   Prior attempts to correct conduct, work rule

   Directives to adhere to reasonable & practical rules
    or expectations of correct behavior or improved
    performance
   Language which indicates that failure to adhere to
    directives, reasonable and practical rules, or to
    improve performance may result in disciplinary
    action, up to and including termination
   Signature line for acknowledgement of receipt of
    the document by the employee
   Document should be copied to AFSCME, HRS
    Personnel file, Office of Employee Relations, and
    Department Chair or Head
