2011 OPERATING BUDGET PREPARATION INSTRUCTIONS
Based on preliminary numbers we are looking at 2% decrease. Please use
this information when preparing your budget request.
Enclosed for your use in preparing your 2011 operating budget are the following printouts and forms:
No Revenue forms are included, please submit your own estimate.
1. Computer printout entitled Detail Ledger in .pdf format
2. Computer printout entitled Budget Worksheet in excel format.
3. Form B-3, Line Item Details , now in excel format.
4. Form B-4, Education & Training/Travel Details , now in excel format.
5. Form B-5, Salary Worksheet , now in excel format.
6. Form B-6, Position Allocation Worksheet , now in excel format.
7. Form B-7, New Position Request Form , now in excel format.
8. Form B-8, New Equipment Request, under $5,000 , now in excel format.
9. Form B-9, Capital Equipment Request, over $5,000 , now in excel format.
Following is a brief description of each budget printout/form:
This is a computer printout showing all expenditures from January 1, 2010 through June 30, 2010.
Please use this reports as a reference. This report will show each vendor paid in a particular line item. In the
event you pay a vendor only once a year, remember to include that in your budget request.
This worksheet is to be used for entering your budget request for each line item after completing forms B-3

Form B-3, Line Item Details
A description/explanation should be given to justify any funds requested in any line item. The explanation
should not be a definition of the line item, but a detailed explanation of what you intend to purchase or any
expenditure for any type of service.
Form B-4, Education & Training/Travel Details
A description/explanation should be given to justify any training and/or travel funds requested, including
information regarding the number of conventions, seminars or training classes, mileage, meal and lodging
expense and the names of employees attending.
Form B-5, Salary Worksheet
This form should be used to list each current employee and their job title and to calculate salaries and FICA
for each employee at the rate we are currently paying. The total amounts for salaries and FICA should be
entered on the Budget Worksheet in the
Form B-6, Position Allocation Form
This form should be used to clarify the number of employees you currently have, the number of new
employees you wish to hire and their position and status (either full time or part time).
Form B-7, New Position Request Form
This form should be completed for any new positions being requested. A job description should be included
for each request. The section entitled Additional Costs refers to any additional costs for the position
requested, i.e., office supplies, capital e
Form B-8, New Equipment Request, under $5,000
This form should be completed for any new equipment that will cost less than $5,000 per unit and that has a
useful life of more than one (1) year.
Form B-9, Capital Equipment Request, over $5,000
This form should be completed for any capital request that will cost in excess of $5,000 and has a useful life
of more than two (2) years
