        COBRA Notification Process

     The University of Rochester will notify Excellus BCBS of
      those who experience a qualifying event.
     Excellus provides notification to EBS (COBRA Administrator
      for the University of Rochester).
     EBS generates a COBRA notification with Election Form and
      HIPAA Certificate of Coverage within 3 business days after
      notification.

    COBRA Election Process

     If the dependent elects to continue benefits on COBRA,
      the Election Agreement should be completed and
      returned to EBS at the address on the form.
     EBS will notify Excellus BCBS of the election to continue
      COBRA benefits. Excellus will move the dependent child
      into a single COBRA policy equivalent to the coverage
      they were on while under the parent.
     COBRA coverage starts the first day the dependent
      becomes eligible and is retro back to that date.
     Excellus will reinstate coverage and bill the dependent
      directly.

        COBRA Billing
     Initial invoice is a “retro” bill issued when the insurance
      coverage is processed. May include the next month’s
      payment due.
     Invoices are then issued mid-month for the following
      month’s coverage.
     Premiums are due by the 1st of each month.
     An invoice for the next month cannot be generated until
      current month is paid.
     Excellus will issue any cancel notices due to non-payment of
      premiums and will also notify individuals at the time COBRA
      coverage is ending.

    COBRA Costs

     Each qualified beneficiary is required to pay the entire
      cost of continuation coverage.
     The amount a qualified beneficiary is required to pay
      cannot exceed 102 percent of the cost for the group
      health plan for coverage.
     In the case of an extension of continuation coverage
      (11 months) due to a disability, cost of premium may
      be increased to 150 percent for the extra months.
     Qualified Beneficiaries are eligible to elect a different
      plan during annual Open Enrollment.
