Welcome to the Monitoring Staff Turnover Calculator. This easy-to-use template is a mechanism for tracking and
monitoring monthly turnover for CNAs, RNs, and LPN/LVNs. This workbook will also help nursing home staff prepare
entries for submission of turnover data for Goal #1 (Staff Turnover) on the
Advancing Excellence in America's Nursing Homes website:

This workbook contains ten [10] worksheets to assist you in your turnover calculations
and monitoring. Each worksheet can be accessed by

Common Qs & As
Q: What is a termination?
A: An employee departure, either due to firing or quitting, whereby the employee receives a final paycheck.
Q. Where can I find the data to fill out this Calculator?
A. Often, the organization has a separate form that must be completed for each termination. The periodic payroll reports
also may list terminations.
Q: Why does a part-time or per diem Staff termination count equally as a full-time Staff termination?
A: The calculation is measuring turnover of all Nursing Staff regardless of individual employment status.
Q: What about a member of the Nursing Staff who changes his/her job title and stays in the organization;
does this count as a termination if the employee is still a member of the Nursing Staff?
A: No. Do not count this individual as a termination.
Q: What about a member of the Nursing Staff who changes his/her job title and stays in the organization;
does this count as a termination if the employee leaves the Nursing Staff?
A: No. Do not count this individual as a termination.
Q: What if a Nursing Staff member works at two facilities owned by the same corporation and leaves one of the facilities,
but stays at the other?
A: The Staff member would be coded as a termination by the facility from which he/she departed.
Q: What if a Nursing Staff employee cuts his/her hours from full-time to per diem? Does that count as a termination?
A: No. He/She is still employed by the facility.
Q: What about a Nursing Staff employee who goes on unpaid leave?
A: He/She is still employed and would continue to be included in the denominator.
Common Qs & As
Q: What if a Nursing Staff employee leaves via a termination, but then is re-hired 3 weeks later?
A: He/She would count as a termination.
Q: If a home has 30 budgeted Staff positions, but only 25 of the positions are filled, is the number of
Staff 30 or 25?
A: The number of positions is 25.
Q: What about Agency Staff? Do they count in any of the calculations?
A: No. Agency Staff are not employed by the Nursing Home and therefore are not included.
Q: Does the turnover period need to be a calendar year? Must the year start in January?
A: For the purposes of Phase 2 of the Advancing Excellence Campaign, turnover periods are based on a calendar year.
Q: On the 'CNA Calc' or 'RN Calc' or 'LPN LVN Calc,' where do I enter data?
A: Data is entered in yellow cells only.
Q: Why is the Trending Graph not showing any trends?
A: Be sure data values have been entered in the yellow columns.
