Administrative Office Tuition Reimbursement Agreement 2008-2009 School Year Name _____________________________________ School Current Position/Teaching Area Degree Pursued College / University / Community College A contract must be submitted each year. The Davie County Schools agree to a tuition reimbursement pending availability of funds for work to complete certification requirements or a master’s degree. Reimbursement will be granted for up to 12 semester hours per employee. There will be no reimbursement for travel, books, or other expenses. Special provisions may be granted for required certification through the Human Resources office. Grade Requirements for Reimbursement: A grade of a “C” or better must be earned for undergraduate courses. A grade of a “B” or better must be earned for graduate courses. I, _______________________, will provide documentation. Prior to taking the course: An In-Service Form After Course Completion: A Reimbursement Form (Blue) (signed by principal, supervisor, or staff development coordinator), proof of payment, and an official copy of your grade for the course to the HRMS Data Manager within 30 days of completion of each individual course. I, _______________________, agree to work for the Davie County Schools for a minimum of two (2) years after completion of my degree or refund the tuition reimbursement paid by the Davie County School System. If I, _________________________, resign before completing the degree, I agree to reimburse the Davie County Schools the amount paid toward tuition.
