RE:      2010 Nashville’s Top 30 Under 30 Honoree Nomination


The Cystic Fibrosis Foundation is gearing up for our 2010 Nashville’s Top 30 Under 30! This event
recognizes Nashville’s top 30 community leaders who are 30 years old or younger. Nashville’s Top 30
Under 30 Honorees exemplify leadership qualities, maintain active volunteer roles, and excel in the
Middle Tennessee business community. We need your help in finding the top 30 honorees.

Please take a moment to nominate those special persons you feel should be recognized at the September
11, 2010, Nashville’s Top 30 Under 30 Event –Honoring Middle TN’s Outstanding Philanthropic Men
and Women.

The enclosed information explains the event and the criteria used to select the Honorees. Please review
the selection criteria and complete the nomination form. Feel free to nominate multiple individuals.

                         NOTE: Only completed forms will be considered.
                         Forms must be received by Friday, May 14, 2010.

Once completed nomination forms have been received, the Cystic Fibrosis Foundation will notify each
Nominee on or before May 18th. They will then have the opportunity to become a 2010 Nashville’s Top
30 Under 30 Honoree by submitting information regarding their volunteer and community involvement

               2010 Nashville’s Top 30 Under 30

For the second year, the Cystic Fibrosis Foundation (CFF), Middle TN Chapter, will honor 30 men and
women who exemplify leadership qualities, are active in volunteer roles, and excel in the Nashville
business community. Top 30 Under 30 honorees are among the most prominent and successful
individuals in Middle Tennessee and this program spotlights those influential achievers, honoring both
their professional and charitable accomplishments.

About the Event
Nashville’s Top 30 Under 30 begins with a selection panel reviewing nominee questionnaires and
choosing the 30 honorees. The selected young professionals will be notified by June 29 and will meet
each other and learn more about the program at a kickoff event on July 15. Prior to their expected
attendance at the honoree recognition event on Sept. 11, honorees will be featured in local media
outlets and a TV spot. Each honoree must agree to sell 10 event tickets and raise at least $500. The
Honoree who raises the most money will be recognized at the event as the 2010 Top Honoree.

      Be successful in their career field
      Demonstrate current and/or past success in the philanthropic community
      Exhibit dedication to organizations that better the lives of others
      Be no older than 30 on September 1, 2010

Each Honoree Shall:
    Submit nomination acceptance form and questionnaire by Friday, May 28, 2010
    Attend mandatory honoree fundraising meeting and photo shoot on July 15
    Attend Nashville’s Top 30 Under 30 Party on September 11 at the Icon in The Gulch
    Support fellow honorees in their CFF awareness and fundraising campaigns
    Provide contacts lists for which CFF staff can mail invitations to
    Be available for media promotions, including group photo for a billboard and PSA
    Assume ambassador role for 2010 Nashville’s Top 30 Under 30

      Recognition as one of “Nashville’s Top 30 Under 30”
      Opportunity to network and mingle with members of Middle TN’s elite business community
      Inclusion in local media promotions (billboard, TV PSA, print publications)
      One regular event ticket to Nashville’s Top 30 Under 30 party
      Free VIP event ticket upgrade if more than the mandatory ten tickets are sold by August 27
      Opportunity to aid in finding a cure for the more than 35,000 children and young adults suffering
       from cystic fibrosis
      Award recognizing honorees’ contributions to raising awareness and money for CFF
      A VIP honoree swag bag filled with gifts from local retailers
