This form can be filled out on-line. Print TWO copies: one to sign and submit for processing and one for your records. Attach the fee and/or documentation to the application and mail to the address above. Incomplete applications will be returned unprocessed and result in a $25.00 charge. Additional information is available on the web site or from the Education Advisor. Include a $95.00, non-refundable, fee paid by a cashier's check or money order made payable to the Georgia Real Estate Appraisers Board. The Director must attach a Georgia Criminal Information Center report (GCIC) if a Georgia Resident. If you are not a resident of Georgia, attach an equivalent report from your state of residency. This report must not be more than 60 days old. A report from a party other than a law enforcement agency is not acceptable. Any incident listed on the report should receive clearance. In addition, you must disclose any conviction, nolo contendere plea, or first offender sentence not reflected on the report. Attach supporting documentation, if required, from Background Section C. Documentation Requirements
(See Chapters 539-2-.02 (1) and 539-5-.01, and the regulations noted with each item below. Applicants may view the Board's regulations under "Online Forms" at www.grec.state.ga.us.) . For more information on each of the items below, see Chapters 1 and 2 of the School Policies and Procedures Manual available under "Forms and Applications" at the Commission's website. Attach information in the form of detailed statements for the following:
(items d, f, g, & o may not apply to independent-study, computer-based courses):
(a) a detailed proposal of records management for retaining for at least five years records which will reflect the attendance and all scores earned by a student on all graded exercises and examinations (539-2-.08); (b) if subject to the Nonpublic Post secondary Educational Institutions Act of 1990, O.C.G.A. 20-3250.1 et. seq., a copy of the current certificate issued by the Nonpublic Post secondary Education Commission; (c) a list of all directors and owners of the school including their names and addresses. (i) a detailed outline(s) of courses to be offered in the first year of the school's approval with hours to be spent on each subject area to be covered in the course(s) and all planned classroom and homework exercises. Complete Form V(i) attached to this application (Page 4). For each course which forms all or part of the curriculum for state registered, state licensed, state certified residential, or state certified general real property appraiser, also submit Form D (Page 5 ) which identifies the subject areas to be covered in the course and the number hours for each subject area (539-2-.03); (j) a bibliography of all texts and reference materials for use in the course(s); (k) a comprehensive description of the measures and standards it will employ to evaluate student performance in order to determine whether a student successfully completes a course and copies of all final examinations for classification courses together with proposed answers (539-2-.03, 539-2-.04); (l) a description of the learning objectives for each instructional hour of the course(s) (what the student should know or be able to do upon mastery of the content of each hour of study); (m) a list of proposed instructors and/or applications for new instructors of classification courses (539-2-.06, and 539-5-.01): (n) a schedule of planned offerings of the course(s) for the first calendar year for which the school seeks approval including the date, time, and place of any offering; (o) the fee schedule planned for the course offering(s) during the first year; (p) a copy of the Notice to Students required by Rule 539-2-.09; (q) the name of a school director and/or coordinator who shall be responsible for certifying student completion of all courses covered under chapter 539-2 {type or print the name(s) on page 1 of this application; indicate the daytime phone number(s) of the director and/or coordinator if these are different from the phone number(s) listed for the school on page 1}; (r) for independent-study, computer-based courses only: a description and documentation of the method by which each element of mastery is to be accomplished {539-2-.14(3)} or a copy of the certificate from the International Distance Education Certification Center (IDECC) for each computer-based course; correspondence courses are not allowed (s) for independent-study, computer-based courses only: a statement that the school will utilize the testing program(s) as described in Rule 539-2-.14(4) or a copy of the certificate from the International Distance Education Certification Center (IDECC) for each computer-based course; correspondence courses are not allowed (t) a copy Section Eof the Georgia Crime Information Center report (GCIC) on the director's criminal history must accompany all applications. Classification Courses (R. L, CR, & CG) must be a minimum of 15 credit hours and include an examination. List Continuing Education Courses separately, even if the course will be offered as a classification course with examination during the same class session. In other words, if a school plans to offer a 15 hour classification course with examination, and give credit for 14 hours continuing education to students who do not need classification credit and who do not complete the examination, then the continuing education course must be listed separately below. Both the continuing education course and the classification course may have the same title. The Board will issue a separate course code for the continuing education course, and a school must submit a separate course roster letter for the classification course credit and the continuing education credit courses.
