Date Indicate the starting date of the intervention
Data Indicating Provide the rationale behind the need for the group to receive this intervention. Baselines
Need will be stated below for each individual student: therefore information should be stated here
in more general terms. For example: The DIBELS measures at the winter benchmarking,
indicated students were in need of strategic intervention for the following: Phoneme
segmentation and nonsense word fluency.
Persons Provide the name(s) of those delivering the intervention and analyzing the problem.

Instructional Describe the delivery of the intervention, the instructional strategies. Such as_ The teacher

Progress Explain the way you will gather the data and make decisions about it. For example: Weekly

phoneme segmentation probes. Data will be graphed each time. A decision making rule
should also be stated. For example: Every time 4 data points in a row fall below the line, a
teaching change will be considered.

Baseline Provide student individual score(s) at the beginning of the intervention. For example: 17
sounds in 1 minute on the DIBELS nonsense word probes.
Goal This will be based on the individual and will vary with each student. State the goal in
observable and measurable terms. Such as-35 sounds in 1 minute pm a nonsense word
probe by 1/17/03 (next benchmarking period).
Outcomes State the student’s level of performance at the end of the intervention. Best practice would
be to keep an individual graph for each student. State the results of the intervention in
observable and measurable terms. For example: James reads 3rd grade passages with
55wcpm. Or James reads Nonsense word probes with 28 sounds in 1 minute.
Individual This is based on the needs of each child. For example: James will need to re-read the
Modifications passage 5 times instead of 3, or James will have an additional practice session 2 times a week
with a high school helper to practice sounds being taught in the intervention.
Parental Indicate how the parent will be involved and informed during the intervention.
