NOTICE TO ALL FAUQUIER COUNTY GOVERNMENT AND PUBLIC SCHOOL USING
DEPARTMENTS: This contract is the result of a competitive bid program and its use must
follow the FCG&PS Procurement Policy/Procedures for the purchase of the commodity listed
herein. Please see the reverse side of this notice for further instructions regarding this
contract.

                                            INSTRUCTIONS

1.      Requesting Services: The Owner(FCG&PS Using Departments) will use the Excavation &
        Hauling Contract Proposal Sheet (CPS) to notify Contractor(s) of requirements on an as-
        needed basis. As noted on the CPS, the Owner will indicate the scope of services required
        and the time limit for the project. The Contractor(s) shall meet with the Owner Project
        Manager(s) within twenty-four (24) hours after notification to discuss and review requirements
        for any excavating and/or hauling project, only if further clarification is needed. The
        Contractor(s) shall respond to any Owner request within the time specified, regardless of the
        size and dollar value of the project. The Contractor(s) shall forward their written proposal for
        the project in the area provided on the CPS, based on the contract pricing, within three (3)
        days of receipt of the CPS. Written proposals shall specify each major step or phase of the
        work to be performed and include a written detailed estimate of the costs to complete the
        project, including labor, materials if requested, and a forecast date of completion, expressed
        either as a definite date or the number of days after notification to proceed with the work.
2.      Orders: All FCG&PS Using Departments must order services listed by issuing FCG&PS
        Purchase Orders per FCG&PS Procurement Procedures Manual. An extra copy of the
        Procedures Manual can be obtained by calling Procurement at (540) 428-8717 or 8720.
        The applicable contract number, vendor number, estimated total dollar amount (can be done
        as a “Not to exceed” estimated figure), contact person with phone number, and
        billing/delivery address must be shown on each purchase order for Finance & Contractor
        use.
3.      Inspection of services and approval of contractor’s invoice is the responsibility of the
        receiving using department.
4.      Any complaint as to quality of services, faulty or delinquent performance, or violation of
        contract provisions by contractor shall be reported to FCG&PS Procurement for handling with
        the Contractor. All complaints must be submitted in writing and can be forwarded to
        Procurement via fax or courier.
5.      Renewals: As stated on the face of this notice, four one-year renewals remain on this contract.
        Contract renewal questionnaires will be sent to the appropriate using department
        approximately sixty days in advance of the expiration date of the current term for input on
        renewing the contract.
6.      Contract Prices: The firm, fixed pricing stated in Section I includes the services, an operator
        where indicated, and equipment as specified in the contract documents. The firm, fixed
        pricing stated in Section II includes equipment and a qualified operator, at the rates specified
        for the time required.
7.      Price Adjustments: Changes to Excavating & Hauling prices will be negotiated only at the
        time of renewal. All price increases must be approved by the contract officer. Contract users
        will be sent notification of contract change from the Procurement office as official notification
        of such changes, if approved.
