Monthly Measures Worksheet

This worksheet has been designed to assist with the collection of monthly measures for the Access and Care
Redesign topic. The monthly measures for this topic include: average patient satisfaction score (patient survey),
number of patients whose appointment demands were unmet (unmet demand), number of days to the GP third
available appointment and number of days to the practice nurse third available appointment. This worksheet will

The first tab in this worksheet is a template for the patient survey. This template has been set up to print two survey
sheets to one page. The second tab provides an electronic tally sheet to assist with collating the responses that you

The third tab provides a template for the unmet demand measure. This template has been desinged to print on one
page so that you can use it to record unmet demand over the course of a week.

The fourth tab has the third available appointment calculator, which you can use to determine the third available
appointment for the GPs in your practice. This calculator is repeated in the fifth tab for practice nurses.

Instructions: Each day, enter the number that each patient scored on the patient survey. The spreadsheet will automatically
total the responses for each day and for the entire week. The total is then averaged and the grand total is entered onto the
Online Reporting System (ORS).

Please note that the tally sheet will accept blank responses, therefore, you do not have to enter a response for 15 patients each day.

Instructions: On each day for one week, mark the number of patients who contact the practice (phone,
walk-in, etc.) to make an appointment but do not or cannot make one. Total the number of marks to
determine the number of patients the practice did not see (unmet demand) per week. Enter the weekly
total into the Online Reporting System every month.
