 LIST OF REQUIRED DOCUMENTS FOR SINP ENTREPRENEUR CATEGORY
• The following documents are required for all applicants.
• Please note that the SINP requires specific documents from individual countries. Please consult the country list
 in the appendix to learn about additional requirements for your country.
• All SINP forms require original signatures in ink. Please provide photocopies of all other forms and
 documents.
• If your documents are in a language other than English, you must submit a photocopy of the original document
 and a photocopy of the English translation.
• Translators can be any person other than a family member of the applicant or spouse, and must not work for or be
 a paid consultant or representative of the person who is preparing the application. You must also supply an
 affidavit from the translator describing their translation ability. Most Canadian immigration visa offices may
 require that the translator is certified by a regulatory body as a translator. It is your responsibility to make sure all
 federal requirements are met with respect to translations.
• If documents are missing, not translated, originals without photocopies, or unclear, your application will be
 returned to you without assessment.

• The SINP reserves the right to request additional documentation as required to better assess your
 application.

Copies of the reference letters from your present and
former employers on the official letterhead of the
enterprise, which must include the address and telephone
numbers of the enterprise and clearly indicate the number
of years worked, the position and the responsibilities
related to the position. The reference letters must also state
your salary, including bonus and commission received (if
applicable). In addition, these reference letters must be
signed by a person occupying a position of authority and
must include that person's name and title. If applicable use
Canadian and Saskatchewan colleagues as references. If
these documents cannot be submitted, please attach an
explanations and alternative documents.

Copies of your present and/or former employment
contracts. If these documents cannot be submitted,
please attach an explanation and alternative
documents.

Copies of the organizational chart for the enterprise(s) you
have worked with during the last ten years, indicating your
place on the chart and the number of employees in each
enterprise.

Contact information for the enterprise, including the mailing
address, telephone and fax numbers, and web site address.



MANAGEMENT EXPERIENCE IN A LAWFUL ENTERPRISE
FOR SOLE PROPRIETORS, PARTNERS OF UNICORPORATED                  Submitted   Not Submitted   Not Applicable
BUSINESSES, AND FOR SHAREHOLDERS AND DIRECTORS OF
LIMITED COMPANIES:

Copy of the business registration licence of each
enterprise. If this cannot be submitted, please attach an
explanation and alternative documents.

Copy of the registration with the taxation authorities for
each enterprise. If this cannot be submitted, please
attach an explanation and alternative documents.



Copy of the proof of shareholding of each enterprise you
worked with. If this cannot be submitted, please attach
an explanation and alternative documents.
Copy of the list of employees with the monthly salary and
number of hours of work paid by the employer per week
and proof of payment of salaries. If this cannot be
submitted, please attach an explanation and any alternative
documents.
Contact information for the enterprise, including the
mailing address, telephone and fax numbers, and web site
address.

Documentary proof to support the information provided on the Schedule 4A Form:


 • Copies of statements of bank accounts for a period of
   the most recent 12 months

 • Copies of title deeds and property evaluation reports
   prepared by a specialized firm for each real property.
   Provide copies of the licence authorizing the firm to
   perform professional appraisals.

 • For each real property, include copies of payment
   receipts to show proof of purchase price and taxes for
   real estate.

 • Copy of the purchase agreement for each real property
   and, if applicable, the current registration in the official
   register. In the case of commercial property, a copy of
   the purchase agreement is required.
 • Copies of bank letters indicating outstanding
   mortgages or other liabilities

 • Copies of transaction records of your stocks and shares
   for a period of the most recent 12 months


 • Copies of the most recent financial statements prepared
   by a chartered accountant of the enterprises in which
   you have a vested interest for a period of 3 years.
   Please include a balance sheet, statement of profit and
   loss, copy of the chartered accountant's licence and
   complete contact information. If these cannot be
   submitted, please attach an explanations and
   alternative documents.
• In the case of a sole proprietorship, include copies of
  company bank transactions for the 6 months prior to
  the date of the business net value appraisal. If these
  cannot be submitted, please attach an explanations and
  alternative documents.
• Copy of the proof of shareholding in which you have
  vested interest enterprises. If this cannot be
  submitted, please attach an explanation and
  alternative documents.

• Copy of an income certificate prepared by your current
  employer (if applicable)

• Copy of updated statements for pensions, insurance or
  other investments

• Copy of updated statements for pensions, insurance or
  other investments

• Copies of pay stubs or payrolls for a period of 12
  months from your current employer or your enterprise.
  If these cannot be submitted, please attach an
  explanation and alternative documents.


• In the case of a donation or gift, include identification
  of the donor and their financial capacity to make a
  donation


• In the case of donations or gifts, include copies of
  documents verifying the transfer of the amount
  received

  Donations received less than 6 months before
  submitting the application will not be eligible towards
  your net worth.

• In the case of inheritance, include an official Act of
  Death Certificate, notarized will, or court decision
  documents verifying the transfer of funds to the
  inheritor
• Other documents you believe will support your
  application for nomination to the SINP
