Alternative Format Agreement Form In accordance with applicable federal and state law, as well as the Drexel University’s Office of Disability Services (ODS) policy and procedure, individuals with disabilities must agree to the following in order to receive required reading materials in alternative formats. I, , agree to abide by the following rules in using alternative formats at Drexel University and understand that by completing the Alternative Format Agreement, I affirm that I have read, or have had this agreement read to me, and understand the following: • I have a documented disability on file in the ODS at Drexel University and am eligible to use alternative formats as a reasonable accommodation due to my disability. • I have read, or have had the enclosed Chafee Amendment read to me, and agree to abide by the federal law as it applies to copyright law. • I am enrolled in the courses for which the required reading materials are being requested or have collaborated with my supervisor to determine required reading materials necessary to perform the essential functions of my position. • I understand that in the pursuit of the appropriate format, individual preferences will be given priority, but other appropriate formats may be provided by the ODS. • I understand the ODS will process requests for alternative formats on a first come, first serve basis. • I understand I am responsible for picking up the alternative format in the ODS. • I will submit requests for specific alternative formats for required reading materials converted internally. I understand that the binding of textbooks may need to be removed in order to convert the material. • I agree that I will not duplicate the alternative format. • I understand that if I do not return the alternative format on loan from the ODS within ten University days after the end of each term/semester, a Hold will be placed on my University account. • I agree to abide by all of the policies and procedures of the ODS, as they applies to receiving alternative formats as an accommodation. University days means days when the University is open for normal business operations
