Details of Incident
A farmer had splashed some of the substance in his eye some 20 minutes
previously. The doctor was calling for advice on treatment.
Off-Label Use on ornamentals
Flower-harvester. Crop was treated with Carbendazim, Chlorothalonil and
Azoystroblin (unknown brands) 10 days prior to harvest.
Caller phoned to say that her daughter had been suffering from a skin
condition and flu-like symptoms for the past two weeks and has recently
been admitted to the Norfolk and Norwich University Hospital. She
described the condition as having been caused by "some form of biological
warfare agent". The caller said that the hospital could not identify her
daughters illness as yet, and had placed pictures of her daughter's skin
condition on the web in the hope that it would be identified. The caller then
told me that she had spoken to a local farmer who provided her with the
labels from products which he had been using locally. The caller described
two of the products as having a black cross on an orange square. She
wanted to know if I could help with the identification of her daughter's
illness or if any of the products would cause the symptoms described.
Caller was picking daffodils. She is known to be very sensitive to what she
eats, etc. She has been doing piecework on the farm for a number of years
and always wears gloves.
The caller was dealing with a patient who had used the product in a
knapsack (even though the instructions indicated it should not be). The
sprayer had leaked and the person involved was now suffering some
symptoms. Could the product be the cause of the man's illness? The caller
had spoken to the Poisons Centre, but they could not help as they did not
have any information on the product.
Unloading chemical from lorry and 2x10L of product were leaking and went
over clothes and into boots.
Farmer had been splashed in the eye when using the product.
Farmer had been applying product on his farm over the weekend. Towards
the end of the weekend he began to feel unwell.
Pipe burst while spraying and splashed his face. Had to return to farm to
wash off. Within two hours flu like symptoms developed and not feeling
good. Can't get a doctors appointment until this afternoon.
1 week after spraying product caller developed cough and sore throat, not
helped by Lemsip. Has a doctors appointment this afternoon.
Feeling unwell since spraying with product. (Spot treatment 1 litre used on
8th and 9th June). Worried as he did not wear a respirator. Grower feeling
dizzy, off colour, loss of appetite. He did not have a sore throat. Been to
see local GP and he asked for more information as he did not know the
product very well

Another hospital contacted them about a patient who had ingested the
product. She had no reference material on the product.
0.75 l of the product spilt onto carpet in living room. Product had been
brought into living room to keep away from children. It had been knocked
over by dogs, probably no top on. Home owner had scrubbed the carpet
without protective clothing, was also concerned about pets, fumes, the
carpet, and what the product was for. Advised home owner to wash skin
several times with soap and water and advised they should see a GP or
visit A&E if concerned. Also advised to take the product label as that
would help the Doctor / hospital if needed. Owner asked about pets,
advised to wash their feet and take to the vet, if any concern also take the
label etc. Owner planned to remove carpet as it was unlikely that they
would be able to effectively remove 0.75 l of the product. Carpet should the
be disposed. Owner asked about washing it outside, I reminded them that
they would have to keep any washing water away form drains, streams etc.
Asked if the product would kill rushes said no.
Caller seeking advice after his son complained of feeling unwell. Son had
been helping to top up spray bottles with the product at his school in Castle
Court. The Spray Operators had been wearing masks but son had not. He
breathed in some vapours and began to feel poorly.
Incident occurred two weeks ago. Operator feeling unwell c.14 days after
spraying the product. Doctor did not think the incidents were related but
wanted sds to confirm.
Caller had used product the day before, and overnight suffered from
swelling on the back of left hand, it has subsequently reduced but wanted
to know if product responsible.
Caller called seeking advice after her friend, had contaminated herself with
product. The fumes had made her feel sick and she took her gloves off for
some reason. She then got some of the product on her hands which
became red and sore.
Farmer requesting medical advice following spraying his land with a
knapsack sprayer.
Farmer was spraying a field when a spray nozzle blocked. Tried to clear it
by blowing through it. Got some Reglone on his lips.
A schoolboy sprinkled some of the product over two girl pupils at a school.

An employee of an ornamental production company was using the product
on primroses under the Long Term Arrangements for Extrapolation of Use.
Both primroses and the product are know sensitisers. The symptoms
occurred 8 weeks post-application of Shirlan. None of the employees were
wearing gloves when handling the plants.
An incident had been reported on a farm and one of the farmhands had
been trapped either under or in a tractor. The Farm manager was phoning
for an MSDS in case the farmhand had been exposed. He was just
preparing information if required by the emergency services, the Fire
Service were on the scene trying to free the farmhand.
Callers property surrounded by fields. Farmer applying a number of
products. Callers wife and son standing ~ 30 ft away as sprayer passed.
Within 20 mins all felt tingling on lips and nose. Sensation now passed.
Does not feel need to visit GP. Needed reassurance about possible long
term effects. All companies have been contacted by the caller. They have
all agreed to send msds for their products.
Caller found a bottle of the product in back of the kitchen cupboard - as it
was dusty she washed it- but did not open it -then felt tingling sensation in
her hands wanted to know if she had been contaminated or not and if so
what was the risk.
Child had access to undiluted product and may have consumed some

No direct incident can be traced, but
caller suffers from Multiple Chemical Sensitisation Syndrome and claims to
react to minute quantities of numerous household and industrial chemicals.
The caller had spilt some of the product on his back whilst using it on the
8th of October
The caller was a farmer who had been using the product 10 days
previously. He suspected he had inhaled some dust from the product, two
or three days after he had used it, he developed a rash on the inside of his
mouth. He was calling for advice.
Yuasa Battery UK Ltd took delivery of 4 shipping containers containing
batteries, which had arrived at their Birmingham factory from Oman. Their
own operatives opened the containers to unload the cargo unaware that
the product had been placed inside them in Oman. The foil sachets
containing the Plates had been opened but the Plates had not been
removed from them and had possibly not evolved all their gas. These
plates were removed from the containers and placed outside, where they
caught fire (due to wet conditions). The four operatives were taken to
hospital and kept in overnight, however, their condition was not serious.
Farmer used Fazor – there was a wind speed 2.5 knots – neighbour claims
it crossed 6 meter margin and over 4 metre hedge. Claims tongue was
sore and raw. Told farmer that his neighbour should have nothing to worry
about – the product has no hazard warnings on the label and even if a
minute amount drifted next door was unlikely to cause harm. Did mention
that the neighbour should go to doctor if worried and we could send MSDS
to doc.

had blood in his urine. Wanted assurance That it was nothing to do with
the product incident. Farmer requested we call neighbour. Neighbour did
not want to go to doctor and did not want to make a fuss.
Farmer had given caller on 17/07/2005 the labels for 3 products he had
been using locally (see above). Unclear if there was any contamination. If
there was any, was spray drift exposure not in course of job.
A group of boy scouts at a supervised camp of grass field. Field had been
recently sprayed with the product for weed control. One scout developed
an 'allergic skin reaction' during the period of the camp. Owner of field (who
was not the spray operator) wanted to know if there was any possibility of a
connection between the allergic reaction and herbicide.
Dust blew back into spray operators face when mixing into his mixing bowl
The subject was reported to have used the product among a number of
other products on a given day. Subject may have been a contractor, but not
confirmed. Subsequent to this use, the subject reported feeling unwell and
requested safety data sheet for each product that had been used, including
the product. Subject was understood to be on the way to seek medical
advice at time of enquiry and wanted safety data sheets to take with him.
While working in her greenhouse, caller was affected by a farmer spraying
the product. Symptoms trouble breathing, smell, taste and burning back of
tongue. Farmer contacted distributor.

and explained product not mutagenic,carcinogenic, no reproductive effects
and unlikely to be absorbed by skin, also offered for our doctor to talk to
operator. Called again at midnight very concerned with continued irritation
and any potential side effects. Re-confirmed what said previously and
suggested he see his doctor. He said he would go to the 24 hour doctor,
suggested he take his label with him and the doctor should contact me if
any issues.
Small pipe on sprayer split as operator was travelling to a field . When he
reached a junction operator noticed the leak and repaired it, he then
continued on and to sprayed the field. When he had finished he noticed
that the fire, police and ambulance services were at the scene and had
closed the road because someone had reported the yellow stain. The fire
service were about to wash the road but the operator told them not to,
went to get a label but upon his return the fire officer told him that they had
consulted their “experts” and proceeded to wash the road. There were no
ditches or other water courses in the area, the run off would have ended up

The storeman spilled some product and then decided to smell it. He sat
down afterwards but reported no major problems or side effects again.

Details of Incident
The caller told me that she had a patient that had been using this product
two days before the call. The patient was now complaining of an irate
chest, headaches, aching joints (flu like symptoms). The caller wanted to
know whether the product could have caused this.
Caller suffered skin rash. Another operator suffers from asthma and this
spray brought it on. Now both recovered fully. None of the operators using
Equity had any problems. These were the only ones on Dursban WG.
Subject was contaminated by drift from spray being applied to lawn of a
nearby garden. Spray operator declined to identify product being applied.
Subject approached HSE who suggested product probably MCPA or
mecoprop-p. Subject phoned us for further information on these two
chemicals.
The incident was reported in the Sheffield Star newspaper. The weedkiller
had been obtained from a council store and was stored in an unmarked
bottle in a fridge. The gentleman involved drank from the bottle, unaware
of what it contained. He was admitted to hospital following his collapse and
died three weeks later.
A family arrived at Crewe A & E on Sunday 5th June, Grandfather strongly
suspected poisoning by weedkiller.

Left in warm car.
Caller was landscape gardener who used the product previous week on
client‟s lawn. Client now claiming she and her family were suffering ill
health and they thought product responsible.
Packet had split when caller was opening and he got powder in his face &
up his nose. Had washed the product off but wanted further information.
2 days previously caller had inhaled some of the product when opening the
sachet. Was concerned about long terms effect to his health
Caller had used the product previous day, and got some into a cut on his
hand. He had washed the affected area. He wanted to know if symptoms
related to incident
Caller had used product previous day, was now suffering diarrhoea and felt
unwell, was the product responsible
Caller had spilled product on table, though not on himself, but was

Caller had spilt product on himself while opening the packet, it had got in
his hair and on his clothes, not in his eyes but there was a faint taste of the

Caller had used the product on 13th August. On the 19th August her
grandson had played on the treated area, the child was now unwell. Caller
wanted to know if product had caused his illness.
Caller‟s mother was opening product sachet when it ripped apart and dust
came out, which may have been inhaled and in contact with skin. Caller‟s
mother was taking shower to remove product
Caller‟s husband had used product 5 days before and got spray in face, he
washed his face immediately. Now on holiday in France, the gentleman had
just been admitted to hospital with various chest problems. The French
doctors required further information on the product.
Caller had got some product in eyes during use. He immediately washed
out his eyes, wanted to know of any health effects associated with the
product.
While disposing of empty bottle the label came of and stuck to caller‟s
finger, which subsequent became swollen. Caller wanted to know if
product had caused the swelling.
Caller had used the product few hours earlier, in breezy conditions.
Used product evening before - several hours later rash developed on torso.
Caller‟s spouse had used product round the house, on the furnishings as a
substitute flea spray. Caller wanted to know what the likely hazards and
risks were (and how to clean it off her furniture)
Caller had used the product 8 days before, and experienced problems with
the spray nozzle, resulting in product being splashed over her arms and
legs. The caller had washed off the product.
Caller used product previous day, while his wife was in the garden. They
think she may have inhaled some spray. Overnight she developed a large
swelling and her skin became red and blotchy. Caller was concerned that
the product had caused the symptoms.
Caller had eaten 4 grapes sprayed with product, worried about side effects
Caller had used the product to descale his kettle. He wanted to know if the
kettle would be safe to use.
Caller‟s 5-year-old son had sprayed product on fingers, then licked fingers.
Child‟s mouth had been washed out.
Caller had sprayed some weeds 10 days previously, may have got spray
on hands. Subsequently suffered a rash, became ill but now improving,
caller wanted to know if product may have been responsible, or was his
illness the result of chicken pox, which he had about a month ago.
Caller had got some spray into her eyes. She had already rinsed eye
thoroughly.
Caller‟s two-year-old son had sprayed himself with product, soaked into his
clothes and on his skin. Caller had changed the child‟s clothes and washed
his skin, wanted to know if product was dangerous
Caller‟s 2-yr-old daughter had sprayed product in her mouth, caller was at
hospital A&E, with child, had taken product with them
Caller had come into contact with spray when using the product. She had
rinsed mouth and washed hands and face, wanted more information as is
allergic to several things.
Caller was Sister from A&E Department, had patient who had consumed no
more that 400ml of product.

Caller‟s colleagues had been unloading bags, one of which split and the
man had accidentally caught one in his mouth. He had drunk some water.
They had obtained a product SDS from the manufacturer.
Caller was shop worker who had been contacted by a customer who had
spilt the product on a mat the day before, she attempted to clean up the
spillage today, and believes she may have inhaled some of the vapours
Caller had treated lawn, then 2-year-old son dropped his dummy onto the
lawn, then put it back in his mouth. Caller wanted advice (child‟s mouth
had already been rinsed).
Caller‟s father had used product that morning, later the same afternoon he
stated to complain of a swollen mouth. He had used the product several
times before without problems. As far as the caller knew her father had not
spilt the product on his skin or clothes.
Caller had used product 9 days ago, and since been unwell, was
concerned product was responsible.
Caller had used product 2-3 days previously. This morning he felt dizzy and
wondered if product was responsible. Caller stated he was also on
medication, which had dizziness listed as a potential side effect, he wanted
to know if the product was responsible before he contacted his GP.
Caller had some diluted product in a container in the shed. His 4-year-old
son had got hold of the container and sprayed some into his mouth.
However, the caller did not think anything had been swallowed.
Caller had used product previous day, and since suffered from irritated,
runny nose. Caller did not think she had inhaled the product, but wanted
advice

not right (did not realise product had contacted right foot.).

2.5 yr old child had managed to get the bottle and sprayed his teeth with

The subject inhaled the product whilst putting it into a spreader. The lady is
asthmatic and reported breathing difficulties after doing so. The breathing
difficulties didn‟t last very long, normal breathing resumed after a short rest
and use of an inhaler. A doctor was not consulted
a couple both had upset stomachs a week after applying the product to the
lawn. One was using the product and the other was standing in the garden

pellets got into mouth or were swallowed. There had been rainfall since the
product was applied.
Caller‟s 2-year old-niece had dropped ice-cream onto the pellets then ate
ice-cream. Caller thought she had ingested some of the products, wanted
to know if she would be alright.
Caller‟s one-year-old child may have ingested some of the product.
Caller had found his 2-year-old daughter with a pellet in her mouth. She
spat it out, Does not think she has swallowed any, but wanted further
advice
Caller was Sister from A&E department regarding a female patient who had
drunk 200ml of product that morning.
Spraying product - 3 yr daughter put some on finger and put finger in her
mouth.
Adult - used product 1 week previous. Got some liquid on hand

and splashed liquid on himself. Had already washed the affected area

A male had ingested 2 sachets of the porduct and an unknown quantity of
paracetamol
Caller had treated garden previous evening. Later that night, her 12-yr-old
son stated he may have product in his eye. They rinsed his eye out, but he
suffered stinging overnight.

Caller‟s 2-year-old nephew had been crawling on area that had been
treated with the product last week. Caller was concerned that his nephew
could be harmed.
Caller had used product 2 weeks previously. Recently developed lung
problems, was due to see his GP today, and wanted confirmation whether
the product was responsible for his illness.
Caller had used product three days before, and inhaled a small amount.
She was now experiencing chest pains at night, was the product
responsible? There were no other symptoms and caller had used to
product for some years without any problems.
Call from shop, a customer had bought product previous day, tried to open
product while still in car, and got dust around the car and on himself.
Customer had gone back to shop to ask if product would harm him.
Caller‟s toddler got hold of product and may have sprayed himself. Caller
did not witness the incident, she had already removed clothing and
thoroughly washed face and hands.
Caller had got spray into his face/eyes when using the product. He had
washed his face and eyes.

Action Undertaken by Company
After establishing the product involved and the manufacturer (initially the company
name was given as Alpha Chemicals), I then confirmed the main ingredients -
propachlor and ethylene glycol and that the product was irritant. Based on the
information in the SDS, I advised that treatment should be given symptomatically. I
also advised that further irrigation of the eye be administered for several more
minutes and that if the patient had any lasting effects then the doctor should call back
and I would contact the manufacturer for further advice.
MSDS sent. Nufarm followed up next day and confirmed no further action required by
Nufarm. Supplier commented that an allergic reaction to plant material can sometimes
happen.
I told the caller that the products with the black cross on the orange background were
classed as irritant [or harmful]. I asked her if she had taken the labels to the hospital
and she said that the farmer had only given them to her today. I advised the caller
that the best course of action was to take the labels to the hospital and if needed I
would forward any or all of the SDS to them directly, provided I had them.
Spoke to the Farm Manager about hygiene practices on farm. He stated that there
were always supplies of fresh gloves including in the washrooms. He was going to re-
enforce the message of removing gloves and washing hands both before and after
going to toilet and putting on fresh gloves.
I checked the database and found two entries under this name. I confirmed the
correct product and found the SDS. I offered to send the SDS to the caller and he
said he would like to see it. I suggested that he call the Poisons Centre again, this
time with the name of the active ingredient.

Farmer was looking for information on what to do. He was told to rinse the eye with
clean water for at least 15 minutes and report to local casualty department to seek
further medical attention.

Doctor said it was unlikely to be the product and prescribed anti-biotics.
Went through the msds and concluded symptoms were unlikely to be those caused
by the product as he did not have a sore throat. I Phone caller later in the week to see
if he is still unwell.

product has been sprayed to see if the reaction is to the primroses. As a result of this
incident the Company has introduced improved "safety measures" ensuring people
wash their hands, wear gloves and tie their hair back, when working with the plants.

who talked through the situation and reassured her that symptoms could occur with
direct ingestion but should not be cause for concern.
Called NCEC to ask what to do. Satisfied no symptoms. Was advised to see his GP if

see her GP if symptoms did not subside.
Caller contacted NCEC who suggested symptoms unlikely from product using SDS,
but see GP to make sure.
Caller phoned NCEC who gave him the SDS and informed him effects were unlikely
to be caused by spray and to see his doctor if symptoms persist.
Caller had first contacted NHS Direct who had given him wrong information, he then
called NCEC who advised him of the product's non-irritancy and reassured no long
term effects as he had acted correctly in washing out his eye. Concerning that NHS
Direct told caller the product might cause acid, corneal burns, which is untrue.
An MSDS was offered and reassurance that the product had no smell. Company
doctor contacted after second phonecall was abusive and an individual letter was
sent. The lady changed her allegations several times.
Caller had contacted NCEC who gave her information from SDS that product was not
toxic or irritant and advised her to drink some water. She was advised to see her GP if
the tingling sensation did not pass.
Suggested he contact his doctor if symptoms continue and sent MSDS. Reassured
unlikely to be connected with the product.
Called NCEC to check what should be done to decontaminate. Was given advice to
remove clothing and wash. No symptoms reported
Company retained doctor telephoned and discussed symptoms and gave advice.
Local Authority are aware and should advise prior to spraying so she can stay
indoors.
Company Doctor contacted, who telephoned the caller and offered advice.
Having established how long ago the exposure was, I told him I could not offer any
other advice, other than to visit his doctor to be examined and to seek treatment for
his symptoms. I also told him that although the product was described as mildly
irritant, there was nothing obviously to suggest the product had definitely cause the
symptoms. However, I suggested he passed on our telephone number to the doctor,
so that he or she could speak to us about the product and assess whether it had
caused his symptoms.
The HSE were informed and began an investigation into this incident.
Spoke with farmer. He had spoken with neighbour and passed on our information
regarding the product. Farmer didn't think the neighbour would be going to the doctor
but gave him our details in case he had any further queries.
Phoned neighbour and assured him that the product would not have caused this
problem especially so long after the event. No further action required.
NCEC told caller it was unlikely that the product had caused the problem and advised
her to take labels and Safety Data Sheet to the hospital. No further action. Case
closed.
Posted MSDS on the product to enquirer.
Enquirer requested safety data sheet for each product that had been used prior to
feeling unwell. There was no indication that exposure to the product was the specific
source of the symptoms. The safety data sheet for the product was faxed
immediately, and the approval holder asked to be
kept informed if further information was needed, or if further action was necessary.
There was no further contact from the enquirer.

Sent a letter and fact sheets on the product, together with a copy of the Green Code
of Practice for spraying to explain how farmers should carry out spraying. There was
only anecdotal association between the child's illness and any contact with the
product.
Advice and reassurance given and told if he did start to suffer from any symptoms he
should seek medical attention. Operator was wearing gloves and coveralls.
Patient spoken to on 11th August and she was fully recovered

We would not expect this exposure to the product to give any cause for concern.
He is now going to visit casualty with a copy of the safety data sheet.
Caller contacted NCEC to see what precautions he should take. Was given details
from SDS and told there should be no symptoms as he had washed skin and clothes
immediately.
He had contacted his own GP but was waiting for a reply. In the meantime the caller
wanted some advice from a panel member.
Operator phoned back 9:30am on 26th , 24 hour doctor advised bicarbonate of soda
which had worked. Operator no longer concerned. No further action required.
Operator asked if there was anything else he could do. I explained that the product
was dangerous to aquatic life and that it should be kept out of any water courses etc
and that the preferred method of clean up would be to absorb with sand then sweep
up and dispose of properly. I offered the MSDS but he already had one. Given that
the leak was only a small one and that it was already diluted in the tank and that there
were no water courses involved I said that there was probably little more that could
be done. I said that if he had any further questions or contact from the authorities then
he should contact us.

illness. Child had already seen the Doctor, and caller asked if she should advise
doctor of potential contact with product. Advised that if the child needed to see the
doctor again she could mention it to him, but repeated that it was very unlikely that the
child had come into contact with sufficient amounts of the product to cause the illness.
Advised that the product may cause irritation to nose and throat if inhaled, and to
rinse mouth with plenty of water and to drink a little. If symptoms develop to seek
medical attention taking the product to the GP.
The SDS indicated that active ingredient suppliers should be called when dealing with
medical requests on paraquat. They were contacted and arranged for them to call the
hospital. Also sent an SDS through to the hospital, Contacted caller and explained
course of action taken, she confirmed that the English doctors had already been in
contact.
Advised was a fairly low hazard product, and that had taken correct action. If he does
suffer eye irritation he should consult his GP, taking the product with him, his GP
could contact NCEC for further information if required.
Advised that the product was unlikely to be responsible for swelling. Caller then asked
if the product had reacted with the glue on the label to cause the reaction. Told that
as did not know composition of glue could not say. Possible that he was sensitive to
the product, advised to seek medical advice, and if required NCEC could fax SDS to
GP.
Advised to rinse mouth and wash any exposed skin. Caller replied that she had
already done so. Advised product of low toxicity, but she may be sensitive to
ingredients, if symptoms persist to contact her GP who could contact NCEC if
required.

Advised to wash furniture with warm soapy water, followed by clean water. Advised if
either caller or spouse felt ill to seek medical attention and tell GP of contact with the
product, also monitor any pets.
Caller taking antihistamine. Advised it was unlikely the product was responsible, due
to the time lapse between the incident and development of rash. Recommended she
visit her GP, and mention the incident. The GP could contact NCEC for further
product details if required.
Checked product SDS, advised highly unlikely the product caused the symptoms
described. Advised to seek medical assistance.
Caller went before could offer advice, tried to call back (9 times) and left messages on
answer phones but was unable to contact.
Advised not to use the kettle and dispose of it safely.
Advised had taken correct course of action, given age of child should seek medical
advice. (subsequent call received by A&E, Warwickshire hospital, boy checked and

Advised that although product may be skin irritant, was not a skin sensitiser. As had
only be exposed to product for short time unlikely product was responsible for his

not cause problems unless significant quantities are ingested, but the child should be

up spillage, and ensure ventilation while cleaning.
Advised to give the child water to drink, then seek medical attention. If she did not
wish to see GP should monitor child‟s behaviour and in the event of any change seek
medical attention immediately, taking the product with her.
Advised to rinse out his mouth and rinse the skin around his mouth to remove traces
of any product. If still uncomfortable advised to contact GP, who could contact NCEC
if they required further information on the product.
Advised if he was concerned or continued to feel unwell he should see his doctor,
taking pack with him. His GP could contact NCEC for further information on the
product if required.
Advised that it was unlikely the product was responsible, and suggested he speak to
his GP about the medication. He could mention contact with the product with his GP,
who could contact NCEC for further information if required.
Advised to rinse child‟s mouth thoroughly. If the child experienced any symptoms then
he should seek medical advice.
Advised that product had low toxicity, although possible she was sensitive to it.
Advised to see her GP, taking the product with her. Her GP could contact NCEC for

Call referred to NCEC, who gave her SDS information and said it was unlikely to be
caused by the product, but advised her to show pack to GP. The lady had also been
using a paint product on the same day and it is more likely this could have caused the
reaction.
Call transferred to NCEC who advised product was unlikely to cause symptoms
described. Company-retained doctor contacted caller and reassured her on the safety
of the product and sent the SDS to her by post to show her nurse.
Company retained doctor discussed with caller. Caller was satisfied there was no long
term effect and that rash caused by leaking spray and friction of finger on trigger

Caller had been transferred to NCEC for advice. She was advised from the SDS that
there should be no symptoms as the product was not hazardous and she had washed
it out immediately. Advised to see GP if any symptoms appear.
Company doctor discussed symptoms. Swelling may be connected with a knee

Company doctor contacted and established the lady suffered from hay fever and the
pollen count was high at the time of spraying. This was accepted as the most likely
initial cause of the eye irritation.
NCEC advised on washing out procedure. Caller was not happy about the pack and
felt it was faulty. but his symptoms relieved after washing eyes.

Asked if child had handled or swallowed any of the product. Caller said no. Asked if
the boy had symptoms like redness of skin. Caller said no. Advised that product is of
low toxicity, but if any symptoms appear the caller should take the child to his GP,
with the packet. The GP could contact NCEC if required.

Advised caller to check child‟s mouth for traces of blue, (nothing found). Advised
caller of symptoms of metaldehyde poisoning, Although unlikely that the child has
ingested significant quantity of the product caller was advised to take child to GP or
A&E that day, taking the packet with them.
Advised that the product could be harmful if ingested, and should take the child to
casualty, together with the product pack. The doctors could contact NCEC for SDS,
further information.
Advised that in all cases involving children where the amount ingested was unknown

nature of incident, the company was also immediately notified. NB: Follow up
received Feb 2006. Patient made a successful recovery and no long term sequelae
are anticipated

18th August followed up with the hospital and patient had been released.
Patient transferred from Casualty to Psychiatric ward. Plasma analysis done by
Syngenta for the hospital. All relevant medical information passed by Company
doctor to Newcastle A & E doctor. This was an attempted suicide.
Suggested that they wash out eye for a further 15 minutes, then seek medical
assistance as a precaution due to the sensitive nature of the eye, (whether the
product was responsible or not).
Advised to wash area thoroughly with warm soapy water.
Having confirmed that the child had not ingested any treated grass or plant leaves,
advised that the product would not cause harm to the child.
Advised that the product was not classified, and was of low toxicity, therefore unlikely
to be cause of his illness. If required the SDS could be faxed to his GP on request.
After checking the SDS advised unlikely the product had caused the symptoms.
Advised to contact her GP.
Advised that product is harmful by ingestion and may irritate skin, however customer
did not seem to have any symptoms at this stage. Advised if did suffer any reaction
should seek medical attention and take product with him
