill in the date and sign his/her name and apply an inked notary’s seal. It is imperative that the date, notary’s name, and seal are made in ink so that they can be photocopied. Notary expiration dates must not be handwritten in. 4) To avoid a delay in processing your applications, please submit all three affidavits with the remaining application materials simultaneously: a. Photos, see certified instructor or visit local photo shop; b. Photo Holder, signed, provided by certified instructor; c. Fingerprint Cards, provided by certified instructor; d. Training Form issued by certified Instructor; e. Application: i. On-line application: If you submitted an application on-line, an application confirmation statement will be mailed to you from the department. You must sign the statement and return with all application materials at the same time. ii. If you prefer to submit application by mail, you must obtain an original yellow application form, provided by certified instructor. f. Fee: i. On-line application: Fee not necessary, previously submitted with web application. ii. Mailed application: Please check instructions provided with the application packet for specific applications fees. Fee must be submitted by cashier’s check or money order only, personal check or cash will not be accepted. You may obtain a yellow application form through your certified training instructor. AUTHORIZATION FOR RELEASE OF RECORDS AFFIDAVIT

BEFORE ME, the undersigned authority, personally appeared _______________________________________ who made this Affidavit and, on oath, stated the following: I have read the provisions of Chapter 411, Texas Government Code and 37 Texas Administrative Code 6.15(6)(C) and hereby authorized the release of information and records to the Texas Department of Public Safety for the purpose of a background investigation to determine my eligibility to carry a concealed handgun or to instruct classes on concealed handguns. These documents may include, but are not limited to: INITIAL ______1. Medical records within 5 years preceding the date of application pertaining to chemical dependence or ability to exercise sound judgment with respect to the proper use and storage of a handgun; ______2. Child support payment status as evidence by court documents or records from the attorney general’s office; ______3. Payment status of taxes or other money collected by the comptroller, state treasurer, tax collector of a political subdivision of the state, Texas Alcoholic Beverage Commission, or any other agency or subdivision of the state as evidence by tax or agency records; ______4. Law enforcement records; ______5. Payment status of any loan made under Chapter 57, Texas Education Code as evidenced by agency records; ______6. Current restrictions under court protective order. I understand that any confidential documents obtained by the Texas Department of Public Safety during its investigation of my application for a concealed handgun license will remain confidential.
