PLANNED GIVING COUNCIL OF ACADIANA 2008 Awards for Philanthropy The Planned Giving Council of Acadiana invites you to nominate one or more individuals and/or organizations for our 2008 Philanthropic Awards. It is important that we appropriately recognize those volunteers and professionals in our community who reflect the true spirit of generosity, creative leadership, and community and team building. Deadline for nominations is Friday, September 12. The awards will be presented at our Philanthropy Day Reception on Tuesday, November 11, 2008. AWARDS CATEGORIES & CRITERIA Outstanding Philanthropist For this award, the nominee must have a proven record of exceptional generosity over several years. We are seeking persons whose generosity encourages others to follow that example. We are seeking a candidate who has demonstrated outstanding civic responsibility through financial support. The narrative should specifically address your nominee's outstanding philanthropic services as well as other civic responsibilities or accomplishments. Please consider the following criteria when evaluating your nominee and include this information in your nomination: •Evidence of direct financial support to your organization or to the community •Specific achievements, such as new programs launched through the nominee's efforts •Evidence of encouragement and motivation given to others to take leadership roles toward philanthropy and community involvement •Description of participation in and support of other nonprofit organizations •Evidence of philanthropic support of innovative approaches to solving problems Outstanding Philanthropic Organization This award goes to a corporate or private foundation or other organization whose outstanding financial support of charitable causes sets an example for others to follow. The narrative should specifically address your nominee's outstanding philanthropic services as well as other civic responsibilities and accomplishments. The following criteria should be considered when evaluating nominees: •Evidence of direct gift support, including agencies and causes supported •Description of impact of gift support on projects •Evidence of encouragement and motivation given others to take leadership roles toward philanthropy and community involvement •Evidence of philanthropic support of innovative approaches to solving problems Outstanding Fund Raising Volunteer The individuals eligible for this nomination must have demonstrated exceptional leadership skills in coordinating groups of volunteers for one or more major fund raising projects, as well as commitment for the advancement of philanthropy in general. The narrative should specifically address your nominee's outstanding volunteer services as well as other civic responsibilities and accomplishments. Please consider your nominee's achievement and accomplishments: •Evidence of quality of leadership and organization ability in fund raising campaigns •Description of time, effort and financial commitment •Description of fund raising goals achieved by his/her effort •Evidence of success in recruiting and motivating others •Description of participation in other nonprofit organizations NOMINATION PROCEDURES •Complete the attached nomination form (forms may be photocopied) •Include no more than 3 pages of supporting documents •Submit original of nomination form and 4 copies •Nominations must be received no later than Noon, September 12, 2008 •Mail or deliver nomination packet to: Sarah H.
