 Parent AND Player Contract
 For clarification purposes, the parent/guardian/athlete understands that Command Performance Juniors is a NFP. Parent
 Initial _____

 The parent/guardian is committed to the financial support of Command Performance Juniors Volleyball for the entire 2009-
 2010 season, and understands that there are no refunds regardless of how long the athlete is in the program. The
 parent/guardian agrees that all dues, fees, and fines must be paid in full and on-time regardless of duration of the athlete’s
 participation and court time, and understand that if payments are not received on-time, the athlete will not participate in
 practice or events. The parent/guardian agrees that in the event of a default, Command Performance Juniors is entitled to
 receive the entire contract balance due as liquidated damages. The parent/guardian agrees that the team cost for out-of-
 state travel and two day tournaments will be paid by ALL members of the team with NO exceptions, and if it is not paid
 before the tournament the player won’t be allowed to attend and will have to pay in full before the next tournament. The
 parent/guardian also understands that if a traveling event is changed, the fees will change with it. Parent Initial _____

 The parent/guardian agrees to provide all transportation to and from practices and tournaments. Parent Initial _____

 The parent/guardian completely understands that if outstanding dues, fees and fines are not paid by the 5th of each
 month, Command Performance Juniors will charge those dues, fees and add an administrative fine, and the athlete will be
 suspended until all dues, fees and fines are paid. Parent Initial _____

 The parent/guardian/athlete completely understands and will adhere to all of the Command Performance Juniors Program
 Policies and Guidelines, and we will instruct all family and guests to abide by all rules and regulations of Command
 Performance Juniors and USA Volleyball. Parent Initial _____

 The parent/guardian/athlete understands that playing time is not equal, and understands that Command Performance
 Juniors will not discuss why or how teams were selected. We also understand that Command Performance Juniors is not
 only doing what they feel is right for each individual, but also for each team. We also understand that playing club ball
 provides the athletes with the opportunity to participate in practice and tournaments, though it does not guarantee equal
 playing time in tournaments. Parent Initial _____

 The parent/guardian understands that they are prohibited from talking to coaches, officials, line judges and/or
 scorekeepers at all tournaments. The athlete understands that they have limited contact from talking to
 boyfriends/girlfriends while at tournaments based on team/coach policy, and no non-Command Performance Juniors
 athletes/friends are permitted near the court and/or scorers table. Parent Initial _____

 The parent/guardian/athlete understands that the illegal transportation, possession, sale or use of alcoholic beverages
 and/or illegal drugs or substances banned by the USAV will result in immediate dismissal and an ineligible declaration from
 USAV registration beginning form the date of the infraction to one year as per the USAV Code of Conduct.

 The parent/guardian/athlete understands that players should arrive at practices 15 minutes early and be READY to
 participate in practice at the scheduled time, and that failure to be ready for practice may result in dismissal from practice,
 or is subject to discipline set forth by the coach/Director. Parent Initial _____

 The parent/guardian/athlete understands that if an athlete is having problems with another player, staff, or team THEY (the
 athlete) must set an appointment away from practice or the tournament to speak to their coach immediately. We
 understand that all athletes MUST speak with their coach directly about a grievance before the coach and/or Director will
 address the issue with the parent. We recognize that Command Performance Juniors has a 24 hour complaint policy in

 which we will not contact the Director and/or staff member within 24 hours of our complaint, and no Command
 Performance Junior Director or staff member is obligated to talk to anyone prior to the term of the 24 hour period.

 The parent/guardian/athlete understands that abusive or vulgar language will not be tolerated, and could result in the
 immediate termination of participation in the Command Performance Juniors program without refund. Parent Initial _____

 The parent/guardian/athlete understands that players must wear Command Performance Juniors practice apparel or
 support wear (not game jerseys) to every practice, and all shirts are required to be tucked in. (Girls Only) We understand
 that spandex is required to be worn at its appropriate length, and that long white socks are REQUIRED to be worn as part of
 the team uniform. We understand that kneepads are required to be worn during practice and tournaments. We understand
 that failure to comply may result in dismissal from practice, or is subject to discipline set forth by the coach/Director.

 The parent/guardian/athlete understands that players are required to have their Command Performance Juniors -issued
 ball at every practice and tournament, and the ball should be clean and represent Command Performance Juniors in an
 appropriate manner. Only the players name and Command Performance Juniors name should appear on the ball in the
 event that the ball is lost at a tournament. Parent Initial _____

 The parent/guardian/athlete understands that multi-sport athletes are excused from practices and regional events for high
 school athletic events only (if and only if there is a director scheduling conflict), and not other high school extracurricular
 activities. Athletes must provide coaches with their athletic event schedules, including practice schedules. If high school
 practices do not conflict with club practices, athletes are required to be in attendance. Parent Initial _____

 The parent/guardian/athlete understands that all players are expected to manage their time appropriately. Command
 Performance Juniors policy is the athlete must call knowing he/she is running late to an event, and or if they are going to
 miss a practice. We recognize and will adhere to the policy, if an athlete misses 1 of 2 scheduled weekly practices the
 athlete forfeits starting and or playing time at the next tournament. We recognize and will adhere to the policy if the
 athlete misses both practices they are expected to dress and come to the tournament, but won’t play, except where short-
 handed on a case-by-case basis at the coach’s game-day discretion. We recognize and will adhere to the policy that
 Command Performance Juniors will ONLY accept Family, Illness, and Academic issues for an excused practice. We recognize
 that non-athletic extra curricular activities are NOT acceptable absences. Parent’s Initial _____

 The parent/guardian/athlete understands that all athletes are REQUIRED to attend any and all local competitions, including
 the Regional Qualifier if the team is accepted, unless written explanation is provided to the Coach AND Director TWO
 WEEKS in advance of the first day of the event. If no written explanation is provided, the family is fully responsible for the
 cost of the event, plus an additional $75 administration fee. Parent Initial _____

 The parent/guardian/athlete understands that all athletes are REQUIRED to attend any and all traveling events, including
 National Qualifiers, unless written explanation is provided to the Director THREE MONTHS in advance of the first day of the
 respective competition. If no written explanation is provided, the family is fully responsible for the cost of the event, plus an
 additional $250 administration fee. Parent Initial _____

 The parent/guardian/athlete understands that Command Performance Juniors will only nominate Jr. National teams (ages
 12-18) that have won 50% or more of their matches for the Regional Qualifier. We also understand that the Great Lakes
 Region handles the selection process of which teams make the event based on their own criteria. Parent Initial _____

 The parent/guardian/athlete understands that athletes caught stealing property from Command Performance Juniors, its
 players or from other clubs and their players will be fined $250 and could be dismissed from the organization.

 The parent/guardian/athlete understand that all athletes on a team that qualify for a bid in the 2010 USAV National
 Tournament are REQUIRED to pay for and attend the tournament held in June/July 2010. We also understand that failure to
 attend will be fully responsible for the cost of the event, plus an addition $500 administrative fee. Parent Initial _____

 The parent/guardian/athlete expressly agrees that Command Performance Juniors shall not be liable for any damages
 arising from personal injuries sustained by the athlete in, on or about the facilities, including tournament sites.
 Parent/guardian agrees Command Performance Juniors will not be responsible for any loss or theft of personal property.
 The parent/guardian and the athlete assume all responsibility for any injuries, damages or losses which may occur in or
 about the facilities and does hereby fully and forever release and discharge Command Performance Juniors, its owners,
 coaches, staff and agents from any and all claims, demands, damages, rights of action, or causes of action, present or
 future, whether same be known or unknown, anticipated or unanticipated, provided that nothing herein contained shall
 release or discharge Command Performance Juniors from the willful and wanton acts or mission of Command Performance
 Juniors, its agents or Employees. Parent Initial _____

 The parent/guardian/athlete understands that Command Performance Juniors may exercise the right to move an athlete
 from team to team at the discretion of the Club depending upon the specific team needs or to the benefit of the player. The
 parent/guardian expressly agrees that Command Performance Juniors shall not guarantee placement on a collegiate team.
 Parent /guardian expressly agree that Command Performance Juniors shall not guarantee collegiate scholarship monies.

 Upon review of the above, the parent/guardian and the athlete agree to support Command Performance Juniors and abide
 by its guidelines and rules. Parent/guardian and the athlete understand that once registered with and having competed
 with Command Performance Juniors, the athlete is unable to transfer to another club and is committed to this club until the

 Changes To These Policies May Be Made At The Discretion Of The Club If Necessary, And All Parents/Guardians/Players Will
 Be Notified Of The Change.

 PROGRAM POLICIES AND GUIDELINES

 Facility Care
     1. All parents/guardians/players should be aware that we are guests of the various facilities that Command
          Performance Juniors utilizes, and EVERYONE including CP Juniors staff is responsible for keeping the
          facilities clean leaving the premises in the same order as it had been received.

     2. All parents/guardians/players/coaching staff should be aware that it is Command Performance Juniors
        policy to have the boys and girls volleyball athletes to set-up and strike all net systems plus be
        responsible for club equipment including but not limited to: ball carts, coaches/jump boxes, balls, ball
        travel bags, video taping, cameras, etc. Our program will always be for and about the youth players, and
        since they are the ones who get to enjoy the actual play, then they will be responsible for set-up and
        strike of the equipment.

 Attendance
     1. Practices are mandatory and we expect all athletes to attend practice. The only exceptions are
        addressed are listed in the parent/guardian/player contract, and they are issues pertaining to family,
        illness, academic, or pre-notified school team practice/game schedule conflict. If you are unable to
        attend practice, the athlete must contact the coach anticipating being late. More than 3 unexcused
        absences could result in suspension and/or termination from the program without reimbursement. The
        player would not just be letting down CP Juniors management, but also teammates as well. This is a
        personal respect and pride issue that the player must show commitment too.

     2. All athletes, ages 12-18, are allowed to miss 3 personal days per year. These days may to be taken
        Monday through Thursday, but not on Friday, Saturday or Sunday. To schedule a personal day, you
        MUST inform your coach a MINIMUM of 3 hours in advance of the practice.

     3. The practice environment is a place to work and improve your skills. All social activities shall be
        conducted after practice. All players are expected to arrive early and executing their pre-practice warm-
        ups 15 minutes prior to practice and (1) hour before games.

 Student Athletes
 Student Athletes are expected to hold a minimum of a 2.3 GPA to stay eligible to participate with Command
 Performance Juniors. Anyone challenged by this should see our club director to make arrangements for tutoring
 assistance.

 Common Sense
    1. Any and all jewelry is prohibited. This is a safety issue and a USA Volleyball rule.

     2. Volleyball shoes will be worn inside the gym, and NEVER outside or to the practice / tournament site.

 Parents at Practices and During Tournaments
     1. All practices are closed to the public, and open to parents to observe only. Parents may not be on the
         same floor level of the practice, and may not interact with their student athlete during the practice, or
         they may be excused by the club coach and or the club director. Parents are welcome to sit away from
         the courts, but cannot be viewable to the athletes. We want our players to have the freedom to make
         mistakes and not feel like they are being watched over at all times.

     2. Parents are prohibited from talking to coaches, officials, line judges and/or scorekeepers during
        tournaments. Club coach and/or club director can initiate conversations with parents during off-time
        away from play/competition.

 Travel and Team Commitment
     1. All Junior National program athletes are expected to make all National Qualifier events. We ask that you
         do not take family vacations during these times in which will cause your son/daughter to miss practices
         or competition in preparation of these important pre-paid events. It is unfair to a team when any
         member is missing for a prolonged period of time and team is preparing for an important competition.
         All teams will recognize Winter, Spring and Summer break away from competition dates that will be
         posted on the website. Only schedules set in advance of the start of the season will be recognized.

     2. When we travel by air please wear appropriate attire. Team warm-ups will be acceptable. Bring your
        uniform with you on the plane in your carry on. When line-judging or at a playing site players should
        wear CP Jrs. warm-ups, sweats or T’s – NO High School wear and NO college wear.

     3. When travelling to a tournament when it involves an over-night stay, parents and or guardians are to
        room with their student athletes at the hotel of Command Performance Junior club choice. Parents and
        players need to have a team meeting with coach and or director to verify head count and share
        pertinent team/club information. CP Juniors will schedule 1-2 team meals during any trip where all
        teams must eat together unless otherwise specified.

     4. There is a scheduled curfew of 10:00pm in their rooms for all volleyball athletes unless directed
        differently (earlier) by parent. Curfew is STRICTLY enforced for the entire team. Failure to comply may
        result in dismissal from the tournament, and/or is subject to discipline set forth by the coach/Director.

     5. Misconduct by any players will not be tolerated. Players will be sent home if the following
        circumstances are discovered: taking drugs, drinking alcohol, opposite sex in rooms, sexual contact,
        being too loud in room after hours, out after curfew, NO COMMUNICATION about what is happening
        with player when circumstances do not go in the right direction.

     6. No person of the opposite sex is allowed in the rooms at anytime; including coaches, parents and any
        miscellaneous persons of the opposite gender. If there is an emergency, the coach or guest parent may
        enter as long as is accompanied by either the chaperone or another adult of the same gender of the
        players.

     7. Tournaments typically run from early morning until late afternoons. Players should remember to pack a
        healthy lunch, and are responsible for eating appropriately for an active day. Junk food is HIGHLY
        discouraged (and will probably be consumed by the coaches). All athletes must abide by the CP Jrs. Food
        Guidelines unless otherwise specified.

 Game Day Responsibilities
    1. All athletes must learn USA Volleyball rules and must complete officiating duties prior to leaving an
       event.

     2. Players MUST wear Command Performance Juniors apparel (practice shirts, spirit wear, warm-ups, etc.)
        with jeans especially when they go out on trips.

 General Rules
    1. Command Performance Juniors reserves the right to add to or change these rules and regulations where
        appropriate at any time for the safety of everyone concerned and betterment of the club.

     2. Failure to follow these policies will result in the athlete not being allowed to compete, dismissal from
        the program or event completely, and/or disciplinary action if the coach/administration deems it
        appropriate.

 Command Performance Juniors Financial Agreement

 I, the Parent/Guardian of ______________________________________________, agree that my child’s/ward’s
 membership with Command Performance Juniors (CPJrs.) will require that he attend team practices, competitions, and
 tournaments regularly. I realize that in giving my child a position on a CP Jrs., the Club has denied a position to others
 seeking the opportunity to play for the Club, and I agree that my child/ward is committed to playing the entire season
 including if he/she is on a National team, the summer session as defined on the team tournament schedule.

 I further understand that I am responsible for paying the full amount of dues regardless of the number of practices or
 competitions in which he participates. The yearly dues will cover the following costs: USAV player, coach and team
 registration; facility rental; team equipment; local and if scheduled, tournament entry fees, team volleyball; and if
 scheduled, coaching stipends through July. I understand that the membership dues do not include the cost of personal
 equipment such as uniforms, related travel and hotel accommodation expenses, athletic development, national
 tournament entry fees and related costs. Club Uniform Fee is separated from Membership Dues and paid upfront.

 I have read the Command Performance Jrs Program Policies and Guidelines and the CP Jrs Parent and Player Contract. By
 my signature and that of my parent/guardian below and by the initials where indicated above, I and my parent/guardian
 have read and understand the terms of this contract. With the consent of my parent/guardian, I freely enter into and I
 agree to abide by the terms stated and Command Performance Jrs Policy. My parent/guardian and I understand the
 commitment to Command Performance Jrs, its athletes and staff.

 Command Performance Juniors Volleyball Club dues must be paid in full by February 18, 2010. Failure to do so may result in
 the suspension of the player from practices and competition until dues are paid in full. Also, in the event of default, if this
 obligation is referred to an attorney, and/or collection agency, the Member agrees to pay, over and above his liabilities
 hereunder, reasonable Club’s attorney’s fees, court costs, and cost of collection.

 All trip and housing fees will be paid prior to trips, unless special arrangements have been made with Command
 Performance Jrs. I agree that I will assist my child in being responsible for arriving at all club designated events unless

 tournaments, national qualifiers, and national tournaments. I understand that if my child’s team qualifies for the USAV
 Junior Olympics, my child will be expected to compete in that tournament in June/July. I understand that all trip fees must
 be paid prior to these scheduled trips and are non-refundable.

Command Performance Juniors                                                                        2009-10

 Club Member Fundraising

 Command Performance Jrs will be hosting a number of tournaments provided that they don’t cancel because of
 the lack of entries. At our events, we accept a gate entry fee, run a concessions during most of our events, all
 while hosting and managing a number one-day tournaments.

 CP Jrs management typically would have gate management and concessions people scheduled to work on the
 day. This year we are offering a club dues credit program for those parents and players who need to do some
 fundraising. CPJs tournament program will contribute $25 for gate management, and $50 for concessions
 management against a junior player’s membership dues.

 Boys dues must be paid by January 1, 2010, and Girls dues must be paid by February 1, 2010.
