What is considered seclusion?
Seclusion is the involuntary confinement of a
person in a room or area where the person
is physically prevented from leaving or
reasonably believes that he or she would be
physically prevented from leaving.
What is the Purpose of Seclusion?
Seclusion is an emergent and temporary measure
employed to maintain safety of patients, staff and
others in circumstances in which there is the
occurrence or imminent substantial risk of serious
self-destructive or assaultive behavior or other
behavior (such as that due to severely impaired
judgment) that would endanger self or others. It is
used only after less restrictive interventions have
been considered or are ineffective.
What patients do we place in
Patients who express intent to harm self
Patients who express intent to harm to others
Patients whose severely impaired judgment would
present a risk to themselves or others
Patients who you are preventing from leaving
Anyone who you would have placed with a safety sitter
for behavioral reasons
What do we need to initiate &
May be initiated by any Registered Nurse
A written physician order within 1 hour of application
and then q 4 hrs thereafter
Physician will write “seclusion” as order
