         Thank you for choosing me as your officiant! My mission is to make the world a happier place, and today that
begins with you. This form is a contract for my services at your event. Please fill out the information below. Make you’re

Contract Terms:
I. Marriage License (Legal Weddings only):
   a. The license, if required, must be obtained in person by the couple at the County Register of Deeds within 60 days
      of the ceremony.
   b. The marriage license must be presented to Rev. Su Windsong prior to the start of the ceremony. Failure to
      produce the license will result in the immediate cancellation of the ceremony, with full balance still due to Rev. Su
      Windsong.
   c. Two (2) legal witnesses must sign the license (two copies) BEFORE participating in group pictures.
II. Payment of Fees:
   a. An $80 retainer is due at the end of the consultation to reserve your date in Rev. Su Windsong’s calendar.
   b. All subsequent rehearsal fees (if applicable) and ceremony fee balances shall be due prior to or at the start of the
       ceremony, and are non-refundable, unless forfeited by Rev. Su Windsong.
   c. All “Accessories” require pre-payment within five (5) calendar days of the consultation (by     ), or they will not
       be provided. If your ceremony is less than ten (10) days away, payment for accessories is required at the
       consultation, and some accessories may not be available.
III. Methods of Payment: Rev. Su Windsong accepts cash, PayPal, personal check, or money order. PayPal full
   balance only to revsu@ladywindsong.com. Checks and money orders for partial or full amount due are made
   payable to “Rev. Su Leone”. Checks and PayPal will not be accepted within ten (10) days of your ceremony date (or
   after      )
IV. Late Start: If for ANY reason by no fault of Rev. Su Windsong the ceremony starts forty-five (45) minutes or more
  after it is scheduled with her (or after    ), the officiant may, at her option, either: 1) perform a very short civil
  ceremony in place of the agreed-upon ceremony, or 2) the ceremony may be rescheduled. An additional $80 fee will
  be assessed if your event is rescheduled by no fault of Rev. Su Windsong.
V. Contract Changes: This contract can be modified by mutual consent of Rev. Su Windsong and the Partners at any
    time in writing.
VI. Contract Termination: Termination of this contract can be done only by Rev. Su Windsong, or Partner 1 or
  Partner 2. Rev. Su Windsong reserves the right to cancel services to the Partners for any of the following:
    1. Unreasonable demands/actions from the wedding couple, from the wedding couple's parents or
           step-parents, or their guests
    2. Under duress/threat from any person
    3. False information provided to Rev. Su Windsong

VII. Refunds: The retainer paid at the consultation is non-refundable. If you cancel your event more than 60 days prior
  to your retained date, there is no additional fee. Cancellations by the Partners within sixty (60) days of the retained
  date (or after      ) will result in a $80 cancellation fee.
