 AWARDS
    FULL IQA DIPLOMA PROGARM : Students who successfully completed and passed all six modules
     (Full Diploma Course) will be eligible for the award of Institute of Quality Assurance Diploma in Quality.
     Successful students will be able to use the designatory letters Dip.Q after their name.

    IQA SINGLE MODULE(s) PROGRAM : Students who successfully completed and passed the selected
     module(s) (Single Module(s) Program) will receive the IQA Certificate for the specific module(s)

    IQCS SINGLE MODULE(s) PROGRAM : Students who do not wish to sit for IQA examination BUT are
     only interested to gain knowledge & skill in specific module(s) (Single Module(s) Program) will receive
     IQCS Certificate upon successfully completed the selected module(s)


COURSE LECTURER & FACILATATOR
    IQA(UK) approved lecturers and facilators who are highly qualified professional graduates, experience in
     the field of QA & Quality Management ,Statistical Techniques, Environmental, Information &
     Communication Management and familiar with local culture and environment.

    CORPORATE COURSE (IN-HOUSE) REGISTRATION DETAILS
 Duration       :       12 months to 6 years (maximum)

 Class Size     :       minimum 20 persons

 Entry Requirements            :
  Over 18 years of age (note : no upper age limit)
  Managers, Engineers, Supervisors, Quality Professionals, Owners and Consultants who wish to
    enhance their knowledge in Quality Management and contribute to the development of their company
    and who aspire to further their careers.
  Pre-course academic qualification : minimum secondary level for local students. For detailed
    information, see section 4.3 in Diploma in Quality Syllabus Handbook

 Modes of Program (Flexible Schemes)           :
  This program is run as a part time course as an evening, day release, block release or correspondence
   course, designed to meet the Corporate’s requirements. Students are able to enter the program at the
   start of any semester.
  The students have an option to do the full IQA Diploma Program or select the individual module(s)
   and will receive a certificate for each unit. To complete the Diploma Course, students must take a total
   of 6 units. Please see section for detailed course time-table.

 Course Fee & Payment Term           :
 (I) For Full Diploma Course
  See course fee structure attached (Inclusive of lecture notes & supplemental documents).
  50% of the full payment shall be made prior to course commencement.
  Full payment shall be made 12 weeks from the registration date or prior to examination.

 (II) For Single Module(s)
  See course fee structure attached (Inclusive of lecture notes & supplemental documents).
  Full payment shall be made prior to course commencement.

 N.B Students applying for IQCS Certificated Course do not need to pay the IQA’s registration and
 examination entry fee. The participating company is responsible to apply for the government’s education
 subsidy for this course.

 Membership              :
 All students register for the Full IQA Diploma Course will receive free student membership to the IQA
 for one year. Please visit IQA MEMBERSHIP website for the benefits and welfare of IQA membership.

 Subsidy        :       This IQA program is eligible for SDF/HRDF subsidy (under corporate sponsorship)

 Documents to be submitted by students :
 (i) Course Enrolment Form IQCS/IQA-001,            (ii) Equal Opportunity Monitoring V2-ED08,
 (iii) Exam Entry Form V2-ED04,                     (iv) IQA Registration Form V2-ED03,
 (v) Academic Qualifications (only applicable to IQA Course)

 Venue          :       Training Premises to be provided by participating company

 Cancellation            :
 (i) Written notice of cancellation should be submitted 30 working days prior to course commencement.
     Registration fee of £37 is not refundable.
 (ii)For registration with the Full Diploma Course or package offers, withdrawal from module(s) not
     attended shall NOT be allowed. The enrolment for the Full Diploma or pakage offers are considered as
     ONE enrolment, based on the condition that ALL modules shall be attended.

Note 1 : All examination fees are to be paid by the 15 November 2004. Examination late entry fee: £45.00. Late
entry can be accepted due to mitigating circumstances. In all cases a late entry fee of £45.00 will be charged.

Note 2 : To be eligible for IQA examination, IQA requires the submission of all assignments by the Education Center
the latest by 10 January 2005 & 7 June 2005 respectively. Please contact IQCS for assignments submission date.
